SYNAPSE Blend Dosage: How Pepti Prescribes Semax, Selank, MOTS-c and Tesamorelin in One Vial

This page explains the SYNAPSE dosage Pepti prescribes, what the units on the syringe mean, how much of each of the four peptides is in one dose, a units to mL to mg chart for the SYNAPSE vial, how a SYNAPSE dose compares with Semax, Selank, MOTS-c and tesamorelin on their own and with the ELEVATE blend, when it is taken, and what can change your directions. It covers how Pepti prescribes SYNAPSE, not online protocols. Nothing here is a reason to change your own dose.

Tesamorelin (Injectable)
Tesamorelin$249/mo

At Pepti, the SYNAPSE blend is prescribed as 0.25 mL, which is 25 units on a U-100 insulin syringe, injected under the skin once daily Monday through Friday. The 5 mL vial holds 5 mg Semax, 5 mg Selank, 10 mg MOTS-c and 10 mg tesamorelin, so every 0.25 mL dose delivers 0.25 mg Semax, 0.25 mg Selank, 0.5 mg MOTS-c and 0.5 mg tesamorelin, and the vial holds 20 doses, about four weeks. Your physician sets your directions after reviewing your intake, and your exact dose and units are printed on the vial.

SYNAPSE Blend Dosage

The short answer

Product Vial Directions Per dose Doses per vial Price
SYNAPSE Blend 5 mL: Semax 1 mg/mL, Selank 1 mg/mL, MOTS-c 2 mg/mL, tesamorelin 2 mg/mL Inject 0.25 mL (25 units) subcutaneously once daily Monday through Friday 0.25 mg Semax, 0.25 mg Selank, 0.5 mg MOTS-c, 0.5 mg tesamorelin 20 $279/mo

The price is all-in, covering medication, physician review, refill management and shipping, with no separate membership fee. Pepti is cash-pay and does not bill insurance. Your own directions are the ones printed on your vial.

SYNAPSE Blend Dosage

SYNAPSE dosage in units, mL and mg

Pepti's directions are written in millilitres and units, because prescriptions are written in millilitres and insulin syringes are marked in units. On a U-100 insulin syringe, 1 unit is 0.01 mL, so 100 units is 1 mL and 25 units is 0.25 mL. A unit is a volume mark on the barrel, not a measure of how strong the medication is.

SYNAPSE is four peptides dissolved together in one vial, so one draw carries all four. The milligrams per dose come from each component's concentration multiplied by 0.25 mL:

Component Concentration Per vial Per 0.25 mL dose
Semax 1 mg/mL 5 mg 0.25 mg
Selank 1 mg/mL 5 mg 0.25 mg
MOTS-c 2 mg/mL 10 mg 0.5 mg
Tesamorelin 2 mg/mL 10 mg 0.5 mg
Total 30 mg 1.5 mg

That arithmetic is a way to read a label, not a way to set a dose. A vial labelled with 30 mg of peptide in total is not a 30 mg dose: it is twenty doses of 1.5 mg combined. If your label and your syringe seem to disagree, do not dose; message your care team. How long does a peptide vial last explains every number on a vial label.

Fixed proportions

The four peptides are in a ratio fixed by the pharmacy's formulation. Every dose delivers the same split, and there is no way to take more of one and less of another from the same vial. MOTS-c and tesamorelin are 20 of the 30 mg, two-thirds of the vial by mass, so although SYNAPSE sits in Pepti's cognitive category, by weight it is mostly a metabolic preparation with two smaller neuropeptide components. If a different balance is wanted, the components are prescribed separately.

SYNAPSE Blend Dosage

SYNAPSE dosage chart: units to mL to mg

The chart below is the conversion for the SYNAPSE vial, drawn with a U-100 insulin syringe. It is math only. It is not a menu of doses, and the only line that applies to you is the one that matches the number printed on your vial.

Syringe mark (U-100) Volume Semax Selank MOTS-c Tesamorelin
5 units 0.05 mL 0.05 mg 0.05 mg 0.1 mg 0.1 mg
10 units 0.1 mL 0.1 mg 0.1 mg 0.2 mg 0.2 mg
15 units 0.15 mL 0.15 mg 0.15 mg 0.3 mg 0.3 mg
20 units 0.2 mL 0.2 mg 0.2 mg 0.4 mg 0.4 mg
25 units (Pepti's typical directions) 0.25 mL 0.25 mg 0.25 mg 0.5 mg 0.5 mg
50 units 0.5 mL 0.5 mg 0.5 mg 1 mg 1 mg
100 units 1 mL 1 mg 1 mg 2 mg 2 mg

The rule behind every row: units divided by 100 gives millilitres; multiply millilitres by 1 for Semax and Selank and by 2 for MOTS-c and tesamorelin. Those multipliers only hold for this vial. A chart written for a different blend or a different concentration gives different milligram figures for the same units, which is the main reason not to dose from a chart, including this one.

SYNAPSE Blend Dosage

SYNAPSE compared with the products next to it

People often compare a SYNAPSE dose with the single-peptide vials, or with the ELEVATE blend, which shares the same Semax and Selank. The draw is 25 units in each, but what that draw delivers differs:

SYNAPSE Semax Selank MOTS-c Tesamorelin ELEVATE
What is in the vial 5 mL: 5 mg Semax, 5 mg Selank, 10 mg MOTS-c, 10 mg tesamorelin 5 mL: 10 mg Semax 5 mL: 10 mg Selank 5 mL: 10 mg MOTS-c 5 mL: 10 mg tesamorelin 5 mL: 5 mg Semax, 5 mg Selank, 10 mg BPC-157, 10 mg KPV

Price $279/mo $229/mo $229/mo $259/mo $249/mo $249/mo
  • What the numbers show

Three things stand out. First, a SYNAPSE dose carries the same 0.5 mg of MOTS-c as the MOTS-c vial and the same 0.5 mg of tesamorelin as the tesamorelin vial, so the metabolic side of the blend is not diluted. Second, it carries half the Semax and half the Selank of the single-peptide vials, because those vials are 2 mg/mL and SYNAPSE carries each at 1 mg/mL. Third, SYNAPSE and ELEVATE deliver identical Semax and Selank per dose; they differ only in their partners, MOTS-c and tesamorelin in SYNAPSE, BPC-157 and KPV in ELEVATE.

  • Why there is no reason to stack SYNAPSE with its components

Adding a tesamorelin, MOTS-c, Semax or Selank vial on top of SYNAPSE would put more of that component into each day than either product's directions describe. The same applies to other blends that share components: FUSION and PHYSIQ both contain tesamorelin and MOTS-c. Whether any combination suits you is set by your physician after reviewing your intake, which asks for every product you take for this reason.

  • Compared with the trial doses

The tesamorelin in each SYNAPSE dose is 0.5 mg on weekdays. The approved tesamorelin product's trials used 2 mg daily for visceral fat, and the 2012 cognition trial in older adults used 1 mg nightly for 20 weeks. The Semax and Selank human studies used intranasal products, so there is no injected trial dose to compare with. MOTS-c has no human trials at all. A SYNAPSE dose is not a scaled-down version of any studied dose.

SYNAPSE Blend Dosage

When SYNAPSE is taken

  • Once daily, Monday through Friday

Pepti's SYNAPSE directions say once daily, Monday through Friday, with weekends off. Weekends off are part of the schedule, not missed doses. Twenty doses at five a week is four working weeks, which is why refills run on a 28-day cycle. The weekday-only pattern is a prescribing convention rather than a schedule taken from any trial.

  • Time of day

The printed SYNAPSE directions say once daily without naming a time, and the components pull in different directions. The single tesamorelin vial is directed at bedtime, because tesamorelin is described as amplifying the growth hormone pulse of early sleep and the 2012 cognition trial dosed at bedtime. The single MOTS-c vial is directed in the morning, and Semax is linked to restlessness if taken late in the day. The live SYNAPSE reference notes that some prescribers direct it in the evening and others in the morning. If your label names a time, follow the label.

  • Under the skin, and where

Subcutaneous means into the fat layer, not the muscle. Abdomen and thigh are the usual sites, rotated so the same spot is not used repeatedly. The route is systemic, so there is no "near the problem" site. Technique is covered in how to inject peptides safely.

Directions say What it means in practice
0.25 mL (25 units) Draw to the 25 mark on a U-100 insulin syringe
Subcutaneously Into the fat layer, abdomen or thigh, rotating sites
Once daily One injection per dosing day
Monday through Friday Five doses a week, weekends off
20 doses per vial About four weeks, then the next refill

SYNAPSE Blend Dosage

How long a SYNAPSE vial lasts, storage and refills

Step Value
Dose 25 units, 0.25 mL
Doses per week 5 (Monday through Friday)
Doses per vial 20 (5 mL divided by 0.25 mL)
Used over the vial 5 mg Semax, 5 mg Selank, 10 mg MOTS-c, 10 mg tesamorelin, the whole vial
Time per vial 20 doses divided by 5 per week = 4 weeks

Vials are multi-dose solutions shipped ready to use, refrigerated with ice packs, with sterile syringes and alcohol prep pads. Nothing is mixed at home. Keep it in the fridge, do not freeze or shake it, and follow the beyond-use date on the label, which runs from first puncture; a multi-peptide vial is not something to keep past it. Cloudiness, particles or a change from what you received is a reason to message rather than inject. One vial per fill, every 28 days. How to store peptides covers storage.

If a vial runs short before the refill, message your care team rather than adjusting anything yourself. If you miss a weekday dose, follow your directions and do not take two doses to make up for one.

SYNAPSE Blend Dosage

Why the research cannot give you a SYNAPSE dose

SYNAPSE has never been studied as a blend, in people or in animals, so there is no trial dose for it. The evidence is per component, and none of it is a dose study of this injection. Tesamorelin has randomized trials of the approved product at 1 to 2 mg daily. Semax's Russian clinical series and Selank's small Russian comparator trials used intranasal products. MOTS-c has mouse and cell work only. The directions on the vial are a prescribing standard set to deliver one vial in four weeks. The component evidence is laid out in SYNAPSE blend benefits and what is the SYNAPSE blend.

SYNAPSE Blend Dosage

What changes the directions

Your physician sets your directions after reviewing your intake. Things that can lead to different directions, or a different product, include:

What can change How it shows up
Whether SYNAPSE is the right formula Focus and stress alone may point to Semax, Selank or Semax + Selank; body composition alone to tesamorelin or FUSION; a gut complaint with focus to ELEVATE
Time of day Some directions name a morning or evening time
Diabetes or borderline glucose Tesamorelin can raise blood sugar; MOTS-c is described in mice improving insulin sensitivity
Existing psychiatric treatment or a stimulant Reviewed at intake; none has been studied alongside Semax or Selank
A cancer history or a pituitary problem The approved tesamorelin product is contraindicated in active malignancy and in disruption of the pituitary axis
How you respond Reactions you report go to your physician, who may revise your directions

When directions change, the new numbers are printed on the new label. Never carry a number over from a previous fill. Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.

SYNAPSE Blend Dosage

Side effects, in summary

What is reported What it tracks What to do
Injection-site redness, mild soreness, occasional bruising Any injection; tesamorelin's label records it most Rotate sites
Joint aches, swelling in the hands or feet, tingling Tesamorelin (fluid-related growth hormone effects) Message your care team
Thirst or frequent urination A possible rise in blood sugar (tesamorelin) Message your care team
Brief jitteriness, a faster pulse or light-headedness after a dose MOTS-c Usually settles; report it if not
Restlessness if dosed late in the day Semax Tell your care team; do not change timing yourself
Early nausea or light-headedness Reported for the blend Report it if it persists
Facial or throat swelling, widespread rash, breathing difficulty Possible with any injected medication Call 911 first, then tell your physician

The full list, with who should not take SYNAPSE, is in SYNAPSE blend side effects. Tested athletes should know that the 2026 WADA Prohibited List names tesamorelin under S2.2.4 and MOTS-c under S4.4.1, both prohibited at all times, which covers SYNAPSE.

SYNAPSE is a compounded prescription; compounded medications are not FDA approved. In July 2026 the FDA's Pharmacy Compounding Advisory Committee voted to recommend adding Semax and MOTS-c to the 503A Bulks List, with the FDA's final determinations pending. Selank's nomination was withdrawn and it has never been reviewed. Tesamorelin is the active ingredient of an FDA-approved product, but compounded tesamorelin is not that product.

Where to get SYNAPSE prescribed

SYNAPSE cannot be bought legitimately without a prescription. Sites selling multi-peptide vials "for research use only" are not accountable for what is in them: identity, purity, sterility and concentration are untested, so the per-dose figures above would not hold. The prescription route:

  1. Complete a free online intake covering your history, medications and what you want help with.
  2. A physician licensed in your state reviews it, usually within 24 hours.
  3. If it is appropriate, a PCAB-accredited, FDA-registered US 503A compounding pharmacy prepares it to that prescription. Lot test results are published at /quality/lab-results.
  4. It ships refrigerated with your directions printed on the vial.
  5. Your physician stays reachable for questions and dose changes.

The components on their own: Semax dosage, Selank dosage, MOTS-c dosage and tesamorelin dosage.
