By using the [pepti] Services, you acknowledge that you have read, understood, and agreed to this Telehealth Informed Consent. You also confirm that you have had the opportunity to ask questions and that all of your questions have been answered to your satisfaction.
What Is Telehealth?
Telehealth is the delivery of healthcare services using electronic communications, including secure messaging, asynchronous (store-and-forward) intake review, video conferencing, and electronic transmission of health information. Through [pepti], you may receive medical evaluation and prescription treatment from a licensed healthcare provider without an in-person visit.
How It Works
- You complete a digital health assessment on our platform.
- A licensed healthcare provider in your state reviews your responses.
- The provider may message you for clarification, request additional information, or order labs if necessary.
- If clinically appropriate, the provider issues a prescription that is sent to a partner pharmacy.
- The pharmacy fills and ships your medication.

Risks and Limitations
You understand and accept the following:
- No physical examination. Without an in-person exam, your provider may not be able to detect certain medical conditions.
- Information sufficiency. In rare cases, information you transmit may not be sufficient to allow for appropriate medical decision-making by the provider. The provider may request additional information or refer you to in-person care.
- Equipment failures. Delays in evaluation or treatment may occur due to deficiencies or failures of equipment, network outages, or software errors.
- Security risks. In very rare instances, security protocols could fail, causing a breach of privacy of personal medical information. We use encryption and other safeguards, but no electronic transmission is 100% secure.
- Incomplete records. Lack of access to all of your prior medical records may result in adverse drug interactions, allergic reactions, or other clinical errors that would not occur with complete records.
- Information may be incomplete. A diagnosis or treatment plan based on telehealth may differ from one made after an in-person visit.
- Treatment may not be appropriate. Your provider may determine that telehealth is not appropriate for your condition and refer you to in-person care.
- Adverse reactions. All medications carry risk of side effects or adverse reactions. Discuss concerns with your provider.
Your Right to Withdraw Consent
You have the right to withdraw your consent to telehealth services at any time, without affecting your right to future care or treatment. Withdrawing consent will end your use of the Services but will not affect any care you have already received. Compounded Medications
You understand that some medications offered through the Services are compounded medications prepared by FDA-registered 503A or 503B compounding pharmacies. Compounded medications are not FDA-approved as products. Compounded medications are prepared based on a prescription from a licensed healthcare provider for an individual patient. Compounded medications may not undergo the same testing, manufacturing controls, or quality assurance as commercially manufactured FDA-approved drugs. You accept these characteristics and agree to discuss any concerns with your provider.
Peptide Therapy Acknowledgment
You understand that many peptides offered through the Services are not FDA-approved for the indications discussed and that clinical research on some peptides is limited or ongoing. You acknowledge that:
- Treatment outcomes vary and results are not guaranteed.
- Some peptide therapies are considered experimental or off-label.
- You are choosing to pursue this treatment after consultation with a licensed provider who has determined it is appropriate for you.
Your Rights
- You may withhold or withdraw consent at any time without affecting your right to future care.
- You may request that your telehealth interaction not be recorded.
- You may ask your provider questions at any time.
- You may request an in-person referral if you prefer.
- You have the right to access your medical records.
Emergencies
The Services are not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately.
