Tesamorelin Dosage: How Pepti Prescribes It, in Units, mL and mg

This page explains the tesamorelin dosage Pepti prescribes, what the units on the syringe mean, a dosage chart for tesamorelin and each blend that contains it, when it is taken, what can change your directions, and the side effects to know about. It covers how Pepti prescribes tesamorelin, not online protocols. Nothing here is a reason to change your own dose.

Tesamorelin (Injectable)
Tesamorelin$249/mo

At Pepti, tesamorelin is prescribed as 0.25 mL, which is 25 units on a U-100 insulin syringe, injected under the skin Monday through Friday nights at bedtime. The vial is 5 mL at 2 mg/mL, so each 0.25 mL dose carries 0.5 mg of tesamorelin and the vial holds 20 doses, about four weeks. Your physician sets your directions after reviewing your intake, and your exact dose and units are printed on the vial. Every Pepti blend that contains tesamorelin uses the same 0.25 mL draw and the same 20 doses per vial.

Tesamorelin Dosage

The short answer

Product Vial Directions Doses per vial Price
Tesamorelin 5 mL, 2 mg/mL (10 mg per vial) Inject 0.25 mL (25 units) subcutaneously Monday through Friday nights at bedtime 20 $249/mo
Tesamorelin + Ipamorelin 5 mL, tesamorelin 2 mg/mL + ipamorelin 1 mg/mL Inject 0.25 mL (25 units) subcutaneously Monday through Friday nights at bedtime 20 $259/mo

Every price is all-in, covering medication, physician review, refill management and shipping, with no separate membership fee. Pepti is cash-pay and does not bill insurance. Your own directions are the ones printed on your vial.

Tesamorelin Dosage

Tesamorelin dosage in units, mL and mg

Pepti's directions are written in two forms, millilitres and units, because prescriptions are written in millilitres and insulin syringes are marked in units. On a U-100 insulin syringe, 1 unit is 0.01 mL, so 100 units is 1 mL and 25 units is 0.25 mL. A unit is a volume mark on the barrel, not a measure of how strong the medication is.

The milligram figure comes from the concentration on the label. Tesamorelin is supplied at 2 mg/mL, so:

What you read Value Where it comes from
Dose volume 0.25 mL Printed on the vial
On a U-100 insulin syringe 25 units 0.25 mL x 100 units per mL
Concentration 2 mg/mL Printed on the vial
Tesamorelin per dose 0.5 mg 2 mg/mL x 0.25 mL
Total in the vial 10 mg in 5 mL Printed on the vial
Doses per vial 20 5 mL divided by 0.25 mL

That arithmetic is a way to read a label, not a way to set a dose. A vial labelled 10 mg is not a 10 mg dose: it is twenty 0.5 mg doses. If your label and your syringe seem to disagree, do not dose; message your care team. How long does a peptide vial last explains every number on a vial label in more detail.

Tesamorelin dosage chart: every Pepti product that contains it

Tesamorelin appears in several Pepti blends. Each one is a 5 mL vial drawn at 0.25 mL, 20 doses per vial. The timing differs: some are directed at bedtime, some once daily on weekdays without a stated time. The tesamorelin per dose is computed from each vial's concentration.

Product Tesamorelin per vial Directions Tesamorelin per dose Doses per vial Price
Tesamorelin 10 mg (2 mg/mL) 0.25 mL (25 units) Monday through Friday nights at bedtime 0.5 mg 20 $249/mo
Tesamorelin + Ipamorelin 10 mg (2 mg/mL), with 5 mg ipamorelin 0.25 mL (25 units) Monday through Friday nights at bedtime 0.5 mg 20 $259/mo
PHYSIQ Blend 10 mg (2 mg/mL), with ipamorelin, AOD-9604 and MOTS-c 0.25 mL (25 units) Monday through Friday nights at bedtime 0.5 mg 20 $289/mo
TITAN Blend 10 mg (2 mg/mL), with CJC-1295, ipamorelin and BPC-157 0.25 mL (25 units) Monday through Friday nights at bedtime 0.5 mg 20 $289/mo
ASCEND Blend 10 mg (2 mg/mL), with CJC-1295, ipamorelin and IGF-1 LR3 0.25 mL (25 units) Monday through Friday nights at bedtime 0.5 mg 20 $289/mo
RADIANCE Blend 10 mg (2 mg/mL), with GHK-Cu 0.25 mL (25 units) once daily Monday through Friday 0.5 mg 20 $249/mo
SYNAPSE Blend 10 mg (2 mg/mL), with semax, selank and MOTS-c 0.25 mL (25 units) once daily Monday through Friday 0.5 mg 20 $279/mo
FUSION Blend 15 mg (3 mg/mL), with BPC-157, MOTS-c and 5-Amino-1MQ 0.25 mL (25 units) once daily Monday through Friday 0.75 mg 20 $279/mo

Two things stand out. First, the volume never changes: every product in the chart is 25 units. Second, FUSION is the one blend where the same 25 units carries more tesamorelin, because its vial is more concentrated. That is why you read your own label rather than a chart, including this one.

In a blend, every ingredient is in fixed proportion. One 0.25 mL draw delivers all of them in the ratio the pharmacy compounded. There is no way to take more tesamorelin and less of something else from the same vial; if a different balance is wanted, that is a different prescription. More on the blends is in what is tesamorelin + ipamorelin and physiq vs shred vs titan.

When tesamorelin is taken

  • At bedtime

Pepti's tesamorelin directions say nights at bedtime. The body's largest natural growth hormone pulse occurs during slow-wave sleep, and tesamorelin prompts the pituitary to release your own growth hormone. Dosing shortly before sleep places that signal on top of a pulse the body is already producing. That is the reason the label says bedtime.

  • Monday through Friday

The weekday schedule, with weekends off, is a prescriber convention rather than something derived from trials. Prescribers describe the two nights off as keeping the pituitary responsive. It also makes the arithmetic clean: 20 doses at five a week is exactly four weeks.

  • Under the skin

Subcutaneous means into the fat layer, not the muscle. The abdomen is the usual site, rotated so the same spot is not used repeatedly. Technique is covered in how to inject peptides safely.

Directions say What it means in practice
0.25 mL (25 units) Draw to the 25 mark on a U-100 insulin syringe
Subcutaneously Into the fat layer, usually the abdomen, rotating sites
Monday through Friday Five doses a week, weekends off
Nights at bedtime Shortly before sleep
20 doses per vial About four weeks, then the next refill

How a vial maps onto refills

Each vial is sized to last about four weeks at the printed directions, which is why refills run every 28 days, one vial per fill. Vials are multi-dose solutions shipped ready to use, refrigerated with ice packs, with sterile syringes and alcohol prep pads. Nothing is mixed at home. Keep the vial in the fridge, and follow the beyond-use date printed on the label, which runs from first puncture. How to store peptides covers storage.

If a vial runs short before the refill, message your care team rather than adjusting anything yourself. If you miss a weekday dose, follow your directions and do not take two doses to make up for one.

What changes the directions

Your physician sets your directions after reviewing your intake. Things that can lead to different directions, or a different product, include:

What can change How it shows up
Which product is prescribed Tesamorelin alone, tesamorelin + ipamorelin, or a blend; each has its own label
Your medical history Diabetes or a borderline glucose, a pituitary condition, or a cancer history change the conversation; see the cautions below
Your other medications The intake asks for everything you take, because growth hormone can alter how some drugs are metabolised
How you respond Side effects you report, such as puffiness or hand tingling, go to your physician, who may revise your directions

When directions change, the new numbers are printed on the new label. Never carry a number over from a previous fill. If your physician wants labs for your care, they'll ask through your account; bloodwork is not required for any Pepti treatment.

Side effects, in summary

The full picture, with the figures from the approved product's label, is in tesamorelin benefits. Those label figures were recorded at a higher daily dose than Pepti's compounded directions.

What is reported What it usually means What to do
Injection-site redness, itching or a small lump The commonest complaint Rotate sites
Puffiness, joint aches, hand tingling Classic growth hormone effects Message your care team; do not change the dose yourself
Raised blood sugar Growth hormone reduces insulin sensitivity Raise diabetes or borderline glucose at intake
Itching, flushing, hives or rash Hypersensitivity Report it; emergency care for throat swelling or breathing difficulty

Who should not take tesamorelin, or should discuss it first: anyone with a pituitary disorder, pituitary surgery, head irradiation or head trauma; anyone with an active cancer; anyone pregnant or breastfeeding; anyone with a known hypersensitivity to tesamorelin. Tested athletes should know the 2026 WADA Prohibited List names tesamorelin under S2.2.4, prohibited at all times.

An approved tesamorelin product exists for one specific indication. Compounded tesamorelin prescribed for other uses is not FDA approved, and compounded medications are not FDA approved.

Where to get tesamorelin prescribed

Tesamorelin cannot be bought legitimately without a prescription. Sites selling it "for research use only" are not accountable for what is in the vial: identity, purity, sterility and concentration are untested. The prescription route:

  1. Complete a free online intake covering your history, medications and what you want help with.
  2. A physician licensed in your state reviews it, usually within 24 hours.
  3. If it is appropriate, a PCAB-accredited, FDA-registered US 503A compounding pharmacy prepares it to that prescription. Lot test results are published at /quality/lab-results.
  4. It ships refrigerated with your directions printed on the vial.
  5. Your physician stays reachable for questions and dose changes.
