What the pen is,
in plain terms.
Two parts: a pen you keep, a cartridge you replace
The injector itself is reusable. It holds a 3 mL cartridge of your medication, compounded and filled by the pharmacy, and the cartridge is what gets replaced — by hand, no tools. The pen stays with you; the medication cycles through it. That is the whole design, and it is why the injector is free with your first order rather than something you buy again every month.
The dose dial, and what a “unit” means here
The dial runs from 0 to 60 units in single-unit clicks, and one unit is 0.01 mL — the same graduation an insulin syringe uses. Your pharmacy label states your dose in the same unit, so the number on the label is the number of clicks — there is no conversion to do and no syringe barrel to read against the light. That is the practical difference from a vial: with a vial you are measuring; with the pen you are counting.
It fires at under 8 newtons of press force at therapeutic doses. Most people describe feeling the click more than the needle.
What “510(k) cleared” means, and what it does not
The injector is a 510(k) cleared medical device. Clearance means the manufacturer demonstrated to the FDA that the device is substantially equivalent to a device already legally on the market, and the FDA allowed it to be sold. It is a different pathway from approval: an approved drug has been reviewed for safety and effectiveness in clinical trials, while a cleared device has been reviewed against an existing comparable device.
Two things follow, and both matter. The clearance covers the injector — the hardware that holds the cartridge and delivers the dose. It says nothing about the medication inside it. Compounded medications are not FDA approved, whichever format they arrive in, and no auto-injector changes that. Anyone implying otherwise is misreading the clearance.
The pen is produced in a cGMP, ISO 13485:2016 facility and takes ISO 11608 compatible needles and cartridges — the same standards the pen injectors used for insulin and GLP-1 medications are built to.
Needles, and what to do with them after
A fresh single-use tip goes on before each injection and comes off after. Reusing a tip dulls it, which is the usual reason an injection that was painless last week stings this week. Used tips go in a sharps container, not household rubbish or recycling, and the FDA publishes guidance on disposal where you live — rules are set locally, not nationally.
Storing it, and travelling with it
The cartridge follows the storage instructions printed on your pharmacy label — that label, not a website, is what governs your medication. The pen body itself is not refrigerated. If you fly, medication and the supplies that go with it are exempt from the liquid limit, but you have to declare them at the checkpoint, and cold packs may be presented frozen, partly frozen or melted.
If the pen is dropped, or a dose is interrupted part-way, do not re-dial and guess. Message your care team and say exactly what happened; guessing a partial dose is how a week of dosing goes wrong.
What the pen does not change
It does not change the medication, the strength, or what a physician will prescribe. The same peptide at the same strength is the same medication in a vial or a cartridge. It does not remove the need for a prescription, a physician review, or the lab work your prescriber asks for. It changes one thing: how the dose gets from the container into you.
