Semax Dosage: How Pepti Prescribes the Injection, in Units, mL and mg
This page explains the Semax dosage Pepti prescribes, what the units on the syringe mean, a dosage chart that converts units to millilitres and milligrams, when Semax is taken, how long a vial lasts, why the published Russian doses do not translate to an injection, what can change your directions, and the side effects to know about. It covers how Pepti's injectable Semax is typically prescribed, not dosing protocols from the internet. Nothing here is a reason to change your own dose.

At Pepti, Semax is prescribed as 0.25 mL, which is 25 units on a U-100 insulin syringe, injected under the skin once daily Monday through Friday. The vial is 5 mL at 2 mg/mL, so each dose carries 0.5 mg of Semax and the vial holds 20 doses, about four weeks. Your physician sets your directions after reviewing your intake, and your exact dose and units are printed on the vial. The ELEVATE and SYNAPSE blends use the same 25 unit draw, but carry Semax at half the concentration, so the same draw holds 0.25 mg.

Semax Dosage
The short answer
| Product | Vial | Typical directions | Semax per dose | Doses per vial | Price |
|---|---|---|---|---|---|
| Semax | 5 mL, 2 mg/mL (10 mg per vial) | Inject 0.25 mL (25 units) subcutaneously once daily Monday through Friday | 0.5 mg | 20 | $229/mo |
| ELEVATE Blend | 5 mL, Semax 1 mg/mL with Selank, BPC-157 and KPV | Inject 0.25 mL (25 units) subcutaneously once daily Monday through Friday | 0.25 mg | 20 | $249/mo |
| SYNAPSE Blend | 5 mL, Semax 1 mg/mL with Selank, MOTS-c and tesamorelin | Inject 0.25 mL (25 units) subcutaneously once daily Monday through Friday | 0.25 mg | 20 | $279/mo |
Every price is all-in, covering medication, physician review, refill management and shipping, with no separate membership fee. Pepti is cash-pay and does not bill insurance. These are the typical printed directions; your own directions are the ones on your vial.
Semax Dosage
Semax dosage in units, mL and mg
Pepti's directions are written in two forms, millilitres and units, because prescriptions are written in millilitres and insulin syringes are marked in units. On a U-100 insulin syringe, 1 unit is 0.01 mL, so 100 units is 1 mL and 25 units is 0.25 mL. A unit is a volume mark on the barrel, not a measure of how strong the medication is. Units only mean something alongside the concentration printed on the label.
The milligram figure comes from that concentration. Pepti's single Semax vial is supplied at 2 mg/mL, so:
| What you read | Value | Where it comes from |
|---|---|---|
| Dose volume | 0.25 mL | Printed on the vial |
| On a U-100 insulin syringe | 25 units | 0.25 mL x 100 units per mL |
| Concentration | 2 mg/mL | Printed on the vial |
| Semax per dose | 0.5 mg | 2 mg/mL x 0.25 mL |
| Total in the vial | 10 mg in 5 mL | Printed on the vial |
| Doses per vial | 20 | 5 mL divided by 0.25 mL |
That arithmetic is a way to read a label, not a way to set a dose. A vial labelled 10 mg is not a 10 mg dose: it is twenty 0.5 mg doses. If your label and your syringe seem to disagree, do not dose; message your care team. How long does a peptide vial last explains every number on a vial label in more detail.
Semax Dosage
Semax dosage chart: units to mL to mg
People search for a "Semax dosage chart" because online sources quote doses in micrograms, milligrams, drops and units interchangeably, and the numbers only line up if you know the concentration and the route. The chart below is the conversion for a 2 mg/mL vial, drawn with a U-100 insulin syringe, with a column for the 1 mg/mL Semax in the blends. It is math only. It is not a menu of doses, and the only line that applies to you is the one that matches the number printed on your vial.
| Syringe mark (U-100) | Volume | Semax from a 2 mg/mL vial | Semax from a 1 mg/mL blend |
|---|---|---|---|
| 5 units | 0.05 mL | 0.1 mg | 0.05 mg |
| 10 units | 0.1 mL | 0.2 mg | 0.1 mg |
| 15 units | 0.15 mL | 0.3 mg | 0.15 mg |
| 20 units | 0.2 mL | 0.4 mg | 0.2 mg |
| 25 units (Pepti's typical directions) | 0.25 mL | 0.5 mg | 0.25 mg |
| 30 units | 0.3 mL | 0.6 mg | 0.3 mg |
| 40 units | 0.4 mL | 0.8 mg | 0.4 mg |
| 50 units | 0.5 mL | 1 mg | 0.5 mg |
| 100 units | 1 mL | 2 mg | 1 mg |
The rule behind every row: units divided by 100 gives millilitres, and millilitres multiplied by the concentration gives milligrams. A chart from another source, written for a different concentration, will give different milligram figures for the same units. That is the main reason not to dose from a chart, including this one.
Semax in the blends
In a blend, every ingredient is in fixed proportion. One 0.25 mL draw delivers all of them in the ratio the pharmacy compounded, so there is no way to take more Semax and less of something else from the same vial.
| Blend | What one 25 unit draw delivers | Directions |
|---|---|---|
| ELEVATE | 0.25 mg Semax, 0.25 mg Selank, 0.5 mg BPC-157, 0.5 mg KPV | Once daily Monday through Friday |
| SYNAPSE | 0.25 mg Semax, 0.25 mg Selank, 0.5 mg MOTS-c, 0.5 mg tesamorelin | Once daily Monday through Friday |
Those figures are each vial's concentration multiplied by 0.25 mL. The combined Semax + Selank vial is a separate product, listed as containing 1 mg Semax and 1 mg Selank; its directions are printed on the vial. More on the pairings is in what is Semax + Selank, what is the ELEVATE blend and what is the SYNAPSE blend.
Semax Dosage
How long a Semax vial lasts
At Pepti's typical directions, one 10 mg vial lasts four weeks:
| Step | Value |
|---|---|
| Dose | 25 units, 0.25 mL, 0.5 mg |
| Doses per week | 5 (Monday through Friday) |
| Doses per vial | 20 (5 mL divided by 0.25 mL) |
| Semax used | 20 x 0.5 mg = 10 mg, the whole vial |
| Time per vial | 20 doses divided by 5 per week = 4 weeks |
Four weeks is why refills run every 28 days, one vial per fill. Vials are multi-dose solutions shipped ready to use, refrigerated with ice packs, with sterile syringes and alcohol prep pads. Nothing is mixed at home. Keep the vial in the fridge between doses, and follow the beyond-use date printed on the label, which runs from first puncture. How to store peptides covers storage.
If a vial runs short before the refill, message your care team rather than adjusting anything yourself. If you miss a dose, follow your directions and do not take two doses to make up for one.

When to take Semax
Once daily, usually in the morning
Pepti's Semax directions say once daily, Monday through Friday, without a fixed hour on the label. Morning dosing is the usual direction in practice, because some people describe transient restlessness if Semax is taken late in the day. If your label names a time, follow the label.
Monday through Friday
The weekday schedule, with weekends off, is a prescriber convention rather than a trial design. It keeps total exposure modest and builds in a regular break, given that long-term data does not exist. It also makes the vial arithmetic clean: 20 doses at five a week is exactly four weeks. Weekends off are part of the schedule, not missed doses.
Under the skin
Subcutaneous means into the fat layer, not the muscle. Abdomen and thigh are the usual sites, rotated so the same spot is not used repeatedly. There is no reason to inject near the head; the peptide is carried systemically. Technique is covered in how to inject peptides safely.

Semax Dosage
What the studies used, and why that is not your dose
Almost all of the human work on Semax comes from Russia, where it is a registered medicine supplied as nasal drops. The studies people quote were done with that intranasal product:
| Study | Who | Route |
|---|---|---|
| Korsakov Journal of Neurology and Psychiatry, 1997 | 30 patients in the acute phase of ischaemic stroke, compared with 80 conventionally treated patients | Intranasal, alongside standard intensive care |
| Korsakov Journal, 2005 | 187 patients with chronic cerebrovascular insufficiency | Intranasal |
| Clinical trial, 2018 | 110 patients in stroke rehabilitation, with and without Semax | Intranasal |
| Bulletin of Experimental Biology and Medicine, 2018 | 14 healthy adults, a single dose, measured on functional MRI | Intranasal |
The honest reading is that none of these sets a dose for an injection. The FDA's 2026 review found no published pharmacokinetic work on subcutaneous Semax, so there is no study that converts a nasal dose into an injected one. The populations were stroke and cerebrovascular patients, plus one small mechanistic study in healthy adults, and there is no published trial of Semax for focus or memory in healthy people. That is why your directions come from a physician who has reviewed your intake, not from a study table. What the research has looked at, finding by finding, is in Semax benefits.
Semax Dosage
What changes the directions
Your physician sets your directions after reviewing your intake. Things that can lead to different directions, or a different product, include:
| What can change | How it shows up |
|---|---|
| Semax alone or in a blend | ELEVATE, SYNAPSE and Semax + Selank each have their own label and their own Semax per dose |
| What you are describing | Stress or anxiety as the main complaint often points to Selank; poor sleep points to a sleep conversation first |
| Your other medications | A stimulant matters, because a mouse study reported Semax increasing amphetamine-induced dopamine release; the intake asks for everything you take |
| How you respond | Restlessness, headache or anything that does not settle goes to your care team |
When directions change, the new numbers are printed on the new label. Never carry a number over from a previous fill. Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.
Semax Dosage
Side effects, in summary
Semax is described as well tolerated in the Russian clinical series, including in older stroke patients; the 2005 cerebrovascular study made tolerability an explicit endpoint and reported it as good.
| What is reported | When | What to do |
|---|---|---|
| Injection-site redness, mild soreness, occasional bruising | Any time | Rotate sites |
| Transient restlessness | Mostly if dosed late in the day | Tell your care team; morning dosing is the usual direction |
| Headache | Occasionally, early on | Report it if it persists |
| A reaction that is severe, spreading, or involves difficulty breathing | Rare | Emergency care for breathing difficulty, then contact your physician |
The FDA's stated safety concern in its 2026 review was a quality one rather than a known toxicity: an injectable peptide could in principle provoke an immune response if it aggregates or carries impurities. That is a reason to care where a compounded peptide comes from. There is no established interaction list, because the trials that would produce one have not been run. Semax is not used in pregnancy or breastfeeding. Tested athletes should check the current prohibited list for their sport.
Semax Dosage
Semax and the FDA, briefly
Semax was placed in Category 2 of the FDA's 503A bulk drug substances list in 2023 and was no longer on that list as of April 2026. On 24 July 2026 the FDA's Pharmacy Compounding Advisory Committee voted 8 to 5, with one abstention, to recommend adding Semax to the 503A Bulks List, against the recommendation of FDA staff. A committee vote is a recommendation, not a decision, and the FDA's final determination is pending. Semax remains legal to prescribe and dispense. Compounded medications are not FDA approved, and Semax is not approved for anything in the United States. More is in are peptides FDA approved.

Where to get Semax prescribed
Semax cannot be bought legitimately without a prescription. Sites selling it "for research use only" are not accountable for what is in the vial: identity, purity, sterility and concentration are untested, so even the concentration your units chart depends on is unknown. The prescription route:
- Complete a free online intake covering your history, medications and what you want help with.
- A physician licensed in your state reviews it, usually within 24 hours.
- If it is appropriate, a PCAB-accredited, FDA-registered US 503A compounding pharmacy prepares it to that prescription. Lot test results are published at /quality/lab-results.
- It ships refrigerated with your directions printed on the vial.
- Your physician stays reachable for questions and dose changes.

Common questions
Semax Dosage, answered.
Semax is typically prescribed as 0.25 mL, which is 25 units on a U-100 insulin syringe, injected subcutaneously once daily Monday through Friday. From the 2 mg/mL vial that is 0.5 mg per dose. Your physician sets your directions after reviewing your intake, and your exact dose is printed on the vial.
The amount printed on your vial, on the schedule printed on your vial. Pepti's typical directions are 25 units five days a week, but your own directions are set by your physician after reviewing your intake. If you have questions about your dose, message your care team.
The number printed on your vial. Pepti's typical directions are 25 units, which is 0.25 mL on a U-100 insulin syringe, where 1 unit is 0.01 mL.
From the 2 mg/mL single vial, 0.5 mg. From the ELEVATE or SYNAPSE blends, which hold Semax at 1 mg/mL, the same 25 units carries 0.25 mg. A different concentration always gives different milligrams for the same units.
The chart above converts units to millilitres to milligrams for the 2 mg/mL vial and the 1 mg/mL blends. It is a conversion, not a list of doses to choose from. Your own label is the one that applies.
The label says once daily, Monday through Friday. Morning is the usual direction, because some people describe restlessness if it is taken late in the day. If your label names a time, follow it.
About four weeks. A 10 mg vial at 25 units (0.5 mg) five days a week is 20 injections, which uses the whole 10 mg in four weeks, matching the 28-day refill.
No. The Russian studies used intranasal drops, and the FDA found no published pharmacokinetic work on subcutaneous Semax, so no study links one to the other. Your injection directions are printed on your vial.
No. Take the dose printed on your vial, on the schedule printed on your vial. There is no study showing that more Semax by injection does more, and restlessness is more often described with late or extra dosing. If you have questions about your dose, message your care team.
No. Pepti vials are multi-dose solutions shipped ready to use, with sterile syringes and alcohol prep pads. Nothing is reconstituted at home.
