Tesamorelin Side Effects: Common, Serious, and What to Report

This page is a practical reference: what each side effect is, how often it was reported, what it usually means, what to report and how quickly, and who should not take tesamorelin. For the molecule, its trials and its regulatory position, see what is tesamorelin.

Tesamorelin (Injectable)
Tesamorelin$249/mo

The most common tesamorelin side effects are injection-site reactions, joint pain, muscle aches, swelling in the hands or feet, and tingling or numbness. On the approved product's label, injection-site reactions were reported by 17% of treated patients against 6% on placebo, and hypersensitivity reactions such as itching, flushing, hives or rash by 4%. The side effect that needs the most attention is blood sugar: growth hormone reduces insulin sensitivity, and the label records new-onset diabetes in 5% of treated patients against 1% on placebo. Those figures were recorded at 2 mg daily, four times Pepti's typical compounded dose of 0.5 mg.

Tesamorelin Side Effects

The short answer

Side effect On tesamorelin (approved label, 2 mg daily) On placebo What to do
Injection-site reactions 17% 6% Rotate sites; report anything spreading
Joint pain (arthralgia) 13% 11% Message your care team; do not change the dose yourself
Pain in an arm or leg 6% Less often Message your care team
Muscle aches (myalgia) 6% Less often Message your care team
Swelling in the hands, feet or ankles (peripheral oedema) 6% Less often Message your care team
Tingling or numbness (paraesthesia) 5% Less often Message your care team
Hypersensitivity: itching, flushing, hives, rash 4% Not stated Report it; emergency care for throat swelling or breathing difficulty
New-onset diabetes 5% 1% Raise diabetes or borderline glucose at intake; report thirst and frequent urination

These rates come from the approved product's label, which pools data from 740 treated patients, 543 of them in the 26-week placebo-controlled phase. The medications this page covers:

Product What is in it Typical directions Tesamorelin per dose Price
Tesamorelin 5 mL multi-dose vial, 2 mg/mL (10 mg per vial) 0.25 mL (25 units) Monday through Friday nights at bedtime 0.5 mg $249/mo
Tesamorelin + Ipamorelin 10 mg tesamorelin, 5 mg ipamorelin in 5 mL 0.25 mL (25 units) Monday through Friday nights at bedtime 0.5 mg $259/mo

Every price is all-in: medication, physician review, refill management and shipping, with no membership fee. Pepti is cash-pay and does not bill insurance. An approved tesamorelin product exists for one indication; compounded tesamorelin is not FDA approved, and compounded medications are not FDA approved. The rates on this page belong to the approved product, which contains the same active peptide.

Tesamorelin Side Effects

Why tesamorelin causes the side effects it does

Tesamorelin is the full 44-amino-acid sequence of growth-hormone-releasing hormone (GHRH), the signal the hypothalamus sends to the pituitary, with a cap that protects it from being broken down. It binds the GHRH receptor and prompts a pulse of your own growth hormone, which works in adults largely through IGF-1 made in the liver. It contains no growth hormone, and the body's brakes, somatostatin and IGF-1 feedback, keep working.

Almost every side effect is that mechanism. Growth hormone makes the body hold more fluid, which shows up as puffiness, joint aches, swelling and tingling. Growth hormone reduces insulin sensitivity, which is the reason for the blood sugar warning. And like any injected peptide, it can irritate the skin where it goes in or, rarely, provoke an allergic reaction. In the trials, IGF-1 rose by 81% in one study and 117% in another while staying within the physiological range; the aim is a physiological level, not the highest one achievable.

Tesamorelin Side Effects

Common tesamorelin side effects

  • Injection-site reactions

The commonest complaint, at 17% against 6% on placebo: redness, itching or a small lump where the dose went in. Rotating sites so the same spot is not used repeatedly is the main fix; the abdomen is the usual area. Technique is in how to inject peptides safely. A site that is hotter, larger or more painful at 48 hours than at 24 hours can be infection rather than irritation and needs a same-day message.

  • The fluid-related effects: puffiness, joint aches, swelling, tingling

Joint pain (13% against 11%), pain in an arm or leg, muscle aches and peripheral swelling (each 6%), and tingling or numbness (5%) are classic growth hormone effects. So, occasionally, are carpal tunnel symptoms in the hand. These usually mean the dose is higher than the person needs, not that something rare has gone wrong. The right response is a message to your care team, not pushing through and not changing the dose yourself. Your physician sets your directions after reviewing how you are doing, and any change is printed on the next label.

  • Sleep and dreams

Pepti's tesamorelin directions say nights at bedtime, because the body's largest natural growth hormone pulse happens during slow-wave sleep. Sleep is what people most often describe noticing first, often deeper and sometimes with more vivid dreams. That is a prescriber-described pattern, not a trial outcome, and it is not usually a reason to report anything unless sleep is worse.

Tesamorelin Side Effects

Tesamorelin and blood sugar

This is the side effect to understand before starting. Growth hormone reduces insulin sensitivity, so tesamorelin can raise blood glucose. In the trials there were no clinically meaningful group differences in glucose at 26 or 52 weeks, and the one trial outside HIV, in 60 abdominally obese adults over 12 months, reported no change in glucose measures. But the approved label records new-onset diabetes in 5% of treated patients against 1% on placebo.

Situation What it means
No diabetes, normal glucose The label figure still applies; report the symptoms below
Borderline fasting glucose or prediabetes Raise it at intake; it means closer supervision, not automatic exclusion
Type 2 diabetes Raise it at intake; it means closer supervision
Insulin or another glucose-lowering medication Say so at intake; your diabetes care needs to know too

Symptoms of raised blood sugar to report include unusual thirst and passing urine more often. Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.

Tesamorelin Side Effects

Serious side effects of tesamorelin: what to report urgently

Symptom What it may be What to do
Swelling of the face, lips or throat, hives spreading away from the site, wheezing or difficulty breathing Serious allergic reaction Call 911 or go to emergency care first, then tell your care team
Widespread itching, flushing or rash after a dose Hypersensitivity Stop and contact your care team the same day
Unusual thirst or frequent urination Raised blood sugar Contact your care team promptly
Swelling that keeps building, or numbness and weakness in the hand Fluid retention or carpal tunnel symptoms Message your care team
An injection site that is hot, spreading or worse after 48 hours Possible infection Same-day message
A new lump or a change in a known cancer Needs assessment Contact your physician promptly
Pregnancy Tesamorelin is contraindicated Stop and message your care team
  • Allergic reactions

Hypersensitivity reactions, including itching, flushing, hives and rash, occurred in 4% of treated patients on the label. Most are mild. The pattern that is an emergency is systemic: facial or throat swelling, hives away from the injection site, wheeze or breathing difficulty. That is 911 first and a message to your care team second. A known hypersensitivity to tesamorelin rules it out.

  • Cancer

Growth hormone signalling is a growth signal, which is why active cancer is a contraindication on the approved label and a prior cancer should be inactive with treatment complete. This is the standard caution for the growth hormone axis. The wider reasoning is in can peptides cause cancer.

  • Interactions

There is no interaction list beyond a label note that growth hormone can alter cytochrome P450 metabolism, the liver system that clears many drugs. That is why the intake asks for every medication you take and a physician reviews it rather than a form.

Tesamorelin Side Effects

Tesamorelin side effects at Pepti's dose vs the label dose

The approved product's trials Pepti's typical compounded directions
Daily dose 1 mg or 2 mg daily 0.5 mg, Monday through Friday nights
Schedule Every day Five nights a week, weekends off
Who was studied Mostly adults with HIV and abdominal fat Adults whose intake a physician has reviewed
Longest follow-up 12 months Ongoing, reviewed at each 28-day refill

The label figures were recorded at a higher daily dose than Pepti's directions, and the fluid-related effects are associated with growth hormone levels above what a person needs. Whether the rates are lower at 0.5 mg on weekdays has not been studied, so this page does not claim they are. The current approved formulation is dosed at 1.28 mg once daily. Its label also states that long-term cardiovascular safety has not been established.

Tesamorelin Side Effects

Side effects in the blends that contain tesamorelin

Tesamorelin appears in several Pepti blends, and in a blend the side effects are those of every ingredient. Each is a 5 mL vial drawn at 0.25 mL.

Product What else is in it Tesamorelin per dose What else to know about
PHYSIQ Blend Ipamorelin, AOD-9604, MOTS-c 0.5 mg MOTS-c can cause early jitteriness; see MOTS-c side effects
TITAN Blend CJC-1295, ipamorelin, BPC-157 0.5 mg A second GHRH-axis route, plus BPC-157
ASCEND Blend CJC-1295, ipamorelin, IGF-1 LR3 0.5 mg More growth-hormone-axis signal, so the same fluid and glucose cautions
RADIANCE Blend GHK-Cu 0.5 mg The GHK-Cu copper cautions
SYNAPSE Blend Semax, selank, MOTS-c 0.5 mg MOTS-c cautions
FUSION Blend BPC-157, MOTS-c, 5-Amino-1MQ 0.75 mg More tesamorelin per dose than the others, from a 3 mg/mL vial

If you are prescribed a blend, read its label and its own page rather than assuming it behaves like tesamorelin alone. The blends are compared in what is tesamorelin + ipamorelin and physiq vs shred vs titan. Every dose and every blend is set out in tesamorelin dosage.

Tesamorelin Side Effects

Who should not take tesamorelin, or should discuss it first

Situation Status
Pituitary disorder, pituitary surgery, head irradiation or head trauma Contraindicated on the approved label; a secretagogue needs a pituitary that can respond
Active cancer Contraindicated
Pregnancy or breastfeeding Contraindicated; animal data describe fetal harm
Known hypersensitivity to tesamorelin Contraindicated
Diabetes or borderline fasting glucose Not a bar, but needs closer supervision
A prior cancer Should be inactive with treatment complete; needs discussion
Tested athletes The 2026 WADA Prohibited List names tesamorelin under S2.2.4, prohibited at all times
Under 18 Pepti prescribes to adults only

Tesamorelin Side Effects

When something else makes more sense

If the side effects above are a concern, the alternatives have their own profiles. Sermorelin is a shorter-acting GHRH analogue on the same receptor, so it shares the class cautions. MOTS-c works through a different, non-pituitary pathway and does not carry the growth-hormone fluid effects. For weight on the scale, which tesamorelin's label calls weight-neutral, semaglutide and tirzepatide have far more evidence, with their own side effects. How they compare: sermorelin vs tesamorelin vs ipamorelin, MOTS-c vs tesamorelin, AOD-9604 vs tesamorelin and tesamorelin vs semaglutide.

Tesamorelin Side Effects

What the evidence shows, honestly

Tesamorelin's side-effect profile is better characterised than that of almost any other compounded peptide, because it comes from randomised, double-blind, placebo-controlled trials, including two phase 3 trials that supported an FDA approval, and a label that pools 740 treated patients. The common effects, the glucose warning and the contraindications are documented rather than inferred.

The limits are real. The trial rates belong to the approved product at 1 to 2 mg daily, not to a compounded preparation at 0.5 mg on weekdays. The trials enrolled mostly people with HIV, ran 12 months at most, and say nothing about long-term cardiovascular safety, which the label itself says has not been established. And no trial has enrolled healthy, non-overweight adults with normal growth hormone secretion. Read the placebo column first: 11% of people on a dummy injection reported joint pain, so not everything you feel on tesamorelin is the drug.

One risk specific to compounded vials is dosing error, which is why Pepti prints the dose in both millilitres and syringe units for the exact vial you receive. If the label does not match what you expected, do not inject; message your care team. The benefit side of the same trials is in tesamorelin benefits.
