Treatments mentioned
PHYSIQ, SHRED and TITAN are all four-peptide blends in a 5 mL vial, dosed at 0.25 mL five days a week, and all three contain ipamorelin, but they are built for different goals. PHYSIQ (tesamorelin, ipamorelin, AOD-9604, MOTS-c) and SHRED (AOD-9604, CJC-1295, ipamorelin, MOTS-c) are body-composition blends that differ in one component: PHYSIQ uses tesamorelin as its growth hormone releasing signal and SHRED uses CJC-1295. TITAN (tesamorelin, CJC-1295, ipamorelin, BPC-157) drops the fat-metabolism and mitochondrial components and adds the repair peptide BPC-157, so it is aimed at sleep, recovery and soft tissue. None of the three has been studied as a blend, and none is FDA approved.

|---|---|---|---|
| Tesamorelin | 0.5 mg | None | 0.5 mg |
| CJC-1295 | None | 0.3 mg | 0.3 mg |
| Ipamorelin | 0.5 mg | 0.5 mg | 0.6 mg |
| AOD-9604 | 0.5 mg | 0.5 mg | None |
| MOTS-c | 0.5 mg | 0.5 mg | None |
| BPC-157 | None | None | 0.5 mg |
What the overlap means
- Ipamorelin is in all three. It is the ghrelin-receptor half of the growth hormone signal in every blend, at the same 0.5 mg per dose in PHYSIQ and SHRED and 0.6 mg in TITAN.
- PHYSIQ and SHRED share three of four components at identical amounts: AOD-9604, ipamorelin and MOTS-c. The only difference is the GHRH analogue, tesamorelin in PHYSIQ and CJC-1295 in SHRED.
- TITAN is the odd one out. It carries two GHRH analogues, tesamorelin and CJC-1295, plus ipamorelin, which is the same three-secretagogue core as the ASCEND Blend. Its fourth component, BPC-157, is not part of the growth hormone axis at all.
- The ratios are fixed. Each pharmacy formulation sets the proportions, and they cannot be adjusted per component. If a physician wants more of one peptide and none of another, the components are prescribed separately.
PHYSIQ vs SHRED vs TITAN
The components, briefly
Tesamorelin (PHYSIQ, TITAN)
A stabilized 44-amino-acid analogue of growth-hormone-releasing hormone that prompts the pituitary to release your own growth hormone. It is the one component in any of the three blends with randomized, placebo-controlled human trials: in people with HIV and abdominal fat accumulation, those trials reported a reduction in visceral fat on CT, with no significant change in subcutaneous fat, and visceral fat returned toward baseline after stopping. The approved product is labelled weight-neutral, and the trials used 1 to 2 mg daily, not the 0.5 mg in a blend dose. See what is tesamorelin.
CJC-1295 (SHRED, TITAN)
A modified 29-amino-acid GHRH analogue, protected from the enzyme that clears native GHRH within minutes. It acts at the same pituitary receptor as tesamorelin. Two 2006 studies in the Journal of Clinical Endocrinology and Metabolism, in healthy adults and of the long-acting DAC form, reported growth hormone and IGF-1 rising for days with the natural pulsatile pattern preserved. No trial has measured a clinical outcome. See what is CJC-1295 / Ipamorelin.
Ipamorelin (all three)
A five-amino-acid peptide acting at the ghrelin receptor, a second route to the same growth hormone pulse. A 1998 paper in the European Journal of Endocrinology described it as the first selective secretagogue, releasing growth hormone in animals without the cortisol rise earlier compounds produced. Its largest human study, a 2014 phase 2 trial of intravenous ipamorelin for postoperative ileus, did not separate from placebo. Because ghrelin is the hunger hormone, some people notice more appetite.
AOD-9604 (PHYSIQ, SHRED)
A modified fragment of the end of the growth hormone molecule, designed to keep fat-metabolism activity without effects on growth. Mouse work reported increased fat oxidation and reduced weight gain. The human obesity programme, oral and intravenous, including a 536-person phase 2B trial, did not show significant weight loss versus placebo, and development for obesity ended in 2007. There is no human data for the subcutaneous route. See AOD-9604 vs tesamorelin.
MOTS-c (PHYSIQ, SHRED)
A 16-amino-acid peptide encoded in mitochondrial DNA, described in a 2015 Cell Metabolism mouse study as acting on muscle through AMPK, the cell's energy sensor. A 2021 Nature Communications paper reported exercise raising the body's own MOTS-c in people. There are no human trials of injected MOTS-c. See MOTS-c vs tesamorelin.
BPC-157 (TITAN)
A 15-amino-acid fragment of a protein from human gastric juice. Animal and cell work describes it around new blood vessel growth, tendon cell migration and the gut lining, and a 2014 study in cultured tendon cells reported more growth hormone receptors on those cells, which is the reason it sits in a growth hormone blend. There are no large randomized human trials. See what is BPC-157.

The real difference is the fourth and fifth slots, not strength
People often ask which blend is "strongest." That is not a meaningful comparison: no study has measured any of the three, and each delivers a similar total amount of peptide. The useful question is what each blend contains beyond the growth hormone pair. PHYSIQ and SHRED add AOD-9604 and MOTS-c, which is a body-composition rationale resting on mouse work and, for AOD-9604, a negative human trial. TITAN adds BPC-157, a repair rationale resting on animal and cell work, and doubles up on the GHRH signal.
PHYSIQ vs SHRED vs TITAN
Who suits which
PHYSIQ
PHYSIQ is the usual choice when the goal is body composition and visceral fat specifically, alongside training. Its GHRH component is tesamorelin, the one peptide across all three blends with randomized trials on visceral fat, which is why a physician who wants the better-evidenced GHRH signal will usually prefer PHYSIQ to SHRED. Full detail: what is PHYSIQ.
SHRED
SHRED is the same concept with CJC-1295 in place of tesamorelin, at $10 a month less. It suits someone whose goal is body composition with the metabolic components, where visceral fat is not the specific target, or whose physician prefers the CJC-1295 and ipamorelin pairing, the more conventional GHRH-plus-ghrelin combination. Full detail: what is SHRED and how to get SHRED prescribed.
TITAN
TITAN suits someone whose goal is sleep, recovery from training, or a nagging tendon or ligament problem alongside the growth hormone pulse, rather than fat loss. It has no AOD-9604 or MOTS-c. Compared with ASCEND, which shares its three secretagogues and adds IGF-1 LR3, TITAN is the one without IGF-1 LR3's blood-sugar and growth-signalling cautions. Full detail: what is TITAN.
Can you take two of these blends together?
This is not done. All three carry ipamorelin, and every pairing duplicates at least one more component: PHYSIQ and SHRED share three of four, PHYSIQ and TITAN share tesamorelin and ipamorelin, and SHRED and TITAN share CJC-1295 and ipamorelin. Taking two would mean changing doses that no one decided should change. If the goal spans both body composition and recovery, the better route is one blend plus a single component, set by your physician after reviewing your intake. More on combining is in can you take two peptides together.
PHYSIQ vs SHRED vs TITAN
When a blend is not the right first step
| Situation | What makes more sense |
|---|---|
| Weight loss is the actual goal | Semaglutide (from $99/mo) or tirzepatide (from $159/mo); they have large randomized trials for weight management, which no component in these blends does |
| Visceral fat is the whole goal | Tesamorelin alone, $249/mo, at a strength set independently rather than fixed at a quarter of a blend |
| The growth hormone pulse is all you want | CJC-1295 / Ipamorelin, $239/mo, or Tesamorelin + Ipamorelin, $259/mo |
| A tendon, ligament or gut problem and nothing else | BPC-157 alone, $209/mo, or the BPC-157 + TB-500 stack, $259/mo |
| Only one of the metabolic components appeals | AOD-9604, $229/mo, or MOTS-c, $259/mo |
| Metabolic support with gut repair, often alongside a GLP-1 | FUSION Blend, $279/mo: BPC-157, MOTS-c, 5-Amino-1MQ and tesamorelin |
PHYSIQ vs SHRED vs TITAN
Alongside a GLP-1
PHYSIQ and SHRED are commonly used alongside semaglutide or tirzepatide. They do a different job: the GLP-1 addresses appetite, and the blend is aimed at the growth hormone axis and muscle fuel handling. No trial has tested any blend with a GLP-1, and no trial shows that a growth hormone secretagogue preserves lean mass during GLP-1 treatment. Growth hormone reduces insulin sensitivity while GLP-1 medications act in the other direction, so glucose is what your physician watches, especially for blends containing tesamorelin. The full list of pairings is in what peptides can you take with tirzepatide.

Where each component stands with the FDA
A blend has no regulatory status of its own; the FDA regulates the bulk substances a pharmacy compounds from. As of the FDA pages checked in September 2026:
| Component | In which blends | Status |
|---|---|---|
| Tesamorelin | PHYSIQ, TITAN | Active ingredient of an approved product, approved only for excess abdominal fat in adults with HIV-associated lipodystrophy; never on the 503A bulks list; other uses are off-label |
| CJC-1295 | SHRED, TITAN | Category 2 in 2023; advisory committee voted 0 to 13 against the free base on 4 December 2024; nomination withdrawn; no final rule |
| Ipamorelin | All three | Category 2 in 2023; advisory committee voted 0 to 12, one abstention, against on 29 October 2024; nomination withdrawn for 503A; no final rule |
| AOD-9604 | PHYSIQ, SHRED | Category 2 in 2023; advisory committee voted 0 to 12 against on 4 December 2024; nomination withdrawn; no final rule |
| MOTS-c | PHYSIQ, SHRED | Removed from Category 2 on 15 April 2026; advisory committee voted 7 to 5, two abstentions, to recommend it for the 503A bulks list on 23 July 2026; final determination pending |
| BPC-157 | TITAN | Removed from Category 2 in April 2026; advisory committee voted 8 to 6 to recommend it on 23 July 2026; final determination pending |
None of the six is currently in 503A Category 2, and none is banned. An advisory committee vote is a recommendation, not a decision. All remain legal to prescribe and dispense as compounded medications, and none of the three blends is FDA approved: compounded medications are not FDA approved. Are peptides FDA approved explains the difference between not approved and not legal.
PHYSIQ vs SHRED vs TITAN
Dosing, side by side
All three are injected under the skin (subcutaneously) into the abdomen or thigh, rotating sites, at 0.25 mL, which is 25 units on a U-100 insulin syringe, five days a week with weekends off. PHYSIQ and TITAN are directed at bedtime, Monday through Friday nights, because the body's largest natural growth hormone pulse happens in the first hours of sleep and the secretagogues are timed to sit alongside it. SHRED is directed once daily Monday through Friday; some prescribers direct a consistent evening time for the same reason. With TITAN, prescribers generally direct the dose away from food, because a rise in glucose and insulin blunts growth hormone release.
You do not need to calculate anything: your physician sets your directions after reviewing your intake, and they are printed on the vial. Twenty doses per vial covers about four weeks, which is why refills run every 28 days, one vial at a time. All three ship refrigerated with ice packs, with sterile syringes and alcohol prep pads, and stay in the fridge between doses. Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.
PHYSIQ vs SHRED vs TITAN
Side effects
Shared by all three. Injection-site redness, mild soreness or occasional bruising, and the effects of raised growth hormone signalling: fluid retention, joint stiffness or aching, tingling in the hands, and headache. These are listed on the approved tesamorelin label at doses well above a blend dose, and are usually described as dose-related. Ipamorelin can raise appetite in some people, and a brief flush after injecting is reported.
PHYSIQ and SHRED. MOTS-c has no human safety data, and AOD-9604's comes from oral and intravenous trials rather than the subcutaneous route, where adverse events were similar between peptide and placebo.
TITAN. The BPC-157 side has no large human safety record. Because it is described around new blood vessel growth, a personal history of cancer and anticoagulant therapy are raised at intake.
For all three, growth hormone reduces insulin sensitivity, so diabetes or impaired glucose tolerance needs discussing first. Stop and contact your physician for any reaction that is severe, spreading, or involves difficulty breathing.
PHYSIQ vs SHRED vs TITAN
Who should not take them, or should discuss it first
| Situation | Why |
|---|---|
| Pregnancy or breastfeeding | Not used; safety data is absent, and the approved tesamorelin label lists pregnancy as a contraindication |
| Active cancer or a personal history of cancer | The approved tesamorelin label is contraindicated in active malignancy; growth signalling and BPC-157's vascular mechanism are reasons to raise any history |
| Diabetes or impaired glucose tolerance | Growth hormone reduces insulin sensitivity |
| Pituitary disorder, pituitary surgery or head irradiation | The secretagogues need a pituitary that can respond |
| Anticoagulant therapy | Especially for TITAN, given BPC-157's vascular mechanisms |
| Tested athletes | Growth hormone secretagogues, GHRH analogues, AOD-9604 and BPC-157 are prohibited in competition; a prescription does not change that |
PHYSIQ vs SHRED vs TITAN
What the evidence shows, honestly
None of the three blends has ever been studied as a blend, in humans or animals, and no study has compared any blend with its components. The evidence is per component and very uneven. Tesamorelin, in PHYSIQ and TITAN, has randomized phase 3 trials on visceral fat in a specific population at higher doses, with weight unchanged. CJC-1295 and ipamorelin have human pharmacology showing growth hormone release, and no outcome trials for body composition, sleep or recovery. AOD-9604's human obesity trials were negative. MOTS-c and BPC-157 rest on animal and cell work. These blends are prescribed on mechanism plus clinical experience; any provider describing them as shown to produce fat loss or muscle gain is overstating what exists.

Where to get PHYSIQ, SHRED or TITAN prescribed
None of the three can be bought legitimately without a prescription; "research use only" sellers are not accountable for what is in the vial. The prescription route:
- Complete a free online intake covering your history, medications and what you want help with.
- A physician licensed in your state reviews it, usually within 24 hours.
- If it is appropriate, a PCAB-accredited, FDA-registered US 503A pharmacy compounds it to that prescription. Lot test results are published at /quality/lab-results.
- It ships refrigerated with your directions printed on the vial.
- Your physician stays reachable for questions and dose changes.
Pepti prescribes all three in all 50 states and DC. Step-by-step pages: how to get SHRED prescribed online, how to get tesamorelin prescribed online and how to get CJC-1295 / Ipamorelin prescribed online.

Common questions
PHYSIQ vs SHRED vs TITAN, answered.
One component. PHYSIQ uses tesamorelin as its GHRH analogue and SHRED uses CJC-1295; both contain the same AOD-9604, ipamorelin and MOTS-c at 10 mg each. Tesamorelin is the one with randomized trials on visceral fat. PHYSIQ is $289 a month and SHRED is $279 a month.
They share tesamorelin and ipamorelin. PHYSIQ adds AOD-9604 and MOTS-c for a body-composition goal; TITAN adds CJC-1295 and BPC-157 for sleep, recovery and soft-tissue repair. Both are $289 a month and both are dosed at bedtime.
None has trial evidence for fat loss. PHYSIQ is the usual choice when visceral fat is the focus, because it carries tesamorelin, and SHRED is the CJC-1295 alternative. TITAN is not a fat-loss blend. For weight loss, semaglutide and tirzepatide are the medications with the evidence.
TITAN is the one built around recovery, with three growth hormone secretagogues and BPC-157. No trial has shown an effect on muscle size or strength for any of its components; training and protein do the work it is meant to support.
No. Ipamorelin is the only component in all three. PHYSIQ and SHRED share three of four components, and TITAN shares tesamorelin with PHYSIQ and CJC-1295 with SHRED.
At 0.25 mL: PHYSIQ delivers 0.5 mg each of tesamorelin, ipamorelin, AOD-9604 and MOTS-c. SHRED delivers 0.5 mg AOD-9604, 0.3 mg CJC-1295, 0.5 mg ipamorelin and 0.5 mg MOTS-c. TITAN delivers 0.5 mg tesamorelin, 0.3 mg CJC-1295, 0.6 mg ipamorelin and 0.5 mg BPC-157.
They are commonly used alongside a GLP-1 because they do a different job. No trial has tested the combination, and glucose is what your physician watches, especially with tesamorelin in the blend.
This is not done. All three contain ipamorelin and each pair duplicates at least one other component, so stacking two blends changes doses no one set. One blend plus a single component, chosen by your physician, is the usual route.
At Pepti, PHYSIQ is $289 a month, SHRED is $279 a month and TITAN is $289 a month, all-in, including the medication, physician review, refill management and shipping, with no membership fee.
