MOTS-c vs Tesamorelin for Fat Loss: Mechanism, Evidence, Cost, and Taking Them Together

This page explains each on its own terms, puts them side by side for fat loss, covers what the research does and does not show, where each stands with the FDA, how each is dosed, what each costs at Pepti, and what is known about taking them together or in the same vial.

Tesamorelin (Injectable)
Tesamorelin$249/mo

MOTS-c and tesamorelin are both prescribed around fat and body composition, but they act on different systems and have very different evidence. Tesamorelin is a growth-hormone-releasing hormone analogue with randomised, placebo-controlled human trials that reported a reduction in visceral fat, the fat around the organs, with little change in body weight. MOTS-c is a peptide encoded in mitochondrial DNA, described around the cell's fuel sensor AMPK, with mouse and cell data and no randomised human trials. They can be taken together, and three Pepti blends (SYNAPSE, PHYSIQ and FUSION) already put both in one vial, but no study has tested the combination, and neither is a weight-loss drug.

Tesamorelin on its own terms

  • What it is

Tesamorelin is the full 44-amino-acid sequence of growth-hormone-releasing hormone, the signal the hypothalamus sends to the pituitary, with a trans-3-hexenoic acid cap that protects it from the enzyme DPP-4. It binds the GHRH receptor and prompts a pulse of the body's own growth hormone. It contains no growth hormone, and because the pituitary does the releasing, the feedback loops involving somatostatin and IGF-1 still apply. The full reference is at what is tesamorelin.

  • How tesamorelin is described to work on fat

Growth hormone is lipolytic: it signals fat cells to release stored triglyceride. Visceral fat carries a higher density of the relevant receptors than subcutaneous fat. That is the described reason every tesamorelin trial reported a measurable reduction in visceral fat on CT with no significant change in subcutaneous fat. Growth hormone acts largely through IGF-1, which rose by 81% over 26 weeks in the 2007 New England Journal of Medicine trial.

  • What human data exists for tesamorelin

    Study Who What was reported
2007, New England Journal of Medicine 412 adults with HIV and abdominal fat accumulation, 26 weeks Visceral fat fell 15.2% on tesamorelin and rose 5.0% on placebo
2010 pooled analysis, Journal of Clinical Endocrinology and Metabolism 806 patients from two phase 3 trials Treatment effect of 15.4% on visceral fat; no significant change in subcutaneous fat
2012, Journal of Clinical Endocrinology and Metabolism 60 abdominally obese adults without HIV, reduced GH secretion, 12 months Visceral fat fell 16 cm² against a 19 cm² rise on placebo, with no change in subcutaneous fat or glucose measures
2014, JAMA 50 adults with HIV, six months Visceral fat fell 34 cm² against an 8 cm² rise; a modest reduction in liver fat

All of these used 1 or 2 mg daily of the approved product. None used the compounded dose or the weekday-night schedule.

MOTS-c vs Tesamorelin for Fat Loss

MOTS-c on its own terms

  • What it is

MOTS-c is a 16-amino-acid peptide whose gene sits in the mitochondrial genome rather than the cell nucleus, inside the gene for the 12S ribosomal RNA. It was identified in a 2015 paper in Cell Metabolism from a group at the University of Southern California. It is not a growth hormone secretagogue, it does not act on the pituitary, it is not a GLP-1 and does not work through appetite, and it is not a stimulant. The full reference is at what is MOTS-c.

  • How MOTS-c is described to work on fat

    • The folate cycle and AMPK. In cultured cells MOTS-c was reported to slow the folate cycle, causing a molecule called AICAR to accumulate, which activates AMPK, the enzyme cells use as a low-fuel sensor. The described chain ends with the cell reading the signal as a reason to burn rather than store. Skeletal muscle appeared to be the primary target.
    • Insulin sensitivity. In the 2015 Cell Metabolism work, MOTS-c treatment in mice was reported to prevent age-related insulin resistance and the insulin resistance and weight gain that develop on a high-fat diet.
    • Brown fat. A 2019 paper in the Journal of Molecular Medicine, in ovariectomised mice, reported more brown-fat activity and less fat accumulation in treated animals.
    • The exercise connection. A 2021 paper in Nature Communications reported better physical performance in treated mice and that a bout of exercise raised the body's own MOTS-c in humans. That human finding is observational, which is why "exercise-mimetic" is a proposed label rather than a demonstrated effect.
  • What human data exists for MOTS-c

There are no randomised controlled trials of MOTS-c in humans. Human data is observational: studies measuring the body's own MOTS-c, some finding lower levels in type 2 diabetes and others finding similar levels with and without obesity. The closest thing to a treatment trial was of a modified analogue called CB4211, a different molecule, in a small company-reported Phase 1 programme that did not advance. Every fat and weight finding for MOTS-c itself is from mice.

The real difference is evidence, and which fat

Asking which is "better for fat loss" runs into two problems. First, the evidence is not comparable: tesamorelin has human trials with fat measured on imaging, while MOTS-c has a coherent mouse story and nothing in people. Second, neither is a weight-loss drug in the sense most people mean. Tesamorelin's own label says its effect on weight is neutral; its trials are about visceral fat, the fat around the organs. MOTS-c's fat findings are in mice, where it was described as preventing gain rather than causing loss. Neither has been compared with the other in any study.

MOTS-c vs Tesamorelin for Fat Loss

Who suits which

  • Tesamorelin

Tesamorelin is the usual choice when the specific concern is fat around the middle, particularly in someone near a healthy weight, and when the person wants the option with human trial data behind it. Progress is judged by waist measurement over months, not the scale.

  • MOTS-c

MOTS-c suits people who understand it is experimental and want it alongside a training and nutrition plan, with metabolism, training tolerance or steadier energy as the goal. Prescribers describe it as something taken with exercise, not instead of it, because its described mechanism is an exercise-signalling pathway.

  • Neither, first

If scale weight and appetite are the actual goal, semaglutide and tirzepatide have large randomised human trials with weight as the primary endpoint. Tesamorelin vs semaglutide covers that comparison, and best peptides for weight loss ranks the options by evidence.

MOTS-c vs Tesamorelin for Fat Loss

Can you take MOTS-c and tesamorelin together?

Yes, they can be prescribed together, and they often are. They act on different systems, one at the cell's fuel sensor and one through the pituitary, so the rationale prescribers give is two routes to the same body-composition goal. What is not known is whether the pair does more than either alone. No study has tested the combination, in humans or in animals.

  • Can MOTS-c and tesamorelin go in the same vial?

Yes, when a licensed pharmacy compounds them together. Three Pepti blends contain both:

Blend What else is in it MOTS-c per dose Tesamorelin per dose Directions Price
PHYSIQ Ipamorelin and AOD-9604 0.5 mg 0.5 mg 0.25 mL weekday nights at bedtime $289/mo
FUSION BPC-157 and 5-Amino-1MQ 0.5 mg 0.75 mg 0.25 mL once daily, Monday to Friday $279/mo
SYNAPSE Semax and Selank 0.5 mg 0.5 mg 0.25 mL once daily, Monday to Friday $279/mo

Each is a 5 mL multi-dose vial with 20 doses. The proportions are fixed by the pharmacy's formulation and cannot be adjusted per component; if a physician wants more of one and less of another, the components are prescribed separately.

The blends differ in their brief. PHYSIQ is an even split built around body composition, adding a second growth hormone route (ipamorelin) and AOD-9604. FUSION carries the higher tesamorelin strength and adds BPC-157 and 5-Amino-1MQ. SYNAPSE pairs the metabolic pair with two neuropeptides for focus and stress, and by mass is two-thirds MOTS-c and tesamorelin.

  • Do not mix your own vials

Drawing MOTS-c and tesamorelin from two separate vials into one syringe, or combining vials at home, is not the same as a pharmacy-compounded blend. If you want both in one injection, ask for one of the blends above. If they are prescribed as two separate vials, take each as printed on its own label.

  • The timing question

MOTS-c on its own is directed in the morning, near the day's activity, because some people report mild alertness after a dose. Tesamorelin on its own is directed at bedtime, so its pulse lands on the natural growth hormone pulse of slow-wave sleep. The blends resolve that differently: PHYSIQ is dosed at bedtime, and for SYNAPSE some prescribers direct evening dosing and others morning. Follow what is printed on your vial.

Where each stands with the FDA

MOTS-c Tesamorelin
Approved product None Approved November 2010 for excess abdominal fat in adults with HIV-associated lipodystrophy
503A bulks list Category 2 in 2023; removed from Category 2 on 15 April 2026 Never on it; the approved product is licensed as a biologic

Final FDA determination Pending Not applicable
Compounded status Legal to prescribe and dispense; not FDA approved Legal to prescribe; not FDA approved; prescribed off-label

An advisory committee vote is a recommendation, not a decision. Compounded medications are not FDA approved, and that includes every blend on this page. Are peptides FDA approved explains the difference between not approved and not legal.

MOTS-c vs Tesamorelin for Fat Loss

Dosing, side by side

Both are injected under the skin (subcutaneously), in the abdomen or thigh, rotating sites, at the same volume: 0.25 mL, which is 25 units on a U-100 insulin syringe, Monday through Friday with weekends off. From the 10 mg vials, that delivers 0.5 mg of either. A 15 mg tesamorelin strength delivers 0.75 mg in the same volume, and MOTS-c comes in several strengths and as a pre-filled Pen cartridge.

The difference is the time of day: MOTS-c mornings, tesamorelin bedtime. You do not need to calculate anything: your physician sets your directions after reviewing your intake, and your directions are printed on the vial. Twenty doses per vial cover four weeks, which is why refills run every 28 days, one vial at a time. Both ship refrigerated with ice packs and stay in the fridge.

Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.

Side effects

Tesamorelin. On the approved product's label, at 2 mg daily, the reactions more frequent than placebo were injection-site reactions, joint aches, muscle aches, peripheral swelling and tingling. Fluid-related effects are classic growth hormone effects and usually mean the dose is higher than needed. Glucose can rise; the label records new-onset diabetes in 5% of treated patients against 1% on placebo.

MOTS-c. There is no human safety data from trials. What gets reported clinically is mostly injection-site redness, mild soreness or bruising, and sometimes transient jitteriness, a faster pulse or mild light-headedness in the hour or two after a morning dose, particularly early in a course. Anyone taking something that lowers blood glucose should say so.

For both, stop and contact your physician for any reaction that is severe, spreading, or involves difficulty breathing.

MOTS-c vs Tesamorelin for Fat Loss

Who should not take either, or should discuss it first

Situation Why
Pregnancy or breastfeeding Tesamorelin is contraindicated; MOTS-c has no safety data
Pituitary disorder, pituitary surgery or head irradiation Tesamorelin needs a pituitary that can respond
Active cancer Contraindicated for tesamorelin on the approved label; see can peptides cause cancer
Diabetes or borderline fasting glucose Tesamorelin can raise glucose; MOTS-c acts on glucose handling in animal work
Tested athletes Both are prohibited: MOTS-c at all times as a metabolic modulator, tesamorelin as a GHRH analogue

MOTS-c vs Tesamorelin for Fat Loss

What the evidence shows, honestly

Tesamorelin has randomised human trials that reported a reduction in visceral fat, mostly in people with HIV, at 1 to 2 mg daily, with no meaningful change in body weight and visceral fat returning toward baseline after stopping. MOTS-c has a coherent mouse and cell story, observational human data on the body's own levels, and no human treatment trial. No study has tested them together or tested any blend that contains both. Any provider describing either as a fat-loss drug, or attaching a number or a timeline to either, is overstating what is known.

Where to get MOTS-c or tesamorelin prescribed

Neither can be bought legitimately without a prescription. Sites selling them "for research use only" are not accountable for what is in the vial: identity, purity, sterility and concentration are untested. The prescription route:

  1. Complete a free online intake covering your history, medications and what you want help with.
  2. A physician licensed in your state reviews it, usually within 24 hours.
  3. If it is appropriate, a state-licensed, FDA-registered US pharmacy compounds it to that prescription. Lot test results are published at /quality/lab-results.
  4. It ships refrigerated with your directions printed on the vial.
  5. Your physician stays reachable for questions and dose changes.

Pepti prescribes both in all 50 states and DC. Step-by-step pages: how to get MOTS-c prescribed online and how to get tesamorelin prescribed online.
