SYNAPSE Blend for Men: What the Research Has Looked At, and What to Check First

This page covers what SYNAPSE is, what research has and has not looked at in relation to men, the questions men most often ask (testosterone, TRT, stimulants, focus, the waist, sport), what a physician rules out first, and how Pepti supplies it. The full reference is what is the SYNAPSE blend, and the evidence component by component is in SYNAPSE blend benefits. The companion page is SYNAPSE blend for women.

SYNAPSE Blend (Semax / Selank / MOTS-c / Tesamorelin)
SYNAPSE Blend (Semax / Selank / MOTS-c / Tesamorelin)$279/mo

SYNAPSE has never been studied as a blend, in men or anyone, so everything said about it for men comes from its four components, and only two of them have anything specific to men. Tesamorelin's pivotal 2007 visceral fat trial enrolled 412 people with HIV and abdominal fat accumulation, 86 percent of them men, and a 2021 genetic study linked a natural MOTS-c variant to a higher prevalence of type 2 diabetes in men; Semax and Selank have no results reported separately for men. SYNAPSE is not a testosterone booster, both tesamorelin and MOTS-c are named on the 2026 WADA Prohibited List, and tesamorelin's effect on blood sugar is the main medical caution. Compounded SYNAPSE is a prescription injection and is not FDA approved.

SYNAPSE Blend for Men

The short answer

Question What the live research shows Strength of evidence
Has SYNAPSE been studied in men? No. The blend has never been studied in anyone None
Tesamorelin and visceral fat Two phase 3 RCTs, 806 patients pooled; the pivotal trial was 86% men, almost all with HIV-associated fat redistribution Strong for that population and a 2 mg daily dose
MOTS-c in men A genetic variant associated with type 2 diabetes in men, particularly sedentary men (Aging, 2021) Observational; nobody was given MOTS-c
Semax and Selank in men No results reported separately for men; one 2024 Semax stress study used male rats No male-specific human data
Does it raise testosterone? No component is described as acting on testosterone Not a testosterone booster
Focus or cognition One tesamorelin trial in adults aged 55 to 87 at 1 mg nightly; no trial of the blend A single trial of one component
Tested sport Tesamorelin (S2.2.4) and MOTS-c (S4.4.1) are named on the 2026 WADA List, prohibited at all times Clear exclusion

The honest summary: one component with good trials in a mostly male but very specific population, one genetic signal in men, and nothing male-specific for the neuropeptides.

SYNAPSE Blend for Men

What SYNAPSE is, in one paragraph

SYNAPSE is four separately studied peptides dissolved together in one 5 mL vial by a compounding pharmacy: 5 mg Semax, 5 mg Selank, 10 mg MOTS-c and 10 mg tesamorelin. Each 0.25 mL dose carries 0.25 mg Semax, 0.25 mg Selank, 0.5 mg MOTS-c and 0.5 mg tesamorelin, a fixed split. The rationale prescribers give is a brain and body pairing: Semax and Selank are described acting on attention and the stress response, MOTS-c on cellular fuel sensing through AMPK, and tesamorelin on the pituitary's own growth hormone pulse. By mass, two-thirds of the vial is the metabolic pair. That is a rationale, not a finding; no study has looked at what the four do together.

Component Per dose What the description rests on
Semax 0.25 mg Rat BDNF work; Russian stroke trials, intranasal
Selank 0.25 mg Rat and cell GABA work; Russian anxiety-disorder trials, intranasal
MOTS-c 0.5 mg Mouse and cell AMPK work; one observational human exercise finding
Tesamorelin 0.5 mg Randomised placebo-controlled human trials of the approved product

What research has looked at in relation to men

  • Tesamorelin: the trials that enrolled mostly men

The first phase 3 trial, in the New England Journal of Medicine in 2007, randomised 412 people with HIV and abdominal fat accumulation, 86 percent men, to tesamorelin 2 mg daily or placebo for 26 weeks. Visceral fat on CT fell 15.2 percent on treatment and rose 5.0 percent on placebo, and IGF-1 rose 81 percent. A pooled analysis with a second trial covered 806 patients. Every trial that measured it found no significant change in the fat under the skin.

For men. These trials describe men more than women because of who enrolled, but they describe HIV-associated fat redistribution at 2 mg daily, four times the tesamorelin in a SYNAPSE dose. One 2012 trial of 60 abdominally obese adults with reduced growth hormone secretion reported the same selective effect outside HIV. None of it is a trial of SYNAPSE, and none measured weight or appearance. More in tesamorelin for men.

  • MOTS-c: a genetic finding in men

A 2021 paper in Aging looked at an East Asian mitochondrial DNA variant that changes one amino acid in MOTS-c. Across three cohorts totalling 27,527 people, the variant was associated with a higher prevalence of type 2 diabetes in men, particularly sedentary men, and in mice and cells the variant peptide lost its insulin-sensitising activity. That is what a natural variant correlates with, not what an injection does. There are no randomised trials of MOTS-c in people of either sex. More in MOTS-c for men.

  • Semax and Selank: no male-specific results

Semax's human evidence is Russian clinical work in stroke and cerebrovascular disease using a nasal product, plus a 14-person brain-imaging study; the one male-specific study in the live research is a 2024 experiment in male rats under chronic stress. Selank's human evidence is a set of small Russian trials in patients with anxiety disorders, compared with benzodiazepines. Neither reports separate results for men, and neither tested the subcutaneous route. See Semax for men and Selank for men.

  • The one cognition trial

The reason tesamorelin sits in a blend with this name is a 2012 Archives of Neurology trial: 152 adults aged 55 to 87, tesamorelin 1 mg or placebo at bedtime for 20 weeks, with a favourable effect on a cognitive composite driven mainly by executive function. It was tesamorelin alone, in older adults, at twice the SYNAPSE dose, and the authors called for longer trials. It is not evidence about this blend or about younger men.

SYNAPSE Blend for Men

What changes for men

  • It is not a testosterone booster

No component of SYNAPSE is described as acting on testosterone. Tesamorelin acts on the growth hormone axis, MOTS-c on a cellular energy pathway, and Semax and Selank on brain signalling; none is a steroid or anabolic agent. Low drive, poor focus and a softening waist can point at genuinely low testosterone, which is a different treatment with its own monitoring. See peptides for testosterone.

  • It is not a weight-loss product

Tesamorelin's approved label states it is not indicated for weight-loss management because its effect on weight is neutral, and MOTS-c has no human trials. Where weight itself is the problem, the GLP-1 medications have the trial evidence. If body composition is the whole goal and focus is not, FUSION and PHYSIQ carry tesamorelin and MOTS-c without the neuropeptides.

  • Alongside testosterone therapy

No study has looked at SYNAPSE or any component with testosterone replacement, and no interaction list exists. Tesamorelin and testosterone move overlapping markers: IGF-1 is tracked on the growth hormone side, and testosterone can raise IGF-1 too. Glucose is the marker they pull on in opposite directions. List your testosterone prescription, your dose and whoever manages it on your intake. More in can you take peptides with testosterone.

  • Stimulants and ADHD medication

A mouse study cited in the FDA's Semax briefing reported Semax potentiating amphetamine-induced dopamine release. No equivalent human finding exists. If you take a stimulant, including prescription ADHD medication, your physician needs to know before starting, and SYNAPSE is not a substitute for that treatment.

  • Blood sugar

Growth hormone reduces insulin sensitivity, and the approved tesamorelin label records new-onset diabetes in 5% of treated patients against 1% on placebo at 2 mg daily. MOTS-c is described in mice improving insulin sensitivity. Nobody has measured the two together. Diabetes, prediabetes or a borderline fasting glucose belongs on the intake, and thirst or frequent urination is a reason to message your care team.

  • Cancer history and the prostate

Tesamorelin raises growth hormone and IGF-1. The approved product is contraindicated in active malignancy, requires any prior malignancy to be inactive with treatment complete, and states that the effects of prolonged IGF-1 elevation are unknown. A personal or family history of prostate or any other cancer goes on the intake in full. See can peptides cause cancer.

  • Fertility

No study in the live research has looked at any SYNAPSE component and male fertility, so the effect is unknown rather than shown to be nil. If conception is a goal, raise it with your physician before starting. See do peptides affect fertility.

  • Tested sport

The 2026 WADA Prohibited List names tesamorelin under S2.2.4 and MOTS-c under S4.4.1, both prohibited at all times, in and out of competition. USADA has stated MOTS-c is not eligible for a therapeutic use exemption. A prescription does not change that, and the small amount in a blend does not matter. See is SYNAPSE blend banned in sports.

Men over 40: what a physician rules out first

"Flat, unfocused and softer round the middle" has common, checkable causes, each with a treatment that is not SYNAPSE.

What it is How it shows up Why it comes first
Obstructive sleep apnoea Snoring, waking unrefreshed, daytime fog Lowers growth hormone and testosterone; fluid retention from growth hormone can complicate it
Genuinely low testosterone Low desire, lost muscle, low mood A different treatment with its own monitoring
Early diabetes Thirst, fatigue, central fat Tesamorelin can raise blood sugar; glucose comes first
Thyroid disease or low B12 Fatigue, brain fog, weight change Not fixed by any peptide
Depression, anxiety or ADHD Poor focus, low drive, stress Established treatments exist; SYNAPSE is not one
Alcohol intake Fragmented sleep, central fat, poor focus A common reversible cause

SYNAPSE Blend for Men

What the evidence does not establish

  • Any effect of SYNAPSE in men. The blend has never been studied.
  • That the four together do more than any one alone.
  • A focus or memory effect in healthy men. The one cognition trial was tesamorelin alone in adults aged 55 to 87.
  • Visceral fat reduction in healthy men with a normal pituitary at 0.5 mg on weekdays.
  • Any effect on testosterone, libido or fertility. None was measured.
  • Long-term safety over years of continuous use.

Side effects

Nothing in the live research reports SYNAPSE's side effects by sex. What gets reported clinically is mostly at the injection site.

What is reported Usually attributed to What to do
Injection-site redness, mild soreness, occasional bruising Any injection; tesamorelin's label records it most Rotate sites
Joint aches, swelling in the hands or feet, tingling Tesamorelin (fluid-related growth hormone effects) Message your care team
Thirst or frequent urination A possible rise in blood sugar (tesamorelin) Message your care team
Brief jitteriness, a faster pulse or light-headedness after a dose MOTS-c Usually settles; report it if not
Restlessness if dosed late in the day Semax Tell your care team; do not change timing yourself
Facial or throat swelling, widespread hives, breathing difficulty Possible with any injected medication Call 911 first, then tell your physician

The full list is in SYNAPSE blend side effects. Do not change your dose yourself.

SYNAPSE Blend for Men

How Pepti supplies SYNAPSE

Product What is in it Typical directions Price
SYNAPSE Blend 5 mL: Semax 1 mg/mL, Selank 1 mg/mL, MOTS-c 2 mg/mL, tesamorelin 2 mg/mL 0.25 mL (25 units) subcutaneously once daily Monday through Friday $279/mo
Semax + Selank 1 mg Semax and 1 mg Selank Printed on the vial $259/mo
Tesamorelin 5 mL, 2 mg/mL (10 mg) 0.25 mL (25 units) Monday through Friday nights at bedtime $249/mo
MOTS-c 5 mL, 2 mg/mL (10 mg) 0.25 mL (25 units) Monday through Friday mornings $259/mo

A SYNAPSE vial holds 20 doses, about four weeks, which is why refills run every 28 days, one vial per fill. Every price is all-in, covering medication, physician review, refill management and shipping, with no membership fee. Pepti is cash-pay and does not bill insurance. Vials arrive ready to use with sterile syringes and alcohol prep pads, shipped refrigerated with ice packs. Your physician sets your directions after reviewing your intake, and they are printed on the vial. The arithmetic is in SYNAPSE blend dosage.

SYNAPSE is compounded; compounded medications are not FDA approved. Tesamorelin is the active ingredient of an FDA-approved product, but compounded tesamorelin is not that product. In July 2026 the FDA's Pharmacy Compounding Advisory Committee voted to recommend adding Semax and MOTS-c to the 503A Bulks List, with the final determinations pending, and Selank's nomination was withdrawn.

Where to get SYNAPSE prescribed

SYNAPSE cannot be bought legitimately without a prescription. Sites selling multi-peptide vials "for research use only" are not accountable for identity, purity, sterility or concentration. The prescription route:

  1. Complete a free online intake covering your history, including glucose, cancer history, any testosterone therapy, any stimulant or psychiatric medication, and whether you are drug tested.
  2. A physician licensed in your state reviews it, usually within 24 hours.
  3. If it is appropriate, a PCAB-accredited, FDA-registered US 503A compounding pharmacy prepares it to that prescription. Lot test results are at /quality/lab-results.
  4. It ships refrigerated with your directions printed on the vial.
  5. Your care team stays reachable for questions and side effects.

Step by step: how to get SYNAPSE blend prescribed online. Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.
