Tesamorelin (Injectable)
Tesamorelin$249/mo

Tesamorelin's best-documented effect is a reduction in visceral fat, the fat around the organs, measured on CT in randomized placebo-controlled trials; the pivotal 2007 trial enrolled 412 people with HIV and abdominal fat accumulation, 86 percent of them men, and visceral fat fell 15.2 percent on tesamorelin against a 5 percent rise on placebo. That makes men the group its evidence describes best, but mostly men with HIV-associated fat redistribution, not healthy men with a softening waist. The approved tesamorelin product's label calls it weight-neutral and not indicated for weight loss, it is not a testosterone booster, and growth hormone reduces insulin sensitivity, so glucose matters. Compounded tesamorelin shares the active peptide, not the approval, and is not FDA approved.

Tesamorelin for Men

The short answer

Question What the live research shows Strength of evidence
Visceral fat Two phase 3 RCTs, 806 patients pooled, adults with HIV and abdominal fat; the pivotal trial was 86% men Strong for that population and dose
Visceral fat without HIV One 12-month RCT of 60 abdominally obese adults with reduced growth hormone secretion (2012) Single trial
Weight on the scale The approved label calls it weight-neutral Evidence says no
Does it raise testosterone? It acts on the growth hormone axis, not the testicular axis Not a testosterone booster
Muscle and strength Not an approved or trial-tested use; a related GHRH analogue trial reported lean-mass gains in men only (1997) Indirect, small, a different molecule
Alongside TRT No randomized trial of tesamorelin with testosterone replacement None
Blood sugar New-onset diabetes in 5% against 1% on placebo, at 2 mg daily A label warning
Tested sport Named under S2.2.4 of the 2026 WADA Prohibited List, prohibited at all times Clear exclusion

Tesamorelin for Men

What tesamorelin is, in one paragraph

Growth-hormone-releasing hormone (GHRH) is the 44-amino-acid signal the hypothalamus sends to the pituitary to trigger a growth hormone pulse. Tesamorelin is that full sequence with a trans-3-hexenoic acid group attached, which protects it from the enzyme that otherwise cuts native GHRH apart within minutes. It prompts release of your own growth hormone, with the body's feedback brakes, somatostatin and IGF-1, still working. It contains no growth hormone, is not a GLP-1 medication, does not act on appetite, and does not act on testosterone production. A tesamorelin product was approved by the FDA in November 2010 for one narrow use: reducing excess abdominal fat in adults with HIV who have lipodystrophy.

Tesamorelin for Men

What research has looked at, and what it means for men

  • The visceral fat trials

The first phase 3 trial, in the New England Journal of Medicine in 2007, randomized 412 people with HIV and abdominal fat accumulation, 86 percent men, to tesamorelin 2 mg daily or placebo for 26 weeks. Visceral fat on CT fell 15.2 percent on treatment and rose 5.0 percent on placebo, triglycerides fell, IGF-1 rose 81 percent, and glycemic measures did not differ. A pooled analysis of that trial and a second of the same design, in the Journal of Clinical Endocrinology and Metabolism in 2010, covered 806 patients and reported a treatment effect of about 15% at week 26, maintained to 52 weeks in those who continued and returning toward baseline in those switched to placebo. Every trial that measured it found no significant change in the fat under the skin.

For men. These trials describe men more than women simply because of who enrolled. They still describe a specific population, HIV-associated fat redistribution, at 2 mg daily, which is four times Pepti's typical compounded direction. They measured the deeper fat on imaging, not appearance, weight or a six-pack.

  • The one trial outside HIV

A 2012 trial in the Journal of Clinical Endocrinology and Metabolism gave 60 abdominally obese adults with reduced growth hormone secretion tesamorelin 2 mg or placebo for 12 months. It reported a selective reduction in visceral fat with no effect on subcutaneous fat, improved triglycerides and C-reactive protein, and no worsening of glucose. It is the most direct evidence that the visceral-fat finding belongs to the drug rather than to HIV. It is also one trial, in people selected for low growth hormone secretion, and a man with a normal pituitary is not quite that population.

  • Liver fat and cognition

A 2014 JAMA trial (50 people) and a 2019 Lancet HIV trial (61 people) in people with HIV reported less liver fat. A 2012 Archives of Neurology trial randomized 152 adults aged 55 to 87 to tesamorelin 1 mg or placebo at bedtime for 20 weeks and reported a favorable effect on a cognitive composite, alongside a 117 percent rise in IGF-1 and a 7.4 percent fall in body fat. None is an approved use.

  • Why growth hormone and the male waist come up together

Growth hormone is released in bursts, the largest during deep sleep. In healthy men sampled around the clock, daily secretion fell by roughly 14 percent per decade, and at any given age each extra unit of body mass index cut it by a further 6 percent or so (Journal of Clinical Endocrinology and Metabolism, 1991). The man who has gained weight since his thirties has lost growth hormone signaling on two counts, and visceral fat is the most growth-hormone-responsive fat in the body. That is the logic for a GHRH analogue in men. It is logic, not a trial in healthy men.

  • The one sex-specific finding in the class

The clearest sex-specific result for a GHRH analogue comes from a related molecule, not tesamorelin. In a 1997 trial in the Journal of Clinical Endocrinology and Metabolism, ten women and nine men aged 55 to 71 self-injected a GHRH(1-29) analogue nightly for 16 weeks. Growth hormone and IGF-1 rose in both sexes, but the increases in lean mass and insulin sensitivity were seen in men only. It is nineteen people, a different molecule, and the investigators said more work was needed.

What changes for men

  • It is not a testosterone booster

Tesamorelin acts on the growth hormone axis. It is not described as moving testosterone, and growth-hormone-axis peptides do not raise it. If two morning draws confirm low testosterone alongside symptoms, that is hypogonadism, treated with testosterone under its own monitoring, not with a GHRH analogue. Peptides for testosterone covers what has and has not been studied.

  • It is not a weight-loss drug

The approved label says plainly that tesamorelin is not indicated for weight-loss management because its effect on weight is neutral. A man expecting the scale to move has picked the wrong tool; where weight itself is the problem, the GLP-1 medications have the trial evidence. A tape measure at the navel, taken the same way each month, matches what the trials measured better than a scale does. The comparison is in tesamorelin vs semaglutide.

  • Alongside testosterone therapy

Tesamorelin is prescribed alongside TRT where visceral fat is the specific goal. The two treatments move overlapping markers, so every prescriber involved should know about the other. IGF-1 is what the growth hormone side is tracked on, and testosterone can raise IGF-1 too, so an IGF-1 that has moved in a man taking both is not automatically the peptide's doing. Glucose is the marker the two pull on in opposite directions: growth hormone reduces insulin sensitivity, while androgens may lower blood glucose in diabetic patients. There is no randomized trial of tesamorelin given alongside testosterone replacement, and no established interaction list. The trials of recombinant growth hormone plus testosterone in older men used the hormone itself, not a GHRH analogue, and cannot be read as evidence for tesamorelin. More in can you take peptides with testosterone.

  • Blood sugar

The approved label records HbA1c of 6.5% or higher in 5% of treated patients against 1% on placebo over 26 weeks, and directs that glucose status be evaluated before starting and during treatment. A prediabetic HbA1c does not exclude tesamorelin, but it is a different prescribing decision. Diabetes, prediabetes or a borderline fasting glucose belongs on the intake.

  • Cancer history and the prostate

Tesamorelin raises growth hormone and IGF-1. The approved label contraindicates it in active malignancy, requires any prior malignancy to be inactive with treatment complete, and states that the effects of prolonged IGF-1 elevation are unknown. Growth-hormone-axis peptides are not described as moving PSA, but a 2004 meta-regression in The Lancet reported associations between circulating IGF-1 and the risk of certain common cancers in observational cohorts. That is an association, not a demonstrated effect of treatment, and it is why a personal or family history of prostate or any other cancer goes on the intake in full.

  • The pituitary and sleep apnoea

The approved product is contraindicated where the hypothalamic-pituitary axis is disrupted, including pituitary surgery, head irradiation or head trauma. Sleep apnoea is worth flagging separately: it lowers growth hormone and testosterone, and the fluid retention on the growth hormone side can complicate it.

  • Fertility

No study in the live research has looked at tesamorelin and male fertility. If conception is a goal, raise it with your physician before starting. See do peptides affect fertility.

  • Tested sport

The 2026 WADA Prohibited List names tesamorelin as a prohibited GHRH analogue, prohibited at all times. If you are tested by any body, tell your physician before starting.

Men over 40: what a physician rules out first

"Low energy and a soft middle" can be sleep apnoea, low testosterone, early diabetes or alcohol, each with a treatment that is not tesamorelin.

What it is How it shows up Why it comes first
Obstructive sleep apnoea Snoring, waking unrefreshed, low testosterone Lowers IGF-1 and testosterone in proportion to severity
Genuinely low testosterone Low desire, poor morning erections, lost muscle A different treatment with its own monitoring
Early diabetes Thirst, fatigue, central fat Growth hormone reduces insulin sensitivity; glucose comes first
Thyroid disease Fatigue, weight change, cold intolerance Not fixed by any peptide
Alcohol intake Fragmented sleep, central fat A common reversible cause

More is in peptides for men over 40.

Side effects

From the approved product's label, pooled from 740 treated patients, at 2 mg daily, four times the standard compounded dose. The live research does not report these by sex.

Side effect On tesamorelin On placebo
Injection-site reactions 17% 6%
Joint pain 13% 11%
Pain in an arm or leg 6% Less often
Muscle aches 6% Less often
Peripheral swelling 6% Less often
Tingling or numbness 5% Less often
Hypersensitivity: itching, flushing, hives, rash 4% Not stated
New-onset diabetes 5% 1%

Puffiness, joint aches and hand tingling are classic growth hormone effects; the right response is a message to your care team, not pushing through and not changing the dose yourself. For facial or throat swelling, spreading hives or difficulty breathing, call 911, then tell your physician. The full list is in tesamorelin side effects.

How Pepti supplies tesamorelin

Product What is in it Typical directions Price
Tesamorelin 5 mL multi-dose vial, 2 mg/mL (10 mg tesamorelin) 0.25 mL (25 units) subcutaneously Monday through Friday nights at bedtime $249/mo
Tesamorelin + Ipamorelin 10 mg tesamorelin and 5 mg ipamorelin in 5 mL 0.25 mL (25 units) Monday through Friday nights at bedtime $259/mo
TITAN Blend 10 mg tesamorelin, 6 mg CJC-1295, 12 mg ipamorelin, 10 mg BPC-157 in 5 mL 0.25 mL (25 units) Monday through Friday nights at bedtime $289/mo
PHYSIQ Blend 10 mg each of tesamorelin, ipamorelin, AOD-9604 and MOTS-c in 5 mL 0.25 mL (25 units) Monday through Friday nights at bedtime $289/mo

On the tesamorelin vial, 0.25 mL is 0.5 mg per dose and 20 doses per vial, about four weeks, which is why refills run every 28 days, one vial per fill. TITAN adds BPC-157 for the tendon or joint that limits training; no trial has tested any blend. The arithmetic is in tesamorelin dosage. Your physician sets your directions after reviewing your intake, and they are printed on the vial.

Every price is all-in, covering medication, physician review, refill management and shipping, with no membership fee. Pepti is cash-pay and does not bill insurance. Vials arrive ready to use with sterile syringes and alcohol prep pads, shipped refrigerated with ice packs. Lot test results are at /quality/lab-results.

Common questions

Tesamorelin for Men, answered.

Its trials reported reduced visceral fat, and the pivotal trial was 86 percent men, but almost all of them had HIV-associated fat redistribution. One 12-month trial in abdominally obese adults with reduced growth hormone secretion reported the same selective effect. No trial has tested it in healthy men with a normal pituitary.
