Sermorelin Dosage: How Pepti Prescribes It, in Units, mL and mg
This page explains the sermorelin dosage Pepti prescribes, what the units on the syringe mean, a dosage chart that converts units to millilitres and milligrams, when sermorelin is taken, how long a vial lasts, what can change your directions, and the side effects to know about. It covers how Pepti's sermorelin is typically prescribed, not dosing protocols from the internet. Nothing here is a reason to change your own dose.

At Pepti, sermorelin is prescribed as 0.25 mL, which is 25 units on a U-100 insulin syringe, injected under the skin Monday through Friday nights. The vial is 5 mL at 2 mg/mL, so each dose carries 0.5 mg of sermorelin and the vial holds 20 doses, about four weeks. Your physician sets your directions after reviewing your intake, and your exact dose and units are printed on the vial. The Ipamorelin + Sermorelin vial uses the same 25 unit draw, five nights a week at bedtime.

Sermorelin Dosage
The short answer
| Product | Vial | Typical directions | Sermorelin per dose | Doses per vial | Price |
|---|---|---|---|---|---|
| Sermorelin | 5 mL, 2 mg/mL (10 mg per vial) | Inject 0.25 mL (25 units) subcutaneously Monday through Friday nights | 0.5 mg | 20 | $229/mo |
| Ipamorelin + Sermorelin | 5 mL, ipamorelin 2 mg/mL + sermorelin 2 mg/mL (10 mg of each per vial) | Inject 0.25 mL (25 units) subcutaneously Monday through Friday nights at bedtime | 0.5 mg, with 0.5 mg ipamorelin | 20 | $219/mo |
Every price is all-in, covering medication, physician review, refill management and shipping, with no separate membership fee. Pepti is cash-pay and does not bill insurance. These are the typical printed directions; your own directions are the ones on your vial.
Sermorelin Dosage
Sermorelin dosage in units, mL and mg
Pepti's directions are written in two forms, millilitres and units, because prescriptions are written in millilitres and insulin syringes are marked in units. On a U-100 insulin syringe, 1 unit is 0.01 mL, so 100 units is 1 mL and 25 units is 0.25 mL. A unit is a volume mark on the barrel, not a measure of how strong the medication is. "Sermorelin units" is one of the most searched questions about this peptide, and the answer is that units only mean something alongside the concentration on the label.
The milligram figure comes from that concentration. Pepti's sermorelin is supplied at 2 mg/mL, so:
| What you read | Value | Where it comes from |
|---|---|---|
| Dose volume | 0.25 mL | Printed on the vial |
| On a U-100 insulin syringe | 25 units | 0.25 mL x 100 units per mL |
| Concentration | 2 mg/mL | Printed on the vial |
| Sermorelin per dose | 0.5 mg | 2 mg/mL x 0.25 mL |
| Total in the vial | 10 mg in 5 mL | Printed on the vial |
| Doses per vial | 20 | 5 mL divided by 0.25 mL |
That arithmetic is a way to read a label, not a way to set a dose. A vial labelled 10 mg is not a 10 mg dose: it is twenty 0.5 mg doses. If your label and your syringe seem to disagree, do not dose; message your care team. How long does a peptide vial last explains every number on a vial label in more detail.
Sermorelin Dosage
Sermorelin dosage chart: units to mL to mg
People search for a "sermorelin dosage chart" because online charts quote doses in micrograms, milligrams and units interchangeably, and the numbers only line up if you know the concentration. The chart below is the conversion for a 2 mg/mL vial, drawn with a U-100 insulin syringe. It is math only. It is not a menu of doses, and the only line that applies to you is the one that matches the number printed on your vial.
| Syringe mark (U-100) | Volume | Sermorelin from a 2 mg/mL vial |
|---|---|---|
| 5 units | 0.05 mL | 0.1 mg |
| 10 units | 0.1 mL | 0.2 mg |
| 15 units | 0.15 mL | 0.3 mg |
| 20 units | 0.2 mL | 0.4 mg |
| 25 units (Pepti's typical directions) | 0.25 mL | 0.5 mg |
| 30 units | 0.3 mL | 0.6 mg |
| 40 units | 0.4 mL | 0.8 mg |
| 50 units | 0.5 mL | 1 mg |
| 100 units | 1 mL | 2 mg |
The rule behind every row: units divided by 100 gives millilitres, and millilitres multiplied by 2 gives milligrams. The same conversion applies to the sermorelin in the Ipamorelin + Sermorelin vial, because it is also at 2 mg/mL; each 25 unit draw from that vial delivers 0.5 mg of sermorelin and 0.5 mg of ipamorelin together, in fixed 1:1 proportion. There is no way to take more of one and less of the other from the same vial.
A chart from another source, written for a different concentration, will give different milligram figures for the same units. That is the main reason not to dose from a chart, including this one. More on the pairing is in what is ipamorelin + sermorelin.
The Pen
Sermorelin also comes as a 3 mL pre-filled pepti Pen cartridge containing 9 mg of sermorelin, used in a reusable click-dial injector. It is dosed by the pen's dial rather than drawn with a syringe, so the units chart above does not apply to it; the directions come printed with your prescription. It is the same medication, and the comparison is in vial vs pen. The Ipamorelin + Sermorelin product is a vial only.
Sermorelin Dosage
How long a sermorelin vial lasts
At Pepti's typical directions, one 10 mg vial lasts four weeks:
| Step | Value |
|---|---|
| Dose | 25 units, 0.25 mL, 0.5 mg |
| Doses per week | 5 (Monday through Friday) |
| Doses per vial | 20 (5 mL divided by 0.25 mL) |
| Sermorelin used | 20 x 0.5 mg = 10 mg, the whole vial |
| Time per vial | 20 doses divided by 5 per week = 4 weeks |
Four weeks is why refills run every 28 days, one vial per fill. Vials are multi-dose solutions shipped ready to use, refrigerated with ice packs, with sterile syringes and alcohol prep pads. Nothing is mixed at home. Keep the vial in the fridge between doses, and follow the beyond-use date printed on the label, which runs from first puncture. How to store peptides covers storage.
If a vial runs short before the refill, message your care team rather than adjusting anything yourself. If you miss a dose, follow your directions and do not take two doses to make up for one.
Sermorelin Dosage
When to take sermorelin
At night
Pepti's sermorelin directions say Monday through Friday nights, and the Ipamorelin + Sermorelin directions say nights at bedtime. The reason is physiology: growth hormone is released in pulses, and the largest natural pulse occurs in the first hours of deep sleep. Sermorelin prompts your pituitary to release its own growth hormone in a short pulse, and it is cleared within minutes, so a dose at night places that pulse alongside the one the body is already producing. The paediatric trial behind sermorelin's original approval also used a once-daily bedtime injection.
Monday through Friday
The five-nights-on, two-off pattern is a prescriber convention rather than a schedule derived from trials. Prescribers commonly build in two nights off each week, and describe the gap as a way of keeping the pituitary responsive. It also makes the vial arithmetic clean: 20 doses at five a week is exactly four weeks. Weekends off are part of the schedule, not missed doses.
Under the skin
Subcutaneous means into the fat layer, not the muscle. Abdomen and thigh are the usual sites, rotated so the same spot is not used repeatedly. Technique is covered in how to inject peptides safely.
Directions say What it means in practice 0.25 mL (25 units) Draw to the 25 mark on a U-100 insulin syringe Subcutaneously Into the fat layer, abdomen or thigh, rotating sites Monday through Friday Five doses a week, weekends off Nights (at bedtime, on the Ipamorelin + Sermorelin label) In the evening, shortly before sleep 20 doses per vial About four weeks, then the next refill
Sermorelin Dosage
What the trials used, and why that is not your dose
Sermorelin has more human data than most compounded peptides, and the doses in those trials are often quoted online as if they were adult protocols. They were not designed as one:
| Study | Who | How it was dosed |
|---|---|---|
| Paediatric trial, Journal of Clinical Endocrinology and Metabolism, 1996 | 110 prepubertal children with growth hormone deficiency | 30 µg/kg subcutaneously at bedtime, once daily, up to a year |
| National Institute on Aging, Journal of Clinical Endocrinology and Metabolism, 1992 | 10 healthy men around age 68 | Twice daily for 14 days at each of two doses; IGF-1 rose into the young-adult range only at the higher dose. At 1 mg twice daily, 24-hour growth hormone and IGF-1 were not significantly different from men in their twenties |
| Johns Hopkins, Metabolism, 1997 | 11 healthy men aged 64 to 76 | 2 mg as a single nightly injection for six weeks; nocturnal growth hormone rose, IGF-1 did not |
| UC San Diego, Journal of Clinical Endocrinology and Metabolism, 1997 | 19 men and women aged 55 to 71 | A related GHRH(1-29) analogue nightly for 16 weeks; IGF-1 rose within two weeks, then drifted back toward baseline |
The honest reading is that there is no human dose-ranging study behind the adult schedules in common use; the two-dose, fourteen-day 1992 study is the closest the literature comes. These trials measured hormone levels and laboratory endpoints, not how people look, sleep or recover, and the populations were children and small groups of healthy older adults. That is why your directions come from a physician who has reviewed your intake, not from a study table. The evidence, benefit by benefit, is in sermorelin benefits.
Sermorelin Dosage
What changes the directions
Your physician sets your directions after reviewing your intake. Things that can lead to different directions, or a different product, include:
| What can change | How it shows up |
|---|---|
| Sermorelin alone or paired | Ipamorelin + Sermorelin has its own label, with bedtime on it |
| Vial or Pen | The Pen is dosed by dial instead of syringe units |
| Your medical history | Diabetes or impaired glucose tolerance, a cancer history, untreated sleep apnoea or a thyroid problem change the conversation |
| Your other medications | Glucocorticoids, thyroid medication, insulin and other glucose-lowering drugs all touch the growth hormone axis; the intake asks for everything you take |
| How you respond | Puffiness, joint aches or tingling hands go to your physician, who may revise your directions |
When directions change, the new numbers are printed on the new label. Never carry a number over from a previous fill. Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.
Sermorelin Dosage
Side effects, in summary
The full safety picture is in is sermorelin safe. Sermorelin has a documented adverse-event record from its years as the active ingredient of an approved drug and from the trials above, which most compounded peptides do not.
| What is reported | How often | What to do |
|---|---|---|
| Injection-site redness, itching or a small lump | Common | Rotate sites |
| Flushing or warmth shortly after a dose | Common | Usually settles; report it if marked |
| Vivid dreams with evening dosing, headache in the first weeks | Common | Report it if it persists |
| Fluid retention, joint aches, tingling hands | Uncommon | Message your care team; it usually means the dose is above what you need. Do not change the dose yourself |
| Facial or throat swelling, widespread rash, breathing difficulty | Rare | Emergency care first, then tell your physician |
The middle-to-lower rows are the ones people misread. Water retention, joint ache and hand tingling are the recognised pattern of growth hormone above your requirement, which is why they go to your physician for a dose review rather than being pushed through.
Who should not take sermorelin, or should discuss it first: anyone with an active cancer or a cancer history; anyone with diabetes or impaired glucose tolerance, because growth hormone reduces insulin sensitivity; anyone pregnant or breastfeeding, where it is not used. Tested athletes should know the 2026 WADA Prohibited List names sermorelin under S2.2.4, growth hormone releasing factors, prohibited at all times.
Sermorelin was the active ingredient of an FDA-approved drug, discontinued by its manufacturer in 2008 for commercial reasons; in 2013 the FDA determined the withdrawal was not for reasons of safety or effectiveness. It does not appear on the FDA's 503A category lists. Compounded sermorelin is not FDA approved, and compounded medications are not FDA approved.

Where to get sermorelin prescribed
Sermorelin cannot be bought legitimately without a prescription. Sites selling it "for research use only" are not accountable for what is in the vial: identity, purity, sterility and concentration are untested, so even the concentration your units chart depends on is unknown. The prescription route:
- Complete a free online intake covering your history, medications and what you want help with.
- A physician licensed in your state reviews it, usually within 24 hours.
- If it is appropriate, a PCAB-accredited, FDA-registered US 503A compounding pharmacy prepares it to that prescription. Lot test results are published at /quality/lab-results.
- It ships refrigerated with your directions printed on the vial.
- Your physician stays reachable for questions and dose changes.

Common questions
Sermorelin Dosage, answered.
At Pepti, sermorelin is typically prescribed as 0.25 mL, which is 25 units on a U-100 insulin syringe, injected subcutaneously Monday through Friday nights. From the 2 mg/mL vial that is 0.5 mg per dose. Your physician sets your directions after reviewing your intake, and your exact dose is printed on the vial.
The amount printed on your vial, on the schedule printed on your vial. Pepti's typical directions are 25 units five nights a week, but your own directions are set by your physician after reviewing your intake. If you have questions about your dose, message your care team.
The number printed on your vial. Pepti's typical directions are 25 units, which is 0.25 mL on a U-100 insulin syringe, where 1 unit is 0.01 mL.
From a 2 mg/mL vial, 0.5 mg. 10 units is 0.2 mg and 50 units is 1 mg. Those figures only hold at 2 mg/mL; a different concentration gives different milligrams for the same units.
The chart above converts units to millilitres to milligrams for a 2 mg/mL vial, the concentration Pepti supplies. It is a conversion, not a list of doses to choose from. Your own label is the one that applies.
Pepti's sermorelin directions say Monday through Friday nights, and the Ipamorelin + Sermorelin directions say nights at bedtime. Night dosing lines the pulse sermorelin prompts up with the body's largest natural growth hormone pulse, which happens in the first hours of deep sleep.
The Monday through Friday schedule is a prescriber convention: two nights off each week, described as keeping the pituitary responsive. It also lines each 20-dose vial up with a 28-day refill.
About four weeks. A 10 mg vial at 25 units (0.5 mg) five nights a week is 20 injections, which uses the whole 10 mg in four weeks, matching the 28-day refill.
The draw is the same, 0.25 mL or 25 units, and the sermorelin per dose is the same 0.5 mg, because both peptides are at 2 mg/mL. Each draw also delivers 0.5 mg of ipamorelin, and the directions specify bedtime.
No. Take the dose printed on your vial, on the schedule printed on your vial. Fluid retention, joint aches and tingling hands are the recognised signs of growth hormone above what a person needs. If you have questions about your dose, message your care team.
