Can You Take SYNAPSE Blend With Tirzepatide or Semaglutide?

This page covers what each product does, where they meet, how the schedules fit, what not to combine, and what to tell your physician. SYNAPSE and compounded tirzepatide are prescription injections, and compounded medications are not FDA approved.

Tirzepatide (Injectable)
Tirzepatide$159/mo

Yes, SYNAPSE can be prescribed alongside tirzepatide or semaglutide, because the two are not doing the same job: tirzepatide reduces appetite through GIP and GLP-1 receptors, while SYNAPSE is four peptides, Semax, Selank, MOTS-c and tesamorelin, described around attention, stress, cellular fuel sensing and the growth hormone pulse. No study has tested SYNAPSE with a GLP-1, and nothing shows the pairing does more than either alone. The point that matters is glucose: SYNAPSE's tesamorelin can raise blood sugar because growth hormone reduces insulin sensitivity, its MOTS-c is described in mice improving insulin sensitivity, and GLP-1 medications lower blood sugar. Everything on this page applies to semaglutide the same way.

Can You Take SYNAPSE Blend With Tirzepatide or Semaglutide?

The short answer

Question Answer
Can SYNAPSE and tirzepatide be taken together? They can be prescribed together; your physician reviews your intake first
Is there a known interaction? None is described; no component of SYNAPSE has an established interaction list
Has the combination been studied? No. SYNAPSE has never been studied as a blend, let alone with a GLP-1
Do they do the same thing? No. Appetite for tirzepatide; brain signalling and metabolism for SYNAPSE's four components
What to watch? Glucose, in both directions, and early nausea, which can come from either
Is it a weight-loss stack? No. Tesamorelin's approved label calls it weight-neutral, and no trial shows SYNAPSE adds weight loss
Same answer for semaglutide? Yes

The medications this page covers:

Product What is in it Typical printed directions Price
SYNAPSE Blend 5 mL multi-dose vial: Semax 1 mg/mL, Selank 1 mg/mL, MOTS-c 2 mg/mL, tesamorelin 2 mg/mL (5 mg / 5 mg / 10 mg / 10 mg per vial) 0.25 mL (25 units) subcutaneously once daily Monday through Friday $279/mo
Tirzepatide Dose-specific 2 mL vials, 2.5 mg to 15 mg weekly 0.50 mL (50 units) once weekly From $159/mo
Semaglutide Dose-specific vials at several strengths Printed on the vial From $99/mo

Every price is all-in, covering medication, physician review, refill management and shipping, with no membership fee. GLP-1 prices are "from" prices because they are priced by dose. Your physician sets your directions after reviewing your intake, and they are printed on the vial.

What each product does

Tirzepatide

Tirzepatide is a single peptide that activates two incretin receptors. It acts on appetite centres in the brain, slows gastric emptying and changes insulin and glucagon signalling, and the weight change reported in its trials follows from people eating less over a long period. It is not a stimulant. See what is tirzepatide.

SYNAPSE: four peptides in one vial

SYNAPSE is not a single molecule. Each dose of 0.25 mL carries a fixed split of four separately studied peptides:

Component Per 0.25 mL dose What it is described around Acts on appetite?
Semax 0.25 mg A fragment of ACTH, described in rat work around BDNF and attention No
Selank 0.25 mg A tuftsin analogue, described around GABA signalling and the stress response No
MOTS-c 0.5 mg A mitochondrial peptide described in mouse and cell work activating AMPK, the cell's low-fuel sensor No
Tesamorelin 0.5 mg A growth-hormone-releasing hormone analogue that prompts a pulse of your own growth hormone No

Those per-dose figures are each concentration multiplied by 0.25 mL, on a U-100 insulin syringe. None of the four acts on the incretin receptors tirzepatide uses, so taking both is not a duplication. The full reference is what is the SYNAPSE blend.

Where the two meet: glucose

This is the main practical point, and it comes from two of SYNAPSE's four components, not from Semax or Selank.

Tesamorelin pulls glucose up. Growth hormone reduces insulin sensitivity. The approved tesamorelin product's label records new-onset diabetes in 5% of treated patients against 1% on placebo, at 2 mg daily, four times the tesamorelin in a SYNAPSE dose. Growth hormone reduces insulin sensitivity while GLP-1 medications act in the other direction, which is why glucose is watched for anyone taking a tesamorelin blend with a GLP-1.

MOTS-c is described pulling the other way. In the 2015 Cell Metabolism work, MOTS-c in mice was reported to prevent age-related insulin resistance and the insulin resistance that develops on a high-fat diet. That has never been tested in people.

Tirzepatide lowers glucose. It amplifies insulin release only when glucose is high, so on its own it rarely causes low blood sugar (under 2 percent in SURPASS-2), though the risk rises with insulin or a sulfonylurea.

Nobody has measured what tesamorelin and MOTS-c do to glucose together, let alone alongside a GLP-1. In practice:

If you What it means
Have diabetes, prediabetes or a borderline fasting glucose Put it on the intake; tesamorelin can raise blood sugar
Take insulin or a sulfonylurea Say so at intake; low blood sugar becomes more likely on a GLP-1
Notice thirst or frequent urination Message your care team; it can be a sign of raised blood sugar
Feel shaky, sweaty or confused Treat it the way you have been taught, then report it the same day

Never change your diabetes medication or either dose on your own. Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.

Nausea and other overlapping effects

Nausea is the most common side effect of GLP-1 medications. Clinical reports on SYNAPSE are mostly injection-site reactions, but some people report nausea or light-headedness early on. Starting both in the same week makes it hard to tell which one is responsible, which is one reason to start them on different dates.

Effect On SYNAPSE On tirzepatide or semaglutide
Nausea Some people, early on The most common side effect
Light-headedness or a faster pulse Brief, after a dose, attributed to MOTS-c Dizziness is reported; resting heart rate can rise slightly
Blood sugar Tesamorelin can raise it; MOTS-c's mouse mechanism points the other way Lowers it; low readings are uncommon alone
Swelling, joint aches, tingling Tesamorelin's fluid-related growth hormone effects Not a typical GLP-1 effect
Restlessness late in the day Attributed to Semax Not a typical GLP-1 effect
Injection-site reactions The most common report Reported

If nausea shows up after a GLP-1 dose increase, the GLP-1 is the more likely cause. Managing it is covered in GLP-1 nausea: how to manage it, and SYNAPSE's full list is in SYNAPSE blend side effects.

Is SYNAPSE ever a reason to add to a GLP-1?

People sometimes come to SYNAPSE because a GLP-1 has left them feeling flat or unfocused, or because they want body composition support on top of weight change. Neither has been tested. The honest position:

  • Focus and stress. Semax and Selank have no appetite mechanism, and nothing shows they change anything on a GLP-1. If the brain side is the whole goal, Semax + Selank delivers those two without tesamorelin's glucose and fluid considerations. More in can you take Semax with tirzepatide and can you take Selank with tirzepatide.
  • Visceral fat. Tesamorelin is sometimes prescribed with a GLP-1 on purpose, with the GLP-1 addressing total weight and tesamorelin addressing visceral fat. A SYNAPSE dose carries 0.5 mg on weekdays, a quarter of the main trial dose. See tesamorelin vs semaglutide.
  • Muscle fuel handling. MOTS-c is paired with GLP-1 medications with muscle in mind, but it has no human trials. See can you take MOTS-c with tirzepatide.

No trial shows that any of these adds weight loss on top of tirzepatide, or that the combination works better than tirzepatide alone. Protein and resistance training remain the countermeasures with the most behind them for lean mass. The wider list is in what peptides can you take with tirzepatide.

How the two schedules fit together

SYNAPSE Tirzepatide
Draw 0.25 mL (25 units) on a U-100 insulin syringe 0.50 mL (50 units) at every strength
Per dose 0.25 mg Semax, 0.25 mg Selank, 0.5 mg MOTS-c, 0.5 mg tesamorelin Depends on the vial strength, 2.5 mg to 15 mg
When Once daily, Monday through Friday Once weekly
Doses per vial 20, about four weeks 4, four weeks
Refills Every 28 days, one vial per fill Every 28 days, one vial per fill

The schedules are independent. Use a separate fresh syringe for each and never draw both into one. Dosing detail is in SYNAPSE blend dosage and tirzepatide dosing chart.

Can You Take SYNAPSE Blend With Tirzepatide or Semaglutide?

What not to combine

Combination Why not
Tirzepatide with semaglutide or any other GLP-1 Same receptor system, stacked side effects, no mechanism gained
SYNAPSE plus a separate tesamorelin or MOTS-c vial, or FUSION or PHYSIQ Each already contains tesamorelin and MOTS-c, so a component is doubled without anyone deciding it should be
Two of your vials in one syringe Neither label describes the mixture
SYNAPSE and a GLP-1 started in the same fortnight If something changes, nobody can tell which one caused it
Anything sold "for research use only" Nobody is accountable for identity, purity, sterility or concentration

Tested athletes

Tesamorelin (S2.2.4) and MOTS-c (S4.4.1) are named on the 2026 WADA Prohibited List, prohibited at all times, so SYNAPSE is prohibited in tested sport. A GLP-1 does not change that. See is SYNAPSE blend banned in sports.

Can You Take SYNAPSE Blend With Tirzepatide or Semaglutide?

Semaglutide works the same way

Everything above applies to semaglutide. It acts at the GLP-1 receptor only, where tirzepatide acts at two, but it reduces appetite and slows gastric emptying in the same way and rarely causes low blood sugar on its own. The glucose point with tesamorelin is identical. Semaglutide and tirzepatide are never taken together. See semaglutide vs tirzepatide.

Can You Take SYNAPSE Blend With Tirzepatide or Semaglutide?

What the evidence shows, honestly

SYNAPSE has never been studied as a blend, in people or in animals, and no component has been studied alongside a GLP-1. The evidence is per component and uneven: tesamorelin has randomised placebo-controlled trials of the approved product for visceral fat in HIV-associated lipodystrophy and one 20-week cognition trial in older adults; Semax and Selank have Russian clinical work in stroke and anxiety-disorder patients, using intranasal products; MOTS-c has mouse and cell work only. Tirzepatide has a large evidence base for weight management in its approved form.

Where SYNAPSE's components stand with the FDA: tesamorelin is the active ingredient of an FDA-approved product, but compounded tesamorelin is not that product; in July 2026 the Pharmacy Compounding Advisory Committee voted to recommend adding Semax and MOTS-c to the 503A Bulks List, with the FDA's final determinations pending; Selank's nomination was withdrawn and it has never been reviewed. SYNAPSE is not an FDA-approved drug. More in SYNAPSE blend benefits.

How to add SYNAPSE to a GLP-1 properly

  1. Name the goal. "I feel foggy and flat since starting tirzepatide" gives your physician more to work with than a product name, and it may point to sleep, food intake or another cause first.
  2. List every glucose-lowering medication. Insulin and sulfonylureas especially, and any diagnosis of diabetes or prediabetes.
  3. Let your physician check for duplication. FUSION and PHYSIQ also contain tesamorelin and MOTS-c.
  4. Start one thing at a time, and report what happens. Including nothing.

Where to get SYNAPSE prescribed

  1. Complete a free online intake covering your history, medications and what you want help with.
  2. A physician licensed in your state reviews it, usually within 24 hours.
  3. If it is appropriate, a PCAB-accredited, FDA-registered US 503A compounding pharmacy prepares it to that prescription. Lot test results are published at /quality/lab-results.
  4. It ships refrigerated with ice packs, with sterile syringes and alcohol prep pads, and your directions printed on the vial.
  5. Your care team stays reachable for side effects and questions. Refills run every 28 days, one vial per fill.
