Can You Take MOTS-c With Tirzepatide or Semaglutide?

This page covers why the two are paired, what each does, where they meet, the MOTS-c blends often used with a GLP-1, how the schedules fit, and what the evidence does and does not show.

MOTS-c (Injectable) — pepti Pen
MOTS-cVial $259 · Pen $339

Yes, MOTS-c is commonly prescribed alongside tirzepatide or semaglutide, because the two are not competing for the same job. Tirzepatide reduces appetite through GIP and GLP-1 receptors; MOTS-c is a mitochondrial peptide described in mouse work around fuel handling in skeletal muscle, through AMPK, the cell's low-fuel sensor. There is no human trial of the pairing and no evidence that MOTS-c adds weight loss on top of a GLP-1. The two points to know are glucose, because both point in the direction of lower blood sugar, and heart rate, because both can nudge it up. Everything on this page applies to semaglutide the same way.

Why the two are paired

Different jobs

Tirzepatide is a single peptide that activates two incretin receptors. It acts on appetite centres in the brain, slows gastric emptying and changes insulin and glucagon signalling, and the weight change reported in its trials follows from people eating less over a long period. It is not a stimulant and does not force the body to burn anything. See what is tirzepatide.

MOTS-c is a 16-amino-acid peptide encoded in the mitochondria's own DNA, identified in a 2015 paper in Cell Metabolism. In cells it slows the folate cycle, a by-product called AICAR builds up, and AICAR activates AMPK, the same low-fuel sensor exercise switches on. Skeletal muscle appeared to be the main target. It is not a GLP-1 medication, not a stimulant, not a growth hormone secretagogue, and does not work through appetite. See what is MOTS-c.

MOTS-c Tirzepatide
Described action AMPK activation, fuel handling in muscle Appetite, gastric emptying, insulin and glucagon
Acts on appetite? No Yes, its main action
Human evidence Observational only; no randomised trials Large randomised trials of the approved product
Schedule Weekday mornings Once weekly

The reason behind the pairing

Weight lost on a GLP-1 is not all fat. In the STEP 1 body-composition substudy of semaglutide, both fat mass and lean mass fell, though fat fell by more. Appetite suppression this strong also makes under-eating protein the default. Most adjuncts to a GLP-1 are prescribed with that gap in mind, and MOTS-c's place is muscle fuel handling: prescribers describe subtle changes such as better training tolerance and steadier afternoon energy. That is a mechanism and clinical experience, not a trial result. Protein and resistance training remain the countermeasures with the most behind them.

Where the two meet

  • Glucose

This is the main practical point. In the 2015 Cell Metabolism work, MOTS-c in mice was reported to prevent age-related insulin resistance and the insulin resistance that develops on a high-fat diet. That has never been tested in people, but the mechanism points toward lower glucose. Tirzepatide amplifies insulin release only when glucose is high, so on its own it rarely causes low blood sugar (under 2 percent in SURPASS-2), though the label notes hypoglycemia has been reported in adults without diabetes, and the risk rises with insulin or a sulfonylurea.

    If you What it means
Take no glucose-lowering medication Low blood sugar is uncommon on either; know the signs anyway
Take insulin or a sulfonylurea Say so at intake; both MOTS-c's mechanism and tirzepatide point toward lower glucose
Feel shaky, sweaty or confused Treat it the way you have been taught, then report it the same day

Never change your diabetes medication on your own. Your physician sets your directions after reviewing your intake, and any change is printed on your vial.

  • Heart rate

Some people describe transient jitteriness, a faster pulse or mild light-headedness in the hour or two after a morning MOTS-c dose, mostly early in a course, which usually settles. On the GLP-1 side, the semaglutide label reports resting heart rate rising by an average of 1 to 4 beats per minute against placebo, and the tirzepatide label asks for heart rate to be checked at routine intervals. A racing heart at rest should be reported on either.

  • Stomach side effects: no overlap

MOTS-c is not described to cause nausea, appetite loss or stomach upset, which separates it from GLP-1 medications. If you get nausea after adding MOTS-c to tirzepatide, the GLP-1 is the more likely cause, particularly around a dose increase. That said, the absence of a pattern in a small evidence base is not proof, so report anything new and persistent. Managing GLP-1 nausea is covered in GLP-1 nausea: how to manage it.

Effect On MOTS-c On tirzepatide or semaglutide
Nausea, appetite loss Not described The most common side effects
Jitteriness, faster pulse Some people, after a morning dose, early on Resting heart rate can rise slightly
Light-headedness Some people, same window Dizziness and low blood pressure are reported
Low blood sugar Mechanism points that way; untested in people Uncommon alone; more likely with insulin or a sulfonylurea
Injection-site reactions The most common report 6 to 8% on tirzepatide against 2%

The MOTS-c blends used with a GLP-1

MOTS-c is more often prescribed inside a blend than alone, and several blends are commonly used alongside tirzepatide or semaglutide.

  • PHYSIQ combines it with tesamorelin, ipamorelin and AOD-9604. Growth hormone reduces insulin sensitivity while GLP-1 medications act in the other direction, so glucose is watched for anyone taking a tesamorelin blend with a GLP-1.
  • SHRED combines it with AOD-9604, CJC-1295 and ipamorelin. Ipamorelin acts at the receptor ghrelin uses to signal hunger, so some people notice more appetite rather than less.
  • FUSION combines it with BPC-157, 5-Amino-1MQ and tesamorelin, and is positioned for use with or without a GLP-1. Prescribers using FUSION alongside a GLP-1 describe less of the gut discomfort patients associate with the GLP-1; that has not been tested in a trial.

Never take MOTS-c alone on top of a blend that already contains it without your physician knowing; that changes the dose without anyone having decided it should change. Comparisons are in MOTS-c vs tesamorelin and MOTS-c vs SLU-PP-332 vs 5-Amino-1MQ.

How the two schedules fit together

MOTS-c Tirzepatide
Draw 0.25 mL (25 units) on a U-100 insulin syringe 0.50 mL (50 units) at every strength
Per dose 0.5 mg MOTS-c (2 mg/mL vial) Depends on the vial strength, 2.5 mg to 15 mg
When Monday through Friday mornings Once weekly
Doses per vial 20, about four weeks 4, four weeks
Refills Every 28 days, one vial per fill Every 28 days, one vial per fill

The schedules are independent. Use a separate fresh syringe for each and never draw both into one. Dosing detail is in MOTS-c dosage and tirzepatide dosing chart.

Can You Take MOTS-c With Tirzepatide or Semaglutide?

What not to combine

Combination Why not
Tirzepatide with semaglutide or any other GLP-1 Same receptor system, stacked side effects, no mechanism gained
MOTS-c alone plus a blend containing MOTS-c Changes the MOTS-c dose without a decision to change it
Two of your vials in one syringe Neither label describes the mixture
Several new agents started in the same fortnight If something changes, nobody can tell which one caused it
Anything sold "for research use only" Nobody is accountable for identity, purity, sterility or concentration

Tested athletes

MOTS-c is on the WADA Prohibited List at all times as an AMPK activator, and USADA has stated it is not eligible for a therapeutic use exemption. A GLP-1 does not change that.

Can You Take MOTS-c With Tirzepatide or Semaglutide?

Semaglutide works the same way

Everything above applies to semaglutide. It acts at the GLP-1 receptor only, where tirzepatide acts at two, but it reduces appetite and slows gastric emptying in the same way, rarely causes low blood sugar on its own, and carries a small average rise in resting heart rate on its label. MOTS-c is doing the same different job alongside it. Semaglutide and tirzepatide are never taken together. See semaglutide vs tirzepatide.

Can You Take MOTS-c With Tirzepatide or Semaglutide?

What the evidence shows, honestly

There are no randomised controlled trials of MOTS-c in humans. Its evidence is mouse and cell work, plus observational human studies of the body's own MOTS-c, which rises with exercise in muscle and blood. A 2021 paper in Nature Communications reported better physical performance in treated mice and the exercise rise in people. None of that tests injected MOTS-c in people, and none of it tests MOTS-c with a GLP-1.

On regulation, the FDA removed MOTS-c from Category 2 of its interim 503A list on 15 April 2026, and its compounding advisory committee voted 7 to 5, with two abstentions, on 23 July 2026 to recommend adding it to the 503A Bulks List. The FDA's final determination is pending.

So the honest statement is narrow. Tirzepatide has the evidence for weight; MOTS-c has a coherent preclinical story about muscle fuel handling; the two are commonly prescribed together; and no trial shows the pairing works better than tirzepatide alone. More is in MOTS-c benefits, MOTS-c side effects and what peptides can you take with tirzepatide.

How to add MOTS-c to a GLP-1 properly

  1. Name the gap. "My training has stalled and I am worried about muscle" gives your physician more to work with than a product name.
  2. List every glucose-lowering medication. Insulin and sulfonylureas especially.
  3. Let your physician check for duplication. PHYSIQ, SHRED, FUSION, SYNAPSE and EUPHORIA all contain MOTS-c.
  4. Start one thing at a time, and report what happens. Including nothing.

Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.

Where to get MOTS-c prescribed

  1. Complete a free online intake covering your history, medications and what you want help with.
  2. A physician licensed in your state reviews it, usually within 24 hours.
  3. If it is appropriate, a PCAB-accredited, FDA-registered US 503A compounding pharmacy prepares it to that prescription. Lot test results are published at /quality/lab-results.
  4. It ships refrigerated with ice packs, with sterile syringes and alcohol prep pads, and your directions printed on the vial.
  5. Your care team stays reachable for side effects and questions. Refills run every 28 days, one vial per fill.
