SYNAPSE Blend (Semax / Selank / MOTS-c / Tesamorelin)
SYNAPSE Blend (Semax / Selank / MOTS-c / Tesamorelin)$279/mo

Yes. In the United States, SYNAPSE Blend (Semax / Selank / MOTS-c / Tesamorelin) meant for a person to inject is a prescription medication: a physician licensed in your state has to prescribe it, and a licensed pharmacy has to compound it for you by name. It is legal to buy online through a telehealth provider that issues that prescription and has a licensed pharmacy fill it. Vials sold with no prescription are sold as "research use only," which disclaims human use. Compounded SYNAPSE is not FDA approved, and as of September 2026 none of its four components sits in the FDA's Category 2 safety-risk list.

Do You Need a Prescription for SYNAPSE Blend? Is It Legal to Buy Online?

The short answer

Question Answer, as of September 2026
Do you need a prescription for SYNAPSE to inject? Yes. It is dispensed lawfully only on a prescription
Is it legal to buy SYNAPSE online? Yes, through a telehealth provider that prescribes it and has a licensed pharmacy compound it
Is SYNAPSE FDA approved? No. The blend is not an FDA-approved drug, and compounded medications are not FDA approved
Does the FDA regulate "SYNAPSE" by name? No. The FDA regulates the bulk substances a pharmacy compounds from, so each component has its own status
Were any components banned? No. Semax, Selank and MOTS-c were in Category 2 from 2023 and are no longer in any active category
Where do they stand now? Semax and MOTS-c recommended for the 503A Bulks List in July 2026; Selank never reviewed; tesamorelin is the active ingredient of an FDA-approved product and outside the bulks-list process
Is it allowed in tested sport? No. MOTS-c is named on the 2026 WADA Prohibited List and prohibited at all times

At Pepti, SYNAPSE Blend is $279 a month all-in: a 5 mL multi-dose vial holding 5 mg Semax, 5 mg Selank, 10 mg MOTS-c and 10 mg tesamorelin, physician review, refill management and refrigerated shipping, with no membership fee.

Why SYNAPSE needs a prescription

  • The rule that makes it prescription-only

Section 503(b) of the Federal Food, Drug, and Cosmetic Act creates prescription-only status for a drug that is not safe to use except under the supervision of a licensed practitioner, and dispensing such a drug without a prescription is treated as misbranding. An injectable four-peptide preparation that includes a growth-hormone-releasing hormone analogue fits that description on its face. The rule attaches to whoever dispenses the drug, not to the person who receives it, which is the structural reason a grey market can exist at all.

  • Why a compounded blend is lawful without FDA approval

A new drug normally needs an approved application before it can be sold. Section 503A of the same Act exempts a drug compounded by a licensed pharmacist for an identified individual patient, on a valid prescription, from the approval requirement. A 503A pharmacy may compound from a bulk substance if it has a USP or NF monograph, is a component of an FDA-approved drug product, or appears on the FDA's 503A bulks list. The prescription is not paperwork added to a sale. It is the legal event that makes the sale possible, and it is the reason your name is on the label. The full background is in are peptides legal in the US.

  • Why the blend has no status of its own

SYNAPSE is a brand name for a combination. It does not appear in any journal and it has no file at the FDA. What the FDA reviews is the bulk drug substance a 503A pharmacy compounds from: here Semax, Selank, MOTS-c and tesamorelin. So "is SYNAPSE legal" is really four questions, one per component. The blend itself is explained in what is SYNAPSE Blend.

Where each SYNAPSE component stands with the FDA

SYNAPSE's four components are in three different positions, and the first three moved in 2026. Many pages online still describe the 2023 position.

Component Per vial 2023 position Now
Semax 5 mg Category 2 (potential immunogenicity for certain routes) Nominated but withdrawn; committee voted 8 to 5, with 1 abstention, to recommend listing on 24 July 2026
Selank 5 mg Category 2 (same concern) Nominated but withdrawn; never reviewed by the committee and not scheduled
MOTS-c 10 mg Category 2 (immunogenicity, no human exposure data) Removed 15 April 2026; committee voted 7 to 5, with 2 abstentions, to recommend listing on 23 July 2026
Tesamorelin 10 mg Never nominated Active ingredient of an FDA-approved product; outside the bulks-list process
  • Semax: recommended in July 2026

Semax was placed in Category 2 of the FDA's 503A bulk drug substances list in 2023, the category for nominated substances with identified safety concerns. It is no longer there: the FDA's safety-risks page, current as of 22 April 2026, lists "Semax (heptapeptide)" among substances nominated but withdrawn, and the categories document updated 14 May 2026 does not carry it in Category 1, 2 or 3. On 24 July 2026 the Pharmacy Compounding Advisory Committee considered Semax for cerebral ischaemia, migraine and trigeminal neuralgia. FDA scientists recommended against inclusion; the committee voted 8 to 5, with one abstention, to recommend adding it.

  • Selank: withdrawn and never reviewed

Selank acetate, listed by the FDA as "Selank acetate (TP-7)," was placed in Category 2 in 2023. Its nomination was later withdrawn, it does not appear in any category of the 14 May 2026 document, it was not on the July 2026 agenda, and it is not among the substances scheduled for review before the end of February 2027. Not flagged as a safety risk, never evaluated, no active nomination.

  • MOTS-c: removed in April, recommended in July 2026

On 15 April 2026 the FDA removed MOTS-c from Category 2, one of twelve peptides removed that day. On 23 July 2026 the committee reviewed it for obesity and osteoporosis and voted 7 to 5, with two abstentions, to recommend adding it to the 503A Bulks List, again against the recommendation of FDA's own reviewers.

  • Tesamorelin: an approved active, compounded off-label

Tesamorelin's position is different and did not move in 2026. An FDA-approved tesamorelin acetate product exists: it was approved on 10 November 2010 for the reduction of excess abdominal fat in HIV-infected adults with lipodystrophy, and its current label carries that single indication. Because tesamorelin is the active ingredient of an approved product, it is compoundable by that route and was never nominated to the bulks list. The tesamorelin in SYNAPSE is not that approved product: it is compounded by a state-licensed pharmacy to a physician's prescription, has not been reviewed by the FDA as a finished product, and is prescribed off-label.

  • What that adds up to

None of the four components is restricted. A committee vote is a recommendation, not a decision; the FDA's own determinations on Semax and MOTS-c are still pending as of September 2026, and a formal listing would go through rulemaking. Even a final listing would not make either an FDA-approved drug. All four remain legal for a physician to prescribe and a state-licensed pharmacy to compound to an individual prescription. SYNAPSE itself is not an FDA-approved drug product. See are peptides FDA approved.

What "research use only" means legally

The words on a no-prescription vial are doing legal work. A regulation, 21 CFR 201.125, exempts a drug from carrying adequate directions for use when it is shipped to people lawfully engaged in research not involving clinical use. A person injecting the contents is the clinical use the exemption excludes. A second regulation, 21 CFR 201.128, says a product's intended use is shown by the seller's objective intent, including advertising, so a site that publishes "nootropic stacks" and injection schedules while its label says "not for human consumption" is describing a human use whatever the label says.

What the label or site says What it means legally What it means for you
"Research use only" The seller is claiming a laboratory exemption No claim about human safety is made or required
"Not for human consumption" Human use is disclaimed The seller is disclaiming the exact use you intend
"No prescription needed" No prescriber is involved Nobody has looked at your psychiatric medication or glucose history
"99% pure" No legal meaning on its own Says nothing about sterility, endotoxin or the actual amounts of four peptides
A prescription label with your name Compounded for an identified patient under 503A A physician and a pharmacy are accountable for it

A four-component vial makes the research-vial problem worse. A purity figure for one peptide says nothing about whether the other three are present at the stated amounts, and the ratio is the product. The longer comparison is in research peptides vs prescription peptides and can you buy peptides without a prescription.

What a lawful SYNAPSE prescription looks like

Three things have to be true at once, and you can check each yourself.

Requirement What it means How to check
A licensed prescriber A physician licensed in your state reviewed your intake Look the physician up on your state medical board's public license search
A licensed pharmacy A state-licensed 503A pharmacy compounded the vial The pharmacy is named on your label; your state board of pharmacy has a license lookup
A prescription in your name Compounded for you, not stock sold to anyone Your name, the drug, the strength, your directions and a beyond-use date are on the vial

At Pepti that looks like this: a physician licensed in your state reviews your intake, usually within 24 hours; a PCAB-accredited, FDA-registered US 503A compounding pharmacy prepares the vial; and it ships refrigerated with ice packs, sterile syringes and alcohol prep pads, as a multi-dose solution ready to use with directions printed on the vial. The typical printed direction is 0.25 mL (25 units on a U-100 insulin syringe) under the skin once daily Monday through Friday. At the vial's concentrations that volume contains 0.25 mg Semax, 0.25 mg Selank, 0.5 mg MOTS-c and 0.5 mg tesamorelin (math only), and the vial holds 20 doses, which is why refills run every 28 days, one vial per fill. Your physician sets your directions after reviewing your intake. Lot test results are published at lab results.

Why the intake matters for this blend

A prescription is not a formality for SYNAPSE. The intake asks questions that a no-prescription seller never would, and several are specific to these four peptides:

What the intake asks Why, for SYNAPSE
Existing psychiatric treatment Selank is described as modifying GABA signalling; anything already acting there is reviewed first
Diabetes or impaired glucose tolerance Tesamorelin can worsen glucose tolerance
Personal history of cancer The approved tesamorelin product is contraindicated in active malignancy
Pregnancy or breastfeeding The approved tesamorelin product is contraindicated in pregnancy; safety data is absent
Pituitary conditions or corticosteroid replacement The approved product's label raises both
Every medication and supplement Growth hormone can alter the metabolism of drugs cleared by cytochrome P450 enzymes
Tested sport MOTS-c is prohibited at all times

There is no established interaction list for the blend, because the trials that would produce one have not been run. Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.

What a prescription does not change

What a prescription changes What it does not change
Who is accountable for the vial That SYNAPSE has never been studied as a combination
Whether a physician has reviewed your history That the one cognition trial is of tesamorelin alone, in adults aged 55 to 87
Whether sterility and endotoxin testing apply That MOTS-c has no human trials
Whether your name and directions are on the label Anti-doping rules: a prescription is not an exemption

On sport: the 2026 WADA Prohibited List names MOTS-c under S4.4.1, activators of AMP-activated protein kinase, prohibited at all times. Semax and Selank are not named, which is not the same as permitted. See is MOTS-c banned in sports, is Semax banned in sports and is Selank banned in sports.
