Is Semax Banned in Sports? WADA Status, the ACTH Question and Drug Tests

This page is general information, not legal advice. The List is revised every year and the duty to check sits with the athlete.

Semax (Injectable)
Semax$229/mo

Semax is not named on the 2026 World Anti-Doping Agency Prohibited List, but a tested athlete should not treat it as cleared. Semax is built from the 4 to 7 region of ACTH, and corticotrophins, the hormone class ACTH belongs to, are listed at S2.2.2; the List's S0 class also covers unapproved substances that are not named. Semax is described as devoid of ACTH's hormonal activity, but whether that settles it is a question for your anti-doping body. Blends that contain Semax are a clearer answer: ELEVATE and SYNAPSE each contain substances the List names. A standard workplace drug screen does not look for Semax.

Is Semax Banned in Sports? WADA Status, the ACTH Question and Drug Tests

The short answer

The question The answer Who it affects
Is Semax named on the WADA List? No, not on the 2026 List Anyone under the World Anti-Doping Code
Is there a reason for caution anyway? Yes. Its parent hormone class, corticotrophins, is listed at S2.2.2, and S0 covers unnamed unapproved substances Tested athletes
Are Semax blends prohibited? ELEVATE contains BPC-157 (S0); SYNAPSE contains tesamorelin (S2.2.4) and MOTS-c (S4.4.1) Users of those blends
Does a prescription protect me? No. Only a therapeutic use exemption granted in advance does Every tested athlete
Will it show up on a work drug test? No. Employment panels screen for drugs of abuse only Employees and job applicants
Is it illegal? No. It is legal to prescribe and dispense with a valid prescription Nobody's possession is a crime

What Semax is, and why the ACTH link matters

Semax is a heptapeptide, seven amino acids: Met-Glu-His-Phe-Pro-Gly-Pro. The first four are positions 4 to 7 of adrenocorticotropic hormone (ACTH), the pituitary hormone that tells the adrenal glands to make cortisol. The last three, Pro-Gly-Pro, were added to slow its breakdown. The design keeps the region of ACTH studied for decades for effects on attention and learning in animals and drops the hormonal activity. The FDA's 2026 briefing describes Semax as the Russian literature does: an analogue "devoid of the hormonal corticotropic and melanotropic properties" of the parent. It is not expected to raise cortisol, and it is not a steroid, an anabolic agent or a stimulant.

It was developed at the Institute of Molecular Genetics of the Russian Academy of Sciences and is a registered medicine in Russia for stroke and chronic cerebrovascular disease. More is in what is Semax.

The S2.2.2 question

The 2026 List, in force from 1 January 2026, does not name Semax. It does list corticotrophins at S2.2.2. That is why the live position on this site is that a tested athlete should ask their anti-doping body about an ACTH fragment before assuming anything. Semax's described lack of corticotropic activity is the argument that it sits outside that class; the anti-doping body that governs your sport is the one entitled to make that call.

The S0 question

S0 prohibits at all times "any pharmacological substance which is not addressed by any of the subsequent sections of the List and with no current approval by any governmental regulatory health authority for human therapeutic use." WADA's explanatory note to the 2027 List adds that even if a peptide is not specifically named, "utmost caution should be taken as it may still be prohibited." How S0 applies to a compounded Semax preparation is, again, for your anti-doping body.

Blends containing Semax

A blend is prohibited by virtue of its components, so the blend answer can be clearer than the Semax answer.

Pepti product What else is in it Price Tested-sport status
Semax Nothing; 10 mg Semax in a 5 mL vial $229/mo Not named; ask your anti-doping body
ELEVATE Blend Selank, BPC-157, KPV $249/mo Prohibited; BPC-157 is named in S0
SYNAPSE Blend Selank, MOTS-c, tesamorelin $279/mo Prohibited; tesamorelin (S2.2.4) and MOTS-c (S4.4.1) are named

Prices are all-in monthly figures covering medication, physician review, refill management and shipping, with no membership fee. Compounded medications are not FDA approved.

Why a prescription does not change the answer

Anti-doping rules decide what is permitted in competition, independently of what is legal or prescribed. Article 2.1.1 of the World Anti-Doping Code makes it each athlete's personal duty to ensure that no prohibited substance enters their body, and says intent, fault, negligence or knowing use need not be shown to establish a violation. That is strict liability. It also means you are responsible for what is actually in the vial, which is why a research-labelled product is a risk in its own right.

If your anti-doping body tells you Semax is prohibited for you, a therapeutic use exemption granted in advance is the only route to permitted use. It requires a diagnosed condition supported by clinical evidence, no enhancement beyond a return to normal health, an indicated treatment with no reasonable permitted alternative, and a need that did not arise from earlier prohibited use. In the United States, USADA directs athletes outside its testing pools to its TUE Pre-Check Form, with a stated response time of three to five business days.

The NCAA and other programs

College athletes are governed by the NCAA's banned drug classes, including "peptide hormones, growth factors, related substances and mimetics," and the NCAA bans anything chemically or pharmacologically related to a banned class even if not named, with the school and the student-athlete both accountable. Semax is not among the NCAA's published examples; ask your athletics health care provider before starting. Professional leagues, military services and some employers run their own policies.

Will Semax show up on a drug test?

Type of test Does it look for Semax? What to do
Standard workplace panel No. The federal panel in 49 CFR 40.85 covers drugs of abuse only Declare prescriptions through the confidential route if asked
Expanded workplace panel No. Expanded panels add more drugs of abuse, not therapeutic classes Ask what your panel covers
Anti-doping test Semax is not named; the S2.2.2 and S0 questions apply. Blends with named components are prohibited Ask your anti-doping body before starting
Clinical testing before a procedure A safety question, not a screen Tell the treating clinician everything you take

Workplace panels have no assay for Semax, and a seven-amino-acid peptide has no structural relationship to the drugs of abuse those panels target, so cross-reactivity is not a realistic worry. On the sport side, a substance being hard to find does not make it permitted, and under Code Article 6.6 stored samples can be reanalysed, with Article 17 allowing a charge within ten years. The two systems are compared in do peptides show up on a drug test.

Where Semax stands with the FDA

A separate axis. On 24 July 2026 the FDA's Pharmacy Compounding Advisory Committee voted 8 to 5, with 1 abstention, to recommend adding Semax to the 503A Bulks List; FDA's own reviewers had recommended against inclusion. A committee vote is a recommendation, not a decision, and adding a substance requires rulemaking. The FDA's stated safety concern in its 2026 review was a quality one rather than a known toxicity: an injectable peptide could in principle provoke an immune response if it aggregates or carries impurities.

Compounded Semax is legal to prescribe and dispense and is not FDA approved; compounded medications never are. None of this changes anti-doping status, because a compounding decision is not a drug approval. Side effects are covered in Semax side effects.

If you compete: what to do

Your situation What applies
In a tested pool, considering Semax alone Ask your anti-doping body about the ACTH fragment and S0 before starting; check Global DRO and keep the reference number
Considering ELEVATE or SYNAPSE Not an option for a tested athlete; each contains a named prohibited substance
Already prescribed and now entering testing Tell your physician through your account and get advice from your anti-doping body or team physician
A college athlete Ask your athletics health care provider first
A recreational athlete never subject to testing The anti-doping rule does not reach you

The athlete overview is in peptides for athletes, and Semax's sibling peptide is covered in Selank vs Semax.

Where to get Semax prescribed

Semax cannot be bought legitimately without a prescription. Sites selling it "for research use only" are not accountable for what is in the vial: identity, purity, sterility and concentration are untested. The prescription route:

  1. Complete a free online intake covering your history, medications and what you want help with. If you are subject to anti-doping testing, say so there.
  2. A physician licensed in your state reviews it, usually within 24 hours.
  3. If it is appropriate, a state-licensed, FDA-registered US pharmacy compounds it to that prescription. Lot test results are published at /quality/lab-results.
  4. It ships refrigerated with your directions printed on the vial, with sterile syringes and alcohol prep pads.
  5. Your physician stays reachable for questions through your account.
