Is MOTS-c Banned in Sports? WADA S4.4.1 Status, Drug Tests and TUEs
This page is general information, not legal advice. Prohibited lists change every year, and the duty to check sits with the athlete.

Treatments mentioned
Yes. MOTS-c is prohibited at all times in tested sport: the 2026 World Anti-Doping Agency Prohibited List names it under S4.4.1, activators of AMP-activated protein kinase, alongside AICAR, and the 2027 List carries that classification forward. USADA has stated it is not eligible for a therapeutic use exemption, and a validated plasma test for it was published in 2019. Any blend containing MOTS-c is prohibited too. A standard workplace drug screen does not look for it.

What the List says about MOTS-c
The Prohibited List is published every year and comes into force on 1 January. Section S4 covers hormone and metabolic modulators. Under S4.4.1, activators of AMP-activated protein kinase are prohibited at all times, and the named examples are BAM15, AICAR and "mitochondrial open reading frame of the 12S rRNA-c (MOTS-c)." S4.4.2 covers insulins and insulin-mimetics, and S4.4.3 and S4.4.4 cover meldonium and trimetazidine.
MOTS-c is named in the List itself, not merely caught by a catch-all. Older pages that describe it as unnamed are out of date. The 2027 Prohibited List, already published, carries MOTS-c forward in S4.4.1 unchanged.
Why an AMPK activator is on the List at all
MOTS-c is a 16-amino-acid peptide encoded in mitochondrial DNA. It was described in Cell Metabolism in 2015 and Nature Communications in 2021, in mice, as an exercise-induced regulator of metabolism and physical decline, and it is described as activating AMPK in muscle. The List places AMPK activators among the metabolic modulators of S4.4, which is where MOTS-c sits. Being on the List is a rule about the mechanism, not proof that MOTS-c improves performance in people: there are no randomised controlled trials of MOTS-c in humans. The research picture is in MOTS-c benefits.
The blends that contain MOTS-c
A blend is prohibited by virtue of its components. Several Pepti blends contain MOTS-c, and some contain a second prohibited substance as well.
| Pepti product | Other components | Price | Tested-sport status |
|---|---|---|---|
| MOTS-c | None; 10 mg MOTS-c in a 5 mL vial | $259/mo | Prohibited at all times, S4.4.1 |
| EUPHORIA Blend | BPC-157, GHK-Cu, kisspeptin-10 | $279/mo | Prohibited; BPC-157 (S0) and kisspeptin (S2.2.1, males) are named too |
| FUSION Blend | BPC-157, 5-Amino-1MQ, tesamorelin | $279/mo | Prohibited; BPC-157 and tesamorelin are named too |
| PHYSIQ Blend | Tesamorelin, ipamorelin, AOD-9604 | $289/mo | Prohibited; three other components are named in S2 |
| SHRED Blend | AOD-9604, CJC-1295, ipamorelin | $279/mo | Prohibited; three other components are named in S2 |
| SYNAPSE Blend | Semax, Selank, tesamorelin | $279/mo | Prohibited; tesamorelin is named in S2.2.4 |

Prohibited at all times
Classes S0 to S5 are prohibited in and out of competition. There is no off-season window for an S4 substance, and out-of-competition testing exists to catch substances used during training blocks. A course of MOTS-c taken months before an event is the same violation as one taken the week of it.

Why a prescription does not change the answer
Anti-doping rules decide what is permitted in competition. They do not care whether something is legal or prescribed. MOTS-c is legal to prescribe and dispense in the United States with a valid prescription, and it is still prohibited for a tested athlete.
Article 2.1.1 of the World Anti-Doping Code makes it each athlete's personal duty to ensure that no prohibited substance enters their body, and says intent, fault, negligence or knowing use need not be shown to establish a violation. That is strict liability. Fault is weighed only at the consequences stage.
The TUE route is effectively closed
A therapeutic use exemption is the formal route by which a tested athlete may use a prohibited substance for a genuine medical need. It requires a diagnosed condition supported by clinical evidence, no performance enhancement beyond a return to normal health, no reasonable permitted alternative, and a need that did not arise from earlier prohibited use. For MOTS-c, USADA has gone further and stated that it is not eligible for a TUE. If you compete, treat MOTS-c and every blend containing it as unavailable to you.

The NCAA and other programs
The NCAA bans eight drug classes, including "hormone and metabolic modulators" and "peptide hormones, growth factors, related substances and mimetics." It states that any substance chemically or pharmacologically related to a banned class is also banned even if it is not listed as an example, and that the school and the student-athlete are both accountable. A college athlete should not assume MOTS-c is permitted because it does not appear in the NCAA's printed examples. Professional leagues, military services and some employers run their own policies; read the current one that applies to you.

Can MOTS-c be detected?
| Type of test | Does it look for MOTS-c? | What to do |
|---|---|---|
| Standard workplace panel | No. The federal panel in 49 CFR 40.85 covers drugs of abuse only | Declare prescriptions through the confidential route if asked |
| Expanded workplace panel | No. Expanded panels add more drugs of abuse, not therapeutic classes | Ask what your panel covers |
| Anti-doping test | Yes. A plasma method for MOTS-c, built for doping control, was published in Rapid Communications in Mass Spectrometry in 2019 | Do not use it if you are tested |
| Clinical testing before a procedure | A safety question, not a screen | Tell the treating clinician, especially as MOTS-c has a blood glucose caution |
Detection windows for small peptides are generally short, but that is not a strategy. Under Code Article 6.6 a sample reported negative can be stored and reanalysed, and Article 17 allows a charge within ten years. MOTS-c also occurs naturally in the body; the human data that exists is observational, measuring the body's own MOTS-c, which rises with exercise. How a laboratory distinguishes a dose from the body's own level is a question for the testing authority, and nothing about natural occurrence creates a permission. The wider picture is in do peptides show up on a drug test.

Where MOTS-c stands with the FDA
A separate question with a separate answer. MOTS-c was placed in Category 2 of the FDA's interim 503A bulk drug substances list in 2023. On 15 April 2026 the FDA removed it from Category 2, one of twelve peptides removed after their original nominations were withdrawn. On 23 July 2026 the FDA's Pharmacy Compounding Advisory Committee voted 7 to 5, with two abstentions, to recommend adding MOTS-c to the 503A Bulks List, against the recommendation of FDA's own reviewers, who cited the absence of human clinical data. The FDA's final determination is pending.
MOTS-c remains legal to prescribe and dispense, and it is not FDA approved; compounded medications never are. None of this affects its anti-doping status. A compounding decision is not a drug approval, and WADA lists MOTS-c by mechanism. More is in what is MOTS-c and MOTS-c side effects.

If you compete: what to do
| Your situation | What applies |
|---|---|
| In a tested pool at any level | Do not use MOTS-c or any blend containing it |
| Already prescribed and now entering testing | Tell your physician through your account and get advice from your anti-doping body or team physician |
| A college athlete | Ask your athletics health care provider before starting anything |
| A recreational athlete never subject to testing | The anti-doping rule does not reach you; the medical cautions in MOTS-c side effects still apply |
| Considering a research-labelled vial | Do not. Contents are unverified, which is a strict-liability problem; see research peptides vs prescription peptides |
Check every medication on Global DRO, the database USADA directs athletes to, and keep the reference number. A physician may still discuss options that are not on the List, each checked against your federation's adopted version; the athlete overview is in peptides for athletes.

Where to get MOTS-c prescribed
MOTS-c cannot be bought legitimately without a prescription. Sites selling it "for research use only" are not accountable for what is in the vial: identity, purity, sterility and concentration are untested. The prescription route:
- Complete a free online intake covering your history, medications and what you want help with. If you are subject to anti-doping testing, say so there.
- A physician licensed in your state reviews it, usually within 24 hours.
- If it is appropriate, a state-licensed, FDA-registered US pharmacy compounds it to that prescription. Lot test results are published at /quality/lab-results.
- It ships refrigerated with your directions printed on the vial, with sterile syringes and alcohol prep pads.
- Your physician stays reachable for questions through your account.

Common questions
Is MOTS-c Banned in Sports? WADA S4.4.1 Status, Drug Tests and TUEs, answered.
Yes. The 2026 WADA Prohibited List names MOTS-c under S4.4.1, activators of AMP-activated protein kinase, in the same line as AICAR. It is prohibited at all times, and the 2027 List keeps it there.
Yes. S4 is prohibited in and out of competition, so there is no permitted window for a tested athlete.
USADA has stated that MOTS-c is not eligible for a therapeutic use exemption. Treat it as unavailable if you are tested.
On an anti-doping test it can: a validated plasma method was published in Rapid Communications in Mass Spectrometry in 2019. On a standard workplace panel it does not, because those panels screen only for drugs of abuse.
The NCAA bans hormone and metabolic modulators as a class and bans anything chemically or pharmacologically related to a banned class even if it is not named. A college athlete should treat MOTS-c as banned and check with their athletics health care provider.
Yes. EUPHORIA, FUSION, PHYSIQ, SHRED and SYNAPSE all contain MOTS-c, and most also contain a second named substance. A blend is prohibited by virtue of its components.
No. The List names MOTS-c directly. That the body makes its own MOTS-c, which rises with exercise, does not create a permission to take it as a medication.
No. The advisory committee's 7 to 5 vote concerned whether pharmacies may compound MOTS-c under section 503A. WADA's listing is about the substance's mechanism, and a compounding decision is not a drug approval.
No. It is legal to prescribe and dispense in the United States with a valid prescription. Prohibition in sport is a competition rule, not a law.
Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account. The free assessment goes to a physician licensed in your state. A consultation does not guarantee a prescription.
