Yes. Every Pepti injectable arrives pre-mixed: a multi-dose vial of ready-to-use solution at a stated concentration, compounded by the pharmacy, so there is no powder to dissolve and no water to add. Each vial ships refrigerated with sterile syringes and alcohol prep pads in the box, and your directions are printed on the vial in both millilitres and units on a U-100 insulin syringe. For some products a pre-filled pepti Pen cartridge is an alternative, dosed with a dial instead of a syringe. The products that must be mixed at home are research-chemical powders sold "not for human consumption," which is one of the main reasons they are riskier.

Do Peptides Come Pre-Mixed, and Do You Get Needles?
The short answer
| Question | Pepti prescription | Research-chemical powder |
|---|---|---|
| Does it come pre-mixed? | Yes, a ready-to-use solution in a multi-dose vial | No, a lyophilised (freeze-dried) powder |
| Do I have to reconstitute it? | No. A pharmacist has already done it | Yes. You choose the diluent and the volume |
| Is the concentration stated? | Yes, on the label, for example 2 mg/mL | Only what you calculate from your own mixing |
| Do needles come with it? | Yes, sterile syringes and alcohol prep pads | Usually not |
| How is the dose written? | In mL and in units on a U-100 syringe | Not written; you work it out |
| Is there a no-syringe option? | The pepti Pen, for eligible products | No |
| Who is accountable? | A physician licensed in your state and a licensed pharmacy | Nobody; human use is disclaimed |

What arrives in the box
| Item | What it is for |
|---|---|
| Your vial | A multi-dose solution, compounded to your prescription, with your name, the medication, the concentration, your directions and the beyond-use date on the label |
| Sterile syringes | U-100 insulin syringes for drawing and injecting each dose; one per injection, never reused |
| Alcohol prep pads | Swabbing the vial stopper and the injection site before each dose |
| Insulated packaging with ice packs | Keeping the vial cold in transit; the vial goes into the refrigerator when it arrives |
You do not need to buy syringes separately. Use a fresh syringe for every injection and a fresh pad for the stopper and the skin. Some directions are intramuscular rather than subcutaneous, such as methylcobalamin and L-Carnitine; the pharmacy supplies the right needle for the route, and a dose above a 1 mL barrel, like methylcobalamin's 1.25 mL, comes with an appropriate syringe, usually marked in millilitres.
The one thing not in the box is somewhere to put used needles. Used syringes go into a sharps container, never household trash. An FDA-cleared container is the recommended option; pharmacies sell them, and a heavy-duty plastic household container with a tight, puncture-resistant lid is the FDA's fallback. How to inject peptides safely covers disposal.

Why pre-mixed matters
Reconstitution is where the arithmetic goes wrong
A powdered peptide has no concentration until someone adds liquid to it. Whoever mixes it chooses the diluent and the volume, and every dose after that depends on that choice and on the maths that follows. Add 2 mL instead of 1 mL and every draw delivers half what was intended; misplace a decimal and it is ten times. The research-vs-prescription comparison calls dose arithmetic the single biggest practical risk with research vials.
A pre-mixed vial removes that step. The pharmacy compounds the solution to a stated concentration, the label states it, and your directions say how much to draw. You are matching a plunger to a printed line, not converting anything.
Sterility is done in a cleanroom, not on a kitchen counter
An injectable has to be sterile, and there is a written standard for how that is achieved: USP Chapter 797, which covers the cleanroom or segregated compounding area, the ISO 5 environment for open manipulations, gowning and environmental monitoring. Mixing a powder at home skips all of it. Combining several powders into one homemade blend is the exact scenario that standard exists to prevent.
Pepti's pharmacies are PCAB-accredited, FDA-registered US 503A compounding pharmacies, and each lot of finished solution is tested by an independent laboratory for sterility, bacterial endotoxin and potency. The certificates are published by lot at /quality/lab-results.
Blends arrive in one vial
Where a combination is prescribed, the pharmacy compounds it into a single vial, which is what the blends are. KLOW at $259 a month holds 50 mg GHK-Cu, 10 mg KPV, 10 mg BPC-157 and 10 mg TB-500 in one 5 mL vial; BPC-157 + TB-500 at $259 a month is both peptides at 1 mg/mL each. Do not mix two products in one syringe yourself. See can you take two peptides together.

Examples of what pre-mixed looks like
These are the standard vials and directions for several products. Your own directions are printed on your vial and are the ones that apply.
| Product | Vial, as supplied | Typical directions | Price |
|---|---|---|---|
| BPC-157 | 5 mL, 1 mg/mL | 0.18 mL (18 units) once daily as directed | $209/mo |
| Tesamorelin | 5 mL, 2 mg/mL | 0.25 mL (25 units) Monday through Friday at bedtime | $249/mo |
| MOTS-c | 5 mL, 2 mg/mL | 0.25 mL (25 units) Monday through Friday mornings | $259/mo |
| NAD+ Injection | 10 mL, 100 mg/mL | 0.83 mL (83 units) three times per week | $249/mo |
| Glutathione | 5 mL, 200 mg/mL | 0.41 mL (41 units) three times per week | $199/mo |
| Tirzepatide | 2 mL, dose-specific | 0.50 mL (50 units) once weekly | From $159/mo |
| Semaglutide | Dose-specific, 0.25 mg strength is 1 mL at 1 mg/mL | 0.25 mL (25 units) once weekly at that strength | From $99/mo |
Almost every vial is sized to last about four weeks at the printed dose, which is why refills run every 28 days, one vial per fill. How long does a peptide vial last has the full table.

Reading units on a U-100 syringe
The conversion never changes
U-100 is a concentration standard, not a syringe size: 100 units per millilitre. Every U-100 insulin syringe sold in the US is marked on that basis, so you move the decimal two places.
| Directions in mL | On a U-100 syringe |
|---|---|
| 0.10 mL | 10 units |
| 0.18 mL | 18 units |
| 0.25 mL | 25 units |
| 0.41 mL | 41 units |
| 0.50 mL | 50 units |
| 0.83 mL | 83 units |
| 1.0 mL | 100 units, the whole barrel |
A unit here is a volume mark on a barrel, not a measure of potency. Two people drawing 25 units of different medications are drawing the same volume and very different amounts of medication. The milligram figure on the label is what is in the vial or the weekly dose, not what you draw: a vial labelled 10 mg is not a 10 mg dose.
How to read the barrel
Hold the syringe vertical at eye level and read the flat front edge of the rubber plunger tip against the marking, not the raised ring behind it. Reading at an angle biases every dose the same way. Draw at a table in good light, say the number in units as you draw, and never round to an easier mark: if your label says 18 units, 20 is not close enough.
Bubbles take up volume. If 3 of your 25 units are air, you have drawn 22 units of medication. Draw slightly past your mark, hold the syringe needle-up, tap the barrel, push the bubbles back into the vial, and settle on your line.
If your directions and your syringe markings seem to disagree, do not dose. Message your care team. And never carry a unit number over from a previous fill: when the strength changes, read the new label.

The pepti Pen option
What it is
The pepti Pen is a reusable injector that takes a pre-filled cartridge, filled and sealed by the pharmacy to your prescription. You attach a fresh tip, turn the dial to the number on your label, and press. There is no draw, no vial entry and no unit conversion. The injector ships free with your first cartridge and is yours to keep; each monthly cartridge arrives with fresh tips. One click on the dial is 0.01 mL, the same as one unit on a U-100 syringe, so a 0.25 mL dose is 25 units on a syringe or 25 clicks on the dial. The dial covers 0 to 60 units per injection.
Which products offer it
Product Vial Pen cartridge BPC-157 5 mL, 1 mg/mL 3 mL, 9 mg BPC-157 BPC-157 + TB-500 5 mL 3 mL, 9 mg BPC-157 + 13.5 mg TB-500 KLOW 5 mL 3 mL CJC-1295 / Ipamorelin 5 mL 3 mL, 1.5 mg CJC-1295 + 6 mg Ipamorelin Sermorelin 5 mL, 2 mg/mL 3 mL, 9 mg Sermorelin MOTS-c 5 mL, 2 mg/mL 3 mL, 30 mg MOTS-c NAD+ Injection 10 mL, 100 mg/mL 3 mL, 600 mg NAD+ PT-141 5 mL, 2 mg/mL 3 mL, 10 mg PT-141 Thymosin Alpha-1 5 mL, 2 mg/mL 3 mL, 15 mg Thymosin Alpha-1 Every other product, including semaglutide, tirzepatide and the four-peptide blends, is vial only. The vial is the lower price on every product that offers both, and cartridges are sometimes temporarily sold out while the vial is unaffected. The pen is sold into every state Pepti serves except Alabama, where orders ship as a vial. The live list and both prices are on the pen page and each product page, and checkout confirms the format.
Pen technique in brief
Prime as the pen's instructions direct, which clears air from the needle path. Keep the needle in the skin for the count the instructions give after the dial reaches zero, because the dose is still being delivered. Remove the tip after every injection and put it in a sharps container. A cartridge is refrigerated as its label directs, in the pen or out of it; the pen itself needs no refrigeration. The full comparison is in vial vs pen.

Research powders versus a prescription vial
A research-use-only peptide is a powder sold under a written statement that it is not for human consumption. That statement is what makes the sale lawful, and it switches off every standard that would otherwise apply: no prescriber, no pharmacy licence, no required sterility process, no finished-product test, and nobody to contact if something goes wrong. Where a vendor publishes a certificate, it usually describes the purity of bulk powder before it was portioned, not the amount in your vial, its sterility or its endotoxin content.
The prescription route changes what is in the vial and who is accountable for it. It does not change what the research on a molecule has established. For most peptides that research is preclinical, and compounded medications are not FDA approved. More detail is in research peptides vs prescription peptides.

What the evidence shows, honestly
Subcutaneous self-injection with a U-100 syringe is one of the best-established home techniques in medicine, built on decades of insulin use. The pen research is also from insulin: bench and crossover studies found doses prepared with a pen were more consistent than doses drawn by eye, especially among inexperienced users, and patients mostly preferred the pen. None of that involved a compounded peptide, and it is not evidence that a peptide works better from one format. What can be said plainly is narrower: a pre-mixed vial at a stated concentration removes the reconstitution step where ten-fold errors start, and a pen removes the draw.

Common questions
Do Peptides Come Pre-Mixed, and Do You Get Needles?, answered.
Pepti's do. Every injectable arrives as a ready-to-use solution in a multi-dose vial at a stated concentration, so there is no powder and nothing to add. Research-chemical peptides usually arrive as powder you mix yourself.
No. Reconstitution has already been done by a pharmacist. Draw the volume printed on your vial with the syringes supplied.
Yes. Each Pepti vial ships with sterile U-100 insulin syringes and alcohol prep pads. Pen cartridges ship with fresh pen tips.
No. A diluent is only needed to dissolve a powder, and Pepti vials are already in solution. Do not add anything to your vial.
The number printed on your vial. Your physician sets your directions after reviewing your intake, and the label states them in both mL and units. The most common is 0.25 mL, which is 25 units.
50 units. On a U-100 syringe, 100 units is 1 mL, so move the decimal two places.
Not as a needle-free injection, but the pepti Pen replaces drawing from a vial with a dial and a short, fine pen tip. It is available for products including BPC-157, KLOW, sermorelin, MOTS-c, NAD+, PT-141 and thymosin alpha-1, in every state Pepti serves except Alabama.
Yes. The cartridge is filled and sealed by the pharmacy to your prescription and clicks into the injector. There is no draw and nothing to mix.
No. They come in the box with every vial. Use a fresh syringe and fresh pads for every dose. A sharps container is the one thing to have on hand yourself.
Not on your own initiative. Where a combination is prescribed, the pharmacy compounds it into a single vial, such as KLOW or BPC-157 + TB-500.
Yes. They ship refrigerated with ice packs and go straight into the refrigerator. Beyond-use dating runs from first puncture and is printed on the label. See which peptides need refrigeration.
No. Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account. The free assessment goes to a physician licensed in your state. A consultation does not guarantee a prescription.
