5-Amino-1MQ vs Semaglutide: Enzyme Blocker vs GLP-1, the Evidence, Cost, and Taking Them Together
This page explains each on its own terms, puts them side by side, covers what the research does and does not show, where each stands with the FDA, how each is dosed, what each costs at Pepti, and what is known about taking them together.

Treatments mentioned
5-Amino-1MQ is not a GLP-1 and does not work like semaglutide. Semaglutide is a peptide analogue of the gut hormone GLP-1 that acts on appetite, gastric emptying and insulin release, and its large randomised trials, run on the approved branded products, had weight as a primary endpoint. 5-Amino-1MQ is a small synthetic molecule, not a peptide, that blocks an enzyme called NNMT inside fat cells; in obese mice it limited fat gain with food intake unchanged, and it has never been tested in a human trial. They act on unrelated targets, some physicians prescribe them together, and compounded versions of both are not FDA approved.

5-Amino-1MQ vs Semaglutide
The short answer
| If your main goal is | The usual prescription | What is in it | Price |
|---|---|---|---|
| Appetite and body weight, first GLP-1 | Semaglutide | Dose-specific vials, from 1 mg in 1 mL at the starting strength | From $99/mo, priced by dose |
| Fat-cell metabolism through the NNMT enzyme, alongside diet and activity | 5-Amino-1MQ | 5 mg 5-Amino-1MQ in a 5 mL vial | $219/mo |
| Several metabolic mechanisms in one vial | FUSION Blend | BPC-157, MOTS-c, 5-Amino-1MQ and tesamorelin | $279/mo |
| NAD+ itself | NAD+ Injection | 10 mL vial, NAD+ 100 mg/mL (1000 mg per vial) | $249/mo |
| A dual-receptor GLP-1 option | Tirzepatide | Dose-specific 2 mL vials | From $159/mo, priced by dose |
Every figure is an all-in monthly price covering the medication, physician review, refill management and shipping. There is no separate membership fee. Semaglutide and tirzepatide are "from" prices because the vials are dose-specific and the price moves with the strength. Current pricing always lives on each product page.
5-Amino-1MQ vs Semaglutide
Is 5-Amino-1MQ a natural semaglutide?
No, and it is not marketed honestly as one. Semaglutide's main effect in the weight trials runs through appetite: it activates GLP-1 receptors in the brain that regulate hunger and fullness. The mouse study most often cited for 5-Amino-1MQ reported the opposite pattern, body weight and fat mass lower with total food intake unchanged. Whatever 5-Amino-1MQ does in mice, it does it inside cells rather than by changing how much is eaten. It is also not a peptide, which is 5-Amino-1MQ a peptide explains.
5-Amino-1MQ vs Semaglutide
Semaglutide on its own terms
What it is
Semaglutide is a 31-amino-acid analogue of glucagon-like peptide-1, a hormone the small intestine releases after a meal. Native GLP-1 is destroyed by the enzyme DPP-4 within about two minutes; semaglutide swaps one amino acid to block that, changes one other, and adds a fatty-acid chain that binds it to albumin in the blood, giving a half-life of about a week. It is injected once weekly and is not a stimulant. The full reference is at what is semaglutide, and the hormone background is in what is a GLP-1.
How semaglutide is described to work
- Appetite signalling in the brain. Receptors in the hypothalamus and brainstem regulate hunger. Patients most often describe "food noise" going quiet, and in the weight trials this carries most of the effect.
- Slower gastric emptying. Fullness lasts longer, which also explains the most common side effects: nausea, fullness and constipation.
- Glucose-dependent insulin release. Insulin is released only when blood glucose is raised, and glucagon is suppressed.
What human data exists for semaglutide
A great deal: the STEP programme for weight management, SUSTAIN in type 2 diabetes, and SELECT for cardiovascular outcomes. SELECT enrolled 17,604 adults with cardiovascular disease and no diabetes and reported cardiovascular death, heart attack or stroke in 6.5% on semaglutide against 8.0% on placebo. In STEP 4, people switched to placebo regained weight. Every trial ran the drug alongside diet and activity counselling, and the data belongs to the branded products rather than compounded versions.
5-Amino-1MQ vs Semaglutide
5-Amino-1MQ on its own terms
What it is
5-Amino-1MQ is short for 5-amino-1-methylquinolinium: a small synthetic molecule built on a two-ring quinolinium core, with no amino acids in it. It came from a medicinal chemistry programme at the University of Texas Medical Branch in Galveston, led by Stanley Watowich, which screened compounds against the enzyme nicotinamide N-methyltransferase (NNMT) in a 2017 Journal of Medicinal Chemistry paper. The target came from a 2014 paper in Nature reporting that knocking NNMT down protected mice against diet-induced obesity. The full reference is at what is 5-Amino-1MQ.
How 5-Amino-1MQ is described to work
- Blocking NNMT. NNMT takes nicotinamide, the form of vitamin B3 cells recycle into NAD+, and attaches a methyl group from SAM, so the product is excreted rather than reused. Blocking it is proposed to keep the NAD+ precursor and SAM in the cell.
- NAD+ and SAM in cultured fat cells. A 2018 paper in Biochemical Pharmacology reported higher intracellular NAD+ and SAM and less fat-building in cultured adipocytes.
- Energy expenditure in fat tissue. The 2014 Nature work, which used gene knockdown rather than this molecule, reported increased cellular energy expenditure in fat and liver.
All of the mechanism detail is from cells and mice.
What human data exists for 5-Amino-1MQ
None. There are no human trials of any design, and as of September 2026 none registered under its name. The efficacy evidence is in mice: the 2018 study reported lower body weight and white fat mass in diet-induced obese mice with food intake unchanged, and a 2024 paper in Diabetes, Obesity and Metabolism dosed it once daily for 28 days and reported limited fat gain, improved glucose tolerance and insulin sensitivity, and less fatty liver in obese mice. The only human data concerns the target enzyme: a 2015 Diabetologia study reported roughly twofold higher NNMT expression in the fat tissue of people with type 2 diabetes.

The real difference is evidence and mechanism, together
People ask which works better. No study has compared them, but the more useful answer is that they are not trying to do the same thing. Semaglutide changes how much a person wants to eat and has human trials showing that matters for weight. 5-Amino-1MQ is described as changing how fat cells handle energy, and the only place that has been measured is a mouse. One is a weight-management medication with human evidence; the other is an experimental metabolic compound. A physician reviewing an intake where weight is the main goal will usually look at a GLP-1 first.
5-Amino-1MQ vs Semaglutide
Who suits which
Semaglutide first
Semaglutide is the usual starting point when appetite and total body weight are the problem, within the BMI ranges its trials enrolled: 30 or more, or 27 or more with a weight-related condition. It has the large cardiovascular outcomes trial, the longer track record of the two GLP-1s Pepti prescribes, and the lower starting price. Semaglutide vs tirzepatide covers the GLP-1 choice.
5-Amino-1MQ
5-Amino-1MQ suits someone whose interest is fat-cell metabolism or the NAD+ angle, who is not a GLP-1 candidate on weight grounds or already has diet and activity in place, and who is comfortable with a compound tested only in mice and cells. Both of its main mouse studies paired it with a lifestyle change, a diet switch or exercise, and reported the combination doing more than either alone.
5-Amino-1MQ vs Semaglutide
Can you take 5-Amino-1MQ with semaglutide?
They act through entirely different mechanisms, and some physicians prescribe them together. What is known:
| Question | Answer |
|---|---|
| Has the combination been studied? | No. There is no human study, so no data on whether it adds anything |
| Do they compete? | Not mechanistically; semaglutide acts on receptors, 5-Amino-1MQ on an intracellular enzyme |
| Any interaction concern? | NNMT also methylates some foreign compounds, a theoretical reason to list every medication; nothing specific is known about semaglutide |
| Same syringe? | No. Each vial's directions are calculated against that vial's concentration; never mix two vials |
| Same schedule? | No. Semaglutide once weekly; 5-Amino-1MQ once daily on weekdays |
| Start both at once? | Starting on different dates makes it easier to tell which one caused any change, especially in the early weeks of a GLP-1 |
Your physician sets your directions for each after reviewing your intake and full medication list. The common pairings are in what peptides can you take with tirzepatide, which applies to semaglutide too.
5-Amino-1MQ vs Semaglutide
Where each stands with the FDA
| Semaglutide | 5-Amino-1MQ | |
|---|---|---|
| Approved product | Approved for type 2 diabetes from 2017 and chronic weight management from 2021 | None; not a component of any approved drug, no USP monograph |
| Shortage history | Shortage declared resolved 21 February 2025 | Not applicable |
| 503A bulks list | Not applicable; approved active ingredient | Not on it, and not in Category 1, 2 or 3 of the nominated list updated 14 May 2026 |
| Advisory committee | Not applicable | Never reviewed |
| Pending action | 30 April 2026 proposal to exclude it from the 503B bulks list, not final | None; never evaluated |
| Compounded status | Not FDA approved; 503A compounding for an identified patient where the prescriber documents the reason | Not FDA approved; legal to prescribe and compound |
The 503B proposal concerns outsourcing facilities that compound in bulk and does not by itself change the 503A pathway for a named patient. The FDA has never published a safety-risk finding about 5-Amino-1MQ; it has simply not been through the evaluation process. Compounded medications are not FDA approved. Are peptides FDA approved explains the difference between not approved and not legal.
5-Amino-1MQ vs Semaglutide
Dosing, side by side
Both are injected under the skin (subcutaneously), rotating sites.
Semaglutide. Once weekly, the same day each week. Each strength is a separate dose-specific vial: the starting vial holds 1 mg in 1 mL, drawn at 0.25 mL (25 units), and higher strengths use different concentrations and volumes. Treatment starts at the lowest strength and steps up, typically at roughly four-week intervals as tolerance allows. The semaglutide dosing chart shows every rung.
5-Amino-1MQ. 0.25 mL, which is 25 units on a U-100 insulin syringe, once daily Monday through Friday. From the 1 mg/mL vial that is 0.25 mg per dose. Twenty doses cover about four weeks. 5-Amino-1MQ dosage has the conversion table.
You do not need to calculate anything: your physician sets your directions after reviewing your intake, and your directions are printed on the vial. Both ship refrigerated with ice packs, with sterile syringes and alcohol prep pads, and refills run every 28 days, one vial per fill. Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.
5-Amino-1MQ vs Semaglutide
Side effects
Semaglutide. Nausea, diarrhoea, vomiting, constipation and reduced appetite are the most common, mostly during dose increases and usually easing. Less common but important: pancreatitis, gallbladder disease and dehydration. Low blood sugar is possible on insulin or a sulfonylurea, and slower gastric emptying can change how oral medications are absorbed. The approved labels carry a warning about thyroid C-cell tumours seen in rodents. Semaglutide side effects covers them.
5-Amino-1MQ. There is no human safety database. The mouse studies reported no observable adverse effects at the doses used, though they were not toxicology studies. Clinically, mostly injection-site redness, mild soreness or occasional bruising. 5-Amino-1MQ side effects has more.
For either, stop and contact your physician for any reaction that is severe, spreading or involves difficulty breathing, and for severe, persistent abdominal pain. For swelling of the face or throat or trouble breathing, call 911.

Who should not take them, or should discuss it first
| Situation | Semaglutide | 5-Amino-1MQ |
|---|---|---|
| Pregnancy or breastfeeding | Not used | Not used; safety data is absent |
| Personal or family history of medullary thyroid carcinoma or MEN 2 | Contraindicated | Not a specific concern |
| History of pancreatitis or gallbladder disease | Needs careful discussion | Not a specific concern |
| Taking several prescription medications | Raise it; absorption of oral drugs can change | Raise it; theoretical NNMT interaction point |
| Tested athletes | Confirm your own medication | Check the current list |
5-Amino-1MQ vs Semaglutide
What the evidence shows, honestly
Semaglutide has one of the largest human evidence bases of any weight-management medicine, for the branded products, run alongside diet and activity, and weight tends to return after it is stopped. 5-Amino-1MQ has mouse and cell studies from essentially one research group and human data only on its target enzyme. No trial has tested a compounded vial of either, or the two together. Anyone presenting 5-Amino-1MQ as a semaglutide alternative, or as shown to change weight in people, is describing research that does not exist. Best peptides for weight loss and peptides vs GLP-1 put both in context.

Common questions
5-Amino-1MQ vs Semaglutide, answered.
No. It is a small molecule that blocks the enzyme NNMT inside cells. It does not act on the GLP-1 receptor, appetite or gastric emptying.
There is no study comparing them. Semaglutide has large human trials for weight management; 5-Amino-1MQ has only mouse and cell studies. For weight, semaglutide is the one with human evidence.
No. The 2018 mouse study reported total food intake unchanged in treated animals. Its described mechanism is intracellular, around NNMT, NAD+ and energy use in fat tissue.
Some physicians prescribe them together because they act through different mechanisms. There is no human study of the combination. They are separate prescriptions on separate schedules, drawn in separate syringes.
There is no human evidence for that. A 2024 study in aged mice reported greater grip strength with an NNMT inhibitor, but that was not on a GLP-1 and not in people. Protein intake and resistance training are the countermeasures with the most behind them.
No. At Pepti, 5-Amino-1MQ is $219 a month and semaglutide starts from $99 a month, priced by dose. They are not interchangeable in any case.
No. Semaglutide is an approved active ingredient, but compounded preparations are not FDA approved. 5-Amino-1MQ has no approved product and has never been evaluated by the FDA.
In cultured fat cells, NNMT inhibitors raised intracellular NAD+ and SAM. That has not been measured in a person. Semaglutide has no described NAD+ effect.
At Pepti, 5-Amino-1MQ is $219 a month and semaglutide starts from $99 a month, rising with the strength. Every figure is all-in, covering medication, physician review, refill management and shipping.
