The mechanism most often cited for Selank is at the GABA-A receptor, the receptor family benzodiazepines act on. A 2018 paper in Protein and Peptide Letters used isolated rat brain-cell membranes and reported that Selank changed the binding of GABA in the manner of a positive allosteric modulator: it did not switch the receptor on directly, but altered how GABA itself bound. Selank and diazepam together produced a binding pattern that was not simply the sum of the two. Gene-expression work in 2016 and 2017 pointed the same way: a peptide that modifies GABA signalling rather than driving it on its own.

Alcohol is a sedative, and sedatives that act on the same receptor system are the combinations physicians are most careful with. Nobody has studied how Selank and alcohol behave together, so nobody can tell you whether they add up, cancel out or do nothing to each other. The Russian trials reported that Selank was not sedating, with no memory impairment or dependence signal, but those patients were not drinking. That is the reason the cautious answer is to keep drinking modest, to tell your physician what you actually drink, and never to treat the absence of data as permission.

There is no meaningful drug-interaction literature for Selank, because the trials that would produce one have not been run. The one combination studied is with a benzodiazepine, under supervision: the 2015 trial gave 40 patients phenazepam plus Selank and reported that the combination brought the benzodiazepine's effect on earlier and reduced its side effects. Selank alongside SSRIs, SNRIs, stimulants, antipsychotics or alcohol has not been studied at all. Judging how a new treatment is going

Selank's described effect is subtle: prescribers commonly describe a sense of being less reactive to stress rather than anything felt at the time of injection, and some people describe early fatigue or a flat feeling, or less often feeling wired. Drinking clouds exactly that kind of signal. If you want to know whether Selank is doing anything for you, the first few weeks are the time to keep everything else, alcohol included, as steady as possible and to keep a simple log.

Dosing and drinking on the same day

Selank acetate was placed in Category 2 of the FDA's 503A bulk substances list on 29 September 2023 and removed on 27 September 2024, after the nominator said it had intended to nominate a different substance. It now appears only as "nominated but withdrawn," and it has not been reviewed by the Pharmacy Compounding Advisory Committee.
