PT-141 + Oxytocin for Women: The Trials Behind Each Component, and the Cautions

This page covers what each component's research in women measured, what it does not cover, the considerations that matter for women, how the combination compares with PT-141 and oxytocin alone, side effects, and how to get it prescribed. The PT-141-only version is PT-141 for women, and the full product reference is what is PT-141 + Oxytocin.

For women, the PT-141 + Oxytocin vial pairs the peptide with the most research in women, PT-141, with oxytocin, whose best-controlled trial in women found no difference from placebo, and the combination itself has never been studied. PT-141 (bremelanotide) has two phase 3 randomised trials in 1,267 premenopausal women with low desire that reported modest, statistically significant improvements in desire and distress. A 2015 randomised crossover trial of nasal oxytocin in 30 pre- and postmenopausal women with sexual dysfunction found both arms improved with no treatment effect, while a 2014 couples study reported women feeling more relaxed after sex. Oxytocin contracts the uterus, so the vial is never used in pregnancy, and compounded medications are not FDA approved.

PT-141 + Oxytocin for Women

The short answer

Question What the research shows Strength of evidence
PT-141 and low desire, premenopausal women Two phase 3 RCTs, RECONNECT, Obstetrics & Gynecology, 2019, 1,267 women Strong for that group; modest in absolute size
PT-141 over a year 52-week open-label extension, 684 women No new safety signals; not placebo-controlled
Oxytocin in women with sexual dysfunction 2015 randomised crossover trial, Fertility and Sterility, 30 women, nasal spray Negative: no better than placebo
Oxytocin and the experience after sex 2014 couples study, Hormones and Behavior, 29 couples, nasal spray Small study; small to moderate effects
After menopause PT-141 not studied; the oxytocin trial included postmenopausal women and was null Little to none
The combination in women Never studied None
Pregnancy Never used: oxytocin contracts the uterus, and the PT-141 label advises against it Firm exclusion

PT-141 + Oxytocin for Women

What is in the vial

Component Per 0.10 mL dose What it is
PT-141 (bremelanotide) 0.2 mg A melanocortin receptor agonist described around the "wanting" side of desire
Oxytocin 0.1 mg A hormone with established roles in labour and milk let-down, released around orgasm

PT-141 + Oxytocin is a 5 mL vial holding 10 mg PT-141 and 5 mg oxytocin, in a fixed ratio. The rationale prescribers give is that PT-141 is aimed at desire and initiation while oxytocin is aimed at the orgasmic phase and closeness afterwards. That is a rationale, not a finding. Neither component raises oestrogen or testosterone, and neither acts on blood flow.

What PT-141's research in women looked at

RECONNECT: the two phase 3 trials

Two identical 24-week, randomised, double-blind, placebo-controlled trials, published together in Obstetrics & Gynecology in 2019, enrolled 1,267 premenopausal women with acquired, generalised hypoactive sexual desire disorder. They self-injected 1.75 mg bremelanotide or placebo as needed.

Measure Result, compared with placebo
Desire domain of the Female Sexual Function Index Improved 0.30 and 0.42 points more in the two trials
Desire-related distress Improved 0.37 and 0.29 points more
Statistical significance Met in both trials, on both co-primary endpoints

Those are modest absolute differences, and they supported FDA approval of a bremelanotide product on 21 June 2019. Low desire explained by a medical or psychiatric condition, a relationship problem or a medication was excluded. A 52-week open-label extension followed 684 women; no new safety signals emerged, and most used it two to three times a month.

How PT-141 is described to work

Animal studies, reviewed in CNS Spectrums in 2022, suggest bremelanotide activates MC4 receptors in the medial preoptic area of the hypothalamus and increases dopamine release there. The approved label states the mechanism for desire is unknown, so that is the leading explanation, not a settled one.

What oxytocin's research in women looked at

  • The 2015 trial in women with sexual dysfunction

A randomised, double-blind, placebo-controlled crossover trial in Fertility and Sterility gave 30 pre- and postmenopausal women with sexual dysfunction 32 IU of intranasal oxytocin or placebo before intercourse, eight weeks on each. Sexual function improved substantially in both arms: the Female Sexual Function Index rose 26% on oxytocin and 31% on placebo, with no treatment effect. It is the best-controlled trial of oxytocin in women with an actual diagnosis, and it was null. It is also a reminder that attending to intimacy on a schedule may itself be part of what people report.

  • The 2014 couples study

29 healthy couples took 24 IU of intranasal oxytocin or placebo before sex at home. Oxytocin did not change drive, arousal or lubrication. It was associated with higher orgasm intensity and contentment after sex; women reported feeling more relaxed and, in subgroups, better able to share sexual desires and empathise with their partner. The effects were described as small to moderate, and more pronounced in men.

  • Physiology

A 1987 paper in the Journal of Clinical Endocrinology and Metabolism measured plasma oxytocin rising during arousal and peaking around orgasm in both sexes. Oxytocin acts on a receptor found on uterine smooth muscle and the milk-producing tissue of the breast, which is the basis of its approved obstetric use and of the cautions below.

What the research in women does not cover

  • The combination. No study has tested PT-141 and oxytocin together.
  • The compounded dose. RECONNECT used 1.75 mg by autoinjector; a combination dose is 0.2 mg PT-141 with 0.1 mg oxytocin.
  • Injected oxytocin. Every oxytocin study used a nasal spray measured in IU, which cannot be converted to milligrams on a vial.
  • Postmenopausal women and PT-141. Both RECONNECT trials enrolled premenopausal women, and the approved product is not indicated after menopause.
  • Arousal, lubrication or orgasm as primary PT-141 outcomes. The phase 3 trials measured desire and distress.
  • Long-term safety of oxytocin injection at these doses. Those studies have not been run.

What changes for women

  • Low desire is rarely one thing

Sleep, pain with intercourse, relationship context, medications such as SSRIs, and hormonal transitions all play a part. SSRIs and SNRIs are among the commonest reversible causes; beta blockers and oral contraceptives are each documented to affect libido in some people. If desire changed when a new medication started, that is a conversation with whoever prescribed it. Peptides for libido sets out what a physician looks at first.

  • Perimenopause and menopause

Vaginal dryness and pain with sex are often the actual problem after menopause, and neither component addresses them; local treatment usually comes first. Where desire itself has changed after menopause, PT-141 is an extrapolation from premenopausal evidence, and the one oxytocin trial that included postmenopausal women found no difference from placebo. More is in peptides for menopause and peptides for women.

  • Pregnancy

The vial is never used in pregnancy, for two separate reasons. Oxytocin contracts the uterus; that is its primary peripheral action and the reason for its approved use in labour. And the approved bremelanotide labeling reports fetal harm in pregnant dogs at exposures at or above 16 times the maximum recommended human dose, directs women to use effective contraception while taking it, and states that use in pregnancy is not recommended. If you think you may be pregnant, stop and contact your physician the same day. See can you take peptides while pregnant.

  • Contraception

Bremelanotide may slow gastric emptying and reduce absorption of oral medications taken around the same time, so tell your physician which contraceptive method you use.

  • Breastfeeding

Oxytocin is the hormone that triggers milk ejection, and there is no human lactation data for bremelanotide. The vial is not used while breastfeeding.

  • Blood pressure and pigmentation

Each dose of PT-141 produces a transient mean rise of about 6 mmHg systolic and 3 mmHg diastolic, peaking two to four hours after the dose and usually resolved by twelve; the approved product is contraindicated in uncontrolled hypertension and known cardiovascular disease. Focal darkening of the face, gums or breasts was reported in 1% of women dosing up to eight times a month, more with more frequent dosing, and not always reversible. That is why the vial is capped by month and used as needed, never daily.

  • Naltrexone

Bremelanotide can significantly reduce exposure to oral naltrexone, so anyone taking naltrexone should not use this vial.

PT-141 + Oxytocin for Women

The combination vs each component alone

PT-141 + Oxytocin PT-141 alone Oxytocin alone
Evidence in women None for the combination Two phase 3 trials, premenopausal One null trial in women with dysfunction; one small couples study
Aimed at Desire plus the orgasmic phase and closeness Desire and initiation The experience around and after orgasm
Main side effects PT-141's nausea, flushing, blood pressure rise Same Few described at these doses
Per dose at Pepti 0.2 mg PT-141, 0.1 mg oxytocin Printed on the vial Printed on the vial
Price $259/mo $259/mo $239/mo

There is no evidence that the combination is better for women than PT-141 alone. If desire is the whole problem, PT-141 alone is the more targeted prescription. If the interest is closeness or the post-orgasmic phase with no complaint about desire, oxytocin alone avoids PT-141's nausea and blood-pressure profile. How the two compare is in PT-141 vs oxytocin, and kisspeptin, which acts on the reproductive hormone axis, is compared in kisspeptin vs oxytocin.

PT-141 + Oxytocin for Women

Side effects, from the women's trials

The combination's side effects are dominated by its PT-141 component. These rates are from the phase 3 trials of bremelanotide at 1.75 mg.

What was reported How often What to do
Nausea 40% of women, against 1.3% on placebo; 8% left the trials because of it Usually worst with the first dose; tell your care team if it is marked
Flushing 20% Usually brief
Injection-site reactions 13% Rotate sites
Headache 11% Report it if it persists
Vomiting 5% Report it
A transient blood pressure rise Each dose Blood pressure should be controlled before starting
New skin darkening 1% at up to eight doses a month Report it promptly
Chest pain, facial or throat swelling, difficulty breathing Rare Call 911, then tell your physician

There are no systematic safety data for oxytocin at the small subcutaneous doses in this vial. Never take a second dose within 24 hours. The approved bremelanotide label instructs stopping after eight weeks if a woman reports no improvement; if nothing has changed after two months used as directed, tell your physician rather than reaching for more doses. More is in PT-141 side effects and oxytocin side effects.

Where it stands with the FDA

Bremelanotide is the active ingredient of a product the FDA approved on 21 June 2019 for premenopausal women with acquired, generalised hypoactive sexual desire disorder. Oxytocin is the active ingredient of an approved hospital injection for labour and postpartum bleeding; intimacy is not an approved use. Both may be compounded by a licensed 503A pharmacy as components of approved products, and neither was ever in Category 2. The combination vial is a different formulation prepared to an individual prescription, has not been reviewed by the FDA, and is not FDA approved.
