PT-141 + Oxytocin Dosage: Units, mL, mg and Timing for the Combination Vial

This page explains the PT-141 + Oxytocin dosage Pepti prescribes, what the units on the syringe mean, how much of each peptide is in a dose, a chart for this vial, why its numbers differ from PT-141 alone and oxytocin alone, how to use it on the day, how long a vial lasts, what the published studies used, and the side effects to know about. It covers how Pepti prescribes this vial, not protocols from the internet. Nothing here is a reason to change your own dose.

PT-141 (Injectable) — pepti Pen
PT-141Vial $259 · Pen $319

At Pepti, PT-141 + Oxytocin is prescribed as 0.10 mL, which is 10 units on a U-100 insulin syringe, injected under the skin 2 to 4 hours before sexual activity, no more than 8 doses a month. The 5 mL vial holds 10 mg of PT-141 (bremelanotide) and 5 mg of oxytocin, so every 10 unit dose delivers 0.2 mg PT-141 and 0.1 mg oxytocin together, and the vial holds 50 doses. It is used on the occasion, never more than once in 24 hours. Your physician sets your directions after reviewing your intake, and your exact dose and units are printed on the vial.

PT-141 + Oxytocin Dosage

The short answer

Product Vial Typical directions Per dose Doses per vial Price
PT-141 + Oxytocin 5 mL, PT-141 2 mg/mL + oxytocin 1 mg/mL (10 mg PT-141 / 5 mg oxytocin per vial) Inject 0.10 mL (10 units) subcutaneously 2 to 4 hours prior to sexual activity. No more than 8 doses per month 0.2 mg PT-141 and 0.1 mg oxytocin 50 $259/mo

The price is all-in, covering medication, physician review, refill management and shipping, with no separate membership fee. Pepti is cash-pay and does not bill insurance. These are the typical printed directions; your own directions are the ones on your vial.

PT-141 + Oxytocin Dosage

What is in the vial

PT-141 + Oxytocin is two separately studied peptides dissolved together in one sterile multi-dose vial by a compounding pharmacy. PT-141 is bremelanotide, a seven-amino-acid cyclic peptide that activates melanocortin receptors in the brain and is described around the desire side of sexual response. Oxytocin is a nine-amino-acid hormone the body already makes, released around orgasm, with established roles in labour and milk let-down. Both are the active ingredients of FDA-approved drugs for other purposes; the combination itself has never been studied as a combination, in people or in animals. The full reference is what is PT-141 + Oxytocin.

Component Concentration Per vial Per 0.10 mL dose
PT-141 (bremelanotide) 2 mg/mL 10 mg 0.2 mg
Oxytocin 1 mg/mL 5 mg 0.1 mg
Total 15 mg 0.3 mg

The ratio is fixed, two parts PT-141 to one part oxytocin by mass, so every draw delivers the same split. Prescribers aim PT-141 at desire and oxytocin at the orgasmic phase and closeness afterwards; that is a rationale, not a finding, and no study has tested whether oxytocin adds anything to PT-141.

PT-141 + Oxytocin Dosage

PT-141 + Oxytocin dosage in units, mL and mg

Pepti's directions are written in two forms, millilitres and units, because prescriptions are written in millilitres and insulin syringes are marked in units. On a U-100 insulin syringe, 1 unit is 0.01 mL, so 100 units is 1 mL and 10 units is 0.10 mL. A unit is a volume mark on the barrel, not a measure of how strong the medication is, and it is not the same thing as the "IU" used for oxytocin in hospital products and nasal spray studies. Units on the syringe only mean something alongside the concentrations on the label.

What you read Value Where it comes from
Dose volume 0.10 mL Printed on the vial
On a U-100 insulin syringe 10 units 0.10 mL x 100 units per mL
PT-141 concentration 2 mg/mL Printed on the vial
PT-141 per dose 0.2 mg 2 mg/mL x 0.10 mL
Oxytocin concentration 1 mg/mL Printed on the vial
Oxytocin per dose 0.1 mg 1 mg/mL x 0.10 mL
Doses per vial 50 5 mL divided by 0.10 mL
Monthly ceiling 8 doses Printed on the vial

That arithmetic is a way to read a label, not a way to set a dose. A vial labelled 10 mg PT-141 and 5 mg oxytocin is not one dose: it is fifty small ones. If your label and your syringe seem to disagree, do not dose; message your care team. How long does a peptide vial last explains every number on a vial label.

PT-141 + Oxytocin Dosage

PT-141 + Oxytocin dosage chart: units to mL to mg

The chart below is the conversion for this vial, drawn with a U-100 insulin syringe. It is math only. It is not a menu of doses, and the only line that applies to you is the one that matches the number printed on your vial.

Syringe mark (U-100) Volume PT-141 Oxytocin
5 units 0.05 mL 0.1 mg 0.05 mg
10 units (Pepti's typical directions) 0.10 mL 0.2 mg 0.1 mg
15 units 0.15 mL 0.3 mg 0.15 mg
20 units 0.20 mL 0.4 mg 0.2 mg
25 units 0.25 mL 0.5 mg 0.25 mg
50 units 0.50 mL 1 mg 0.5 mg
100 units 1 mL 2 mg 1 mg

The rule behind every row: units divided by 100 gives millilitres; multiply millilitres by 2 for PT-141 and by 1 for oxytocin. Those multipliers only hold for this vial. A chart written for the PT-141 vial or the oxytocin vial gives different figures for the same units, which is the main reason not to dose from a chart, including this one.

How the combination compares with PT-141 alone and oxytocin alone

People often assume the combination is PT-141 with a little oxytocin added at the usual PT-141 dose. It is not. The three products are different concentrations with different printed directions:

PT-141 + Oxytocin PT-141 alone Oxytocin alone
What is in the vial 10 mg PT-141, 5 mg oxytocin in 5 mL 10 mg PT-141 in 5 mL (2 mg/mL); also as a Pen 10 mg oxytocin in 5 mL (2 mg/mL)
Typical directions 0.10 mL (10 units) 2 to 4 hours before sexual activity, no more than 8 doses a month 0.50 mL (50 units) 2 to 4 hours before sexual activity, no more than 4 doses a month 0.18 mL (18 units) once daily
Per dose 0.2 mg PT-141 and 0.1 mg oxytocin 1 mg PT-141 0.36 mg oxytocin
Doses per vial 50 10 27 whole doses
Price $259/mo $259/mo $239/mo

What the numbers show

A dose from the combination vial carries one fifth of the PT-141 in a dose from the PT-141 vial, which is why its monthly ceiling is set differently. The oxytocin per dose is also smaller than a daily dose from the oxytocin vial, and it is given on the occasion rather than every day. The labels are not interchangeable: 10 units from the combination vial, 50 units from the PT-141 vial and 18 units from the oxytocin vial are three different products with three different directions. Never carry a number from one label to another, and never use two of these products together unless your physician has prescribed exactly that. The PT-141 numbers are covered in PT-141 dosage.

PT-141 + Oxytocin Dosage

How to use PT-141 + Oxytocin

PT-141 + Oxytocin is not a daily medication. Each dose is taken for an occasion, and each one stands on its own. This is the practical sequence, in the order it happens.

  • 1. Check the label first

Read the dose, the timing window and the monthly ceiling on your vial. The label is the instruction; this page is background. Check the beyond-use date as well, because a vial used a few times a month can sit in the fridge for a while between uses.

  • 2. Time it 2 to 4 hours ahead

Pepti's directions say 2 to 4 hours before sexual activity. That window is set by the PT-141 component: the approved bremelanotide label reports peak blood levels about an hour after a subcutaneous dose and a half-life of about 2.7 hours, and the blood pressure effect peaks two to four hours after a dose. The live oxytocin reference describes oxytocin as fast and short, with a plasma half-life of a few minutes. How the oxytocin component behaves on a 2 to 4 hour timing, given alongside PT-141, has not been studied. Your own directions say which window applies to you.

  • 3. Draw and inject under the skin

Use a fresh sterile syringe every time, wipe the vial stopper and the skin with an alcohol prep pad, draw to the 10 mark if that is what your label says, and inject into the fat layer under the skin, not the muscle. The abdomen and thigh are the usual sites, rotated. A 10 unit draw is small, so read the barrel at eye level. Technique is in how to inject peptides safely.

  • 4. One dose, then wait

Never take a second dose within 24 hours. The approved bremelanotide label states that consecutive doses within 24 hours have not been shown to add anything and may add to the effect on blood pressure. If a dose did not seem to work, that is a message to your care team, not a reason for another draw.

  • 5. Expect the first dose to be about side effects

Nausea, when it happens, usually starts within an hour and lasts about two, and it is most common with the first dose. Prescribers suggest taking a first dose on an occasion where a poor response would not matter.

  • 6. Back in the fridge

Return the vial to the refrigerator and do not freeze it. Vials ship refrigerated with ice packs, and the beyond-use date runs from first puncture. How to store peptides covers storage.

Directions say What it means in practice
0.10 mL (10 units) Draw to the 10 mark on a U-100 insulin syringe
Subcutaneously Into the fat layer, abdomen or thigh, rotating sites
2 to 4 hours prior to sexual activity Dose ahead of time, not at the moment
No more than 8 doses per month A ceiling, not a target
Once in 24 hours at most No second dose the same day

PT-141 + Oxytocin Dosage

How long a PT-141 + Oxytocin vial lasts

Because it is used on occasions, a vial is not consumed on a calendar the way a weekday peptide is. The arithmetic:

Step Value
Dose 10 units, 0.10 mL
Doses per vial 50 (5 mL divided by 0.10 mL)
Used over the full vial 10 mg PT-141 and 5 mg oxytocin
Ceiling 8 doses a month
At the ceiling 50 doses is more than six months of doses
In practice The beyond-use date printed on the label decides when the vial is replaced

The dose count will almost always outlast the beyond-use date, so do not read a half-full vial as a reason to use it more often, and do not keep using it past the date on the label. One vial per fill, never a stockpile. If you have a question about when your next vial ships, message your care team.

PT-141 + Oxytocin Dosage

Why the monthly ceiling exists

The ceiling is a safety limit taken from the approved bremelanotide label, which caps use at eight doses a month and once in 24 hours. It comes from two effects of the PT-141 component:

Effect What was reported Why it limits frequency
Blood pressure A transient rise of about 6 mmHg systolic and 3 mmHg diastolic at most, peaking two to four hours after a dose, usually back to baseline within twelve, with heart rate falling by up to 5 beats a minute Each dose adds hours of raised blood pressure
Focal hyperpigmentation 1% of women dosing up to eight times a month; 38% of people dosed daily for eight days in a separate study; face, gums and breasts; not always reversible Risk rises with more frequent dosing

A compounded vial does not change that pharmacology, and the smaller PT-141 amount per dose in the combination vial has not been studied for these effects either way.

PT-141 + Oxytocin Dosage

What the studies used, and why that is not your dose

Both components have human research, and the doses in it are often quoted online as if they were protocols. None of it tested this vial:

Study Who How it was dosed
RECONNECT, two phase 3 trials, Obstetrics & Gynecology, 2019 1,267 premenopausal women with acquired, generalised hypoactive sexual desire disorder PT-141 1.75 mg by single-dose autoinjector, as needed, for 24 weeks
Two 2004 studies, International Journal of Impotence Research Healthy men and men with mild-to-moderate erectile dysfunction PT-141 subcutaneously above 1 mg, and 4 mg and 6 mg in men who responded inadequately to sildenafil
2014 couples study, Hormones and Behavior 29 healthy couples Oxytocin 24 IU by nasal spray before sex at home
2015 crossover trial, Fertility and Sterility 30 pre- and postmenopausal women with sexual dysfunction Oxytocin 32 IU by nasal spray before intercourse, eight weeks per arm
Any study of the combination Nobody Never run

The honest reading: the PT-141 trials used doses well above the 0.2 mg in a combination dose, and every oxytocin study used a nasal spray measured in IU, which cannot be converted to the milligrams on this vial. The 2014 couples study reported effects on orgasm intensity and contentment after sex, not on desire or arousal; the 2015 trial in women found oxytocin no better than placebo. That is why your directions come from a physician who has reviewed your intake, not from a study table. What the research looked at is in PT-141 benefits and what is oxytocin.

PT-141 + Oxytocin Dosage

What changes the directions

Your physician sets your directions after reviewing your intake. Things that can lead to different directions, or a different product, include:

What can change How it shows up
Combination or a single component If desire is the whole question, PT-141 alone has its own label; if closeness is, oxytocin alone has its own
Blood pressure or heart history Uncontrolled hypertension and known cardiovascular disease are reasons it is not prescribed
Your other medications PT-141 can slow gastric emptying and reduce absorption of oral medicines taken around the same time, and it should not be used with oral naltrexone
Pregnancy Not used; oxytocin contracts the uterus
How you respond Marked nausea, new skin darkening or anything unexpected goes to your physician, who may revise your directions

When directions change, the new numbers are printed on the new label. Never carry a number over from a previous fill. Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.

PT-141 + Oxytocin Dosage

Side effects, in summary

The combination's side effects are dominated by its PT-141 component. The rates below are from the phase 3 trials of bremelanotide at 1.75 mg; there are no systematic safety data for oxytocin at the small subcutaneous doses in this vial. The full picture is in PT-141 side effects.

What is reported How often in the phase 3 trials What to do
Nausea 40% of women vs 1.3% on placebo Usually eases after the first dose; tell your care team if it is marked
Flushing 20% Usually short-lived
Injection-site reactions 13% Rotate sites
Headache 11% Report it if it persists
Vomiting 5% Report it
New darkening of skin, gums or breasts 1% at up to eight doses a month Report it promptly
Chest pain, a severe headache, trouble breathing, or an erection lasting more than four hours Not routine Call 911 or go to an emergency department, then tell your physician

If you think you have drawn too much, for example 50 units instead of 10, call Poison Control at 1-800-222-1222 and message your care team. Do not take another dose to compensate for anything.

Who should not take PT-141 + Oxytocin, or should discuss it first: anyone with uncontrolled high blood pressure or heart disease; anyone taking oral naltrexone; anyone with a history of melanoma or atypical moles; anyone pregnant or breastfeeding. Bremelanotide is not named on the 2026 WADA Prohibited List, but tested athletes should check their own sport's current rules.

Both active ingredients are components of FDA-approved drugs, which is how a licensed pharmacy may compound them, but the combination vial is a different formulation prepared to a prescription. It has not been reviewed by the FDA, and compounded medications are not FDA approved.

Where to get PT-141 + Oxytocin prescribed

PT-141 + Oxytocin cannot be bought legitimately without a prescription. Sites selling it "for research use only" are not accountable for what is in the vial: identity, purity, sterility and concentration are untested, so even the two concentrations your units chart depends on are unknown. The prescription route:

  1. Complete a free online intake covering your history, medications, blood pressure and what you want help with.
  2. A physician licensed in your state reviews it, usually within 24 hours.
  3. If it is appropriate, a PCAB-accredited, FDA-registered US 503A compounding pharmacy prepares it to that prescription. Lot test results are published at /quality/lab-results.
  4. It ships refrigerated with ice packs, with your directions printed on the vial, sterile syringes and alcohol prep pads.
  5. Your physician stays reachable for questions and dose changes.
