Can You Take Dihexa With Tirzepatide or Semaglutide?
This page covers what each medication is, what is and is not known about taking them together, the cancer-history question, how the symptoms of each can overlap in the early weeks, how the schedules fit, and how to add Dihexa properly. Everything here applies to semaglutide the same way.

Treatments mentioned
Dihexa can be prescribed alongside tirzepatide or semaglutide when a physician reviewing your intake judges it appropriate, but nobody has studied the combination, and Dihexa has never been studied in people at all. The two act on unrelated systems: tirzepatide on appetite and gastric emptying through the GIP and GLP-1 receptors, Dihexa on a brain angiotensin IV signal described in rodent memory studies. There is no established interaction list for Dihexa, so "no known interaction" here means "not studied," not "shown to be fine." A personal or family cancer history matters for both, for different reasons, and has to be on your intake.

Can You Take Dihexa With Tirzepatide or Semaglutide?
The short answer
| Question | Answer |
|---|---|
| Can they be prescribed together? | Yes, when a physician reviewing your intake judges it appropriate |
| Has the combination been studied? | No. Dihexa has no human trials of any kind, alone or combined |
| Do they act on the same system? | No. Tirzepatide acts at the GIP and GLP-1 receptors; Dihexa is an angiotensin IV analogue described around brain signalling in rodents |
| Is there an established interaction? | No interaction list exists for Dihexa, because the trials that would produce one have not been run |
| Does Dihexa affect weight or appetite? | Nothing in its research describes either |
| What needs raising first? | Any personal or family history of cancer, including thyroid cancer |
| Same answer for semaglutide? | Yes |
The medications this page covers:
| Product | What is in it | Typical directions | Price |
|---|---|---|---|
| Dihexa | 5 mL multi-dose vial, Dihexa 1 mg/mL (5 mg per vial) | 0.25 mL (25 units) under the skin once daily, Monday through Friday | $219/mo |
| Tirzepatide | Dose-specific 2 mL vials at six strengths, 2.5 mg to 15 mg weekly; glycine in the 2.5 mg and 7.5 mg vials | 0.50 mL (50 units) once weekly at every strength | From $159/mo |
| Semaglutide | Dose-specific vials at six strengths, starting with 1 mg per 1 mL at the 0.25 mg starting dose | Printed on the vial for your strength | From $99/mo |
Prices are all-in, covering medication, physician review, refill management and shipping, with no membership fee. GLP-1 prices are "from" prices, set by dose. Compounded medications are not FDA approved, and Dihexa is not an approved drug anywhere.

Two medications with nothing in common
Tirzepatide
Tirzepatide is a single peptide that activates two incretin receptors, GIP and GLP-1. It acts on appetite centres in the brain, slows gastric emptying and changes insulin and glucagon signalling. It has a very large human trial base in its approved form. See what is tirzepatide.
Dihexa
Dihexa is a very small, chemically stabilised peptide built from a fragment of angiotensin IV. It was developed at Washington State University after rodent work showed angiotensin IV analogues improving memory-task performance. Its developers proposed that it works through hepatocyte growth factor and its receptor, c-Met, but the two papers carrying that mechanism were retracted in April 2025. An independent 2021 mouse study pointed to PI3K/Akt signalling. It is not a stimulant and does not act on dopamine or noradrenaline. See what is Dihexa.
Side by side
Tirzepatide Dihexa What it is A dual GIP and GLP-1 receptor agonist A stabilised angiotensin IV analogue Described target Appetite, gastric emptying, insulin and glucagon Brain signalling linked to memory in rodents Human evidence Large randomised trials of the approved product None. The FDA states it has not identified any human exposure data for dihexa acetate by any route Long-term safety data Years of trial follow-up for the approved product None Schedule at Pepti Once weekly Once daily, Monday through Friday FDA status Compounded versions are not FDA approved Removed from Category 2 in April 2026; advisory committee review scheduled before the end of February 2027 There is no shared receptor and no described pathway linking the two. That makes an interaction unlikely on mechanism. It does not make the pairing tested, and with Dihexa there is no human data of any kind to lean on.

The cancer-history question, from two directions
This is the point that matters most when the two are considered together, because each medication raises it for its own reason.
| Tirzepatide and semaglutide | Dihexa | |
|---|---|---|
| The concern | A boxed warning: thyroid C-cell tumors in rodent studies, with human relevance unknown | Theoretical: the proposed mechanism runs through HGF and c-Met, a growth pathway over-active in many cancers |
| What is known in people | Not shown to cause cancer in humans; human studies point different ways | Nothing either way; there is no human data |
| Who is ruled out or cautioned | A personal or family history of medullary thyroid carcinoma or MEN 2 rules both GLP-1s out | Many physicians will not prescribe Dihexa with a personal cancer history; a strong family history needs discussion |
Neither concern is a finding of harm in people. But a person thinking about both medications should expect their physician to ask about cancer history in detail, and should put any personal or family history on the intake, including the type of cancer. More on how this question is handled across peptides is in can peptides cause cancer.

Overlapping symptoms in the early weeks
Starting two medications close together makes it hard to tell which one is behind a new symptom, and the early weeks of each have something to report.
| Symptom | On tirzepatide | On Dihexa |
|---|---|---|
| Fatigue | 5 to 7% on the approved weight-management label, against 3% on placebo | Not described in prescriber reports |
| Dizziness | 4 to 5% on the label, against 2% | Not described in prescriber reports |
| Headache | Not in the label figures on the tirzepatide side effects page | Prescribers describe mild headache, mostly early in a course |
| Restless or "wired" feeling, poor sleep | Not in the label figures | Described in the first days, and poor sleep if dosed late in the day |
| Nausea, reflux, constipation | The most common effects, around starting and dose increases | Not described in prescriber reports |
| Injection-site redness | 6 to 8% on the label | Reported |
Dihexa's column comes from prescriber reports, not trial figures, because no trial exists. The practical point is timing: start Dihexa at a point when your tirzepatide dose is steady, not in the same week as a dose increase.
A note on "brain fog"
Some people on a GLP-1 describe feeling slower or foggier, and reach for a cognitive peptide. Eating and drinking much less than before is a common reason to feel tired on a GLP-1, and new or worsening memory problems in an adult need a proper work-up before any peptide: thyroid, B12, sleep, mood and medications. Tell your care team what you are eating and how you are sleeping before asking for a second medication.

Fitting the schedules together
| Dihexa | Tirzepatide | |
|---|---|---|
| Draw | 0.25 mL (25 units on a U-100 insulin syringe) | 0.50 mL (50 units) |
| Medication per dose | 0.25 mg Dihexa | Your weekly strength, 2.5 mg to 15 mg |
| How often | Once daily, Monday through Friday, weekends off | Once weekly, same day each week |
| Doses per vial | 20 | 4 |
| One vial lasts | About four weeks | Four weeks |
The Dihexa figures are math from the vial: 0.25 mL of a 1 mg/mL solution is 0.25 mg, and 5 mL at 0.25 mL per dose is 20 doses, five a week for four weeks. Both run on a 28-day refill cycle, one vial per fill. Morning dosing of Dihexa is usual because of the sleep reports; follow the timing on your label.
- Separate syringes, separate sites. Never mix two vials in one syringe; each label was calculated for its own concentration.
- Keep your tirzepatide day fixed. If it falls on a weekday, both injections happen that day at different sites.
- Refrigerate both, away from light, and observe the beyond-use date on each label.

Who should discuss it first
| Situation | Why |
|---|---|
| Personal history of cancer | Many physicians will not prescribe Dihexa; it must be on your intake |
| Personal or family history of medullary thyroid carcinoma or MEN 2 | Rules out tirzepatide and semaglutide |
| Strong family history of cancer | Needs discussion before Dihexa |
| Pregnancy, planning pregnancy or breastfeeding | Neither is used |
| New memory problems not yet evaluated | A medical work-up first |
| Diagnosed or suspected dementia | A neurologist; Dihexa is not a treatment for it |
| Insulin or a sulfonylurea | Low blood glucose on tirzepatide becomes more likely |
| Oral contraceptives | May be absorbed less reliably for four weeks after starting tirzepatide and after each dose increase |
Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.
Can You Take Dihexa With Tirzepatide or Semaglutide?
Semaglutide works the same way
Semaglutide acts at GLP-1 only, where tirzepatide acts at GLP-1 and GIP, but it carries the same boxed thyroid warning and the same family of early side effects. Everything above applies to it unchanged. Semaglutide and tirzepatide are never taken together. The comparison is in semaglutide vs tirzepatide.

How to add Dihexa to a GLP-1 properly
- Name the goal. A specific, checkable goal, such as recall at work, is easier to judge than general fog.
- Put your full history on the intake, especially any cancer history, personal or family.
- Start when your GLP-1 dose is steady, not in the week of an increase.
- Report what you notice, including nothing. With no human data on Dihexa, your own reports are the main record.
Your physician sets your directions after reviewing your intake, and every vial arrives with its directions printed on the label. If your physician would rather start with a cognitive peptide that has a longer human history, Semax is often the first suggestion; see Dihexa vs Semax. More on combining is in can you take two peptides together and what peptides can you take with tirzepatide. Step-by-step: how to get Dihexa prescribed and how to get tirzepatide prescribed.
Can You Take Dihexa With Tirzepatide or Semaglutide?
What the evidence shows, honestly
Tirzepatide has a large human evidence base for weight management in its approved form. Dihexa has rodent behavioural studies, cell work, a retraction history for its proposed mechanism, and no human data at all. There is no study of the two together and no established interaction list for Dihexa. The pairing is plausible to discuss because the two have unrelated targets, and it is one where the honest description is "unstudied" at every level. Any source presenting Dihexa as an add-on that sharpens thinking on a GLP-1 is overstating what exists.

Common questions
Can You Take Dihexa With Tirzepatide or Semaglutide?, answered.
It can be prescribed alongside tirzepatide when a physician reviewing your intake judges it appropriate. The two act on unrelated systems, but the combination has never been studied and Dihexa has no human data.
The answer is the same. Different targets, no study of the pairing, and the same cancer-history questions apply.
No interaction is described, but no interaction list exists for Dihexa because no trials have been run. Your physician reviews every medication you take before prescribing.
Nothing in its research describes an effect on either. It is not a weight treatment.
That has never been studied. Feeling tired or slow on a GLP-1 often comes from eating and drinking much less than before; tell your care team what you are eating and how you are sleeping first.
GLP-1 medications carry a boxed warning about thyroid C-cell tumors in rodents, and a personal or family history of medullary thyroid carcinoma or MEN 2 rules them out. Dihexa's proposed mechanism runs through a growth pathway active in many cancers, and many physicians will not prescribe it with a cancer history.
Yes, as two separate injections from two separate vials, each with its own syringe and at a different site. Follow the directions printed on each vial.
