Semax for Women: What the Research Has Looked At, and What to Check First
This page covers what Semax is, what research has and has not looked at, the considerations that change for women, how it sits alongside perimenopause, contraception, hormone therapy and stimulant medication, and how Pepti supplies it. For the evidence benefit by benefit, see Semax benefits; for the product reference, what is Semax.

Treatments mentioned
No study has looked at Semax in women specifically: its human evidence is Russian clinical work in stroke, chronic cerebrovascular disease and optic nerve disease using a nasal product, plus a 14-person brain-imaging study, and none of it reports separate results for women. Semax is built from a fragment of ACTH with the hormone activity removed, so it is not expected to raise cortisol, is not a hormone or steroid, and is not described as acting on estrogen, progesterone or the menstrual cycle. For women, the practical points are that brain fog has common findable causes, thyroid disease, iron deficiency, broken sleep and the perimenopausal transition among them, and that Semax is not used in pregnancy or while breastfeeding. Compounded Semax is a prescription injection and is not FDA approved.

Semax for Women
The short answer
| Question | What the live research shows | Strength of evidence |
|---|---|---|
| Has Semax been studied in women? | Not as a separate group; the live research reports no sex-specific results | No female-specific data |
| Has it been studied for focus in healthy women? | No; there is no published randomised trial of Semax for attention, memory or productivity in healthy people of either sex | None |
| What human data exists? | Russian clinical studies in stroke, cerebrovascular disease and optic nerve disease, mostly open-label or comparative, using the intranasal product | Real clinical history, no Western-standard trials |
| Does it raise cortisol? | Described as devoid of corticotropic and melanotropic activity | Design-based, from the literature |
| Does it act on estrogen or the cycle? | Not a hormone or steroid; not described as acting on the reproductive axis | No study either way |
| Pregnancy or breastfeeding | No human data; not used | Clear exclusion |
The honest summary: nothing in women as a group, and a clinical history in brain injury rather than in healthy adults.
Semax for Women
What Semax is, in one paragraph
Semax is a heptapeptide, seven amino acids: Met-Glu-His-Phe-Pro-Gly-Pro. The first four are positions 4 to 7 of adrenocorticotropic hormone (ACTH), the pituitary hormone that tells the adrenal glands to make cortisol; the last three were added to slow its breakdown. The design keeps the part of ACTH studied for decades for attention and learning in animals and leaves out the part that raises cortisol and stimulates pigment cells. It was developed at the Institute of Molecular Genetics of the Russian Academy of Sciences, is registered in Russia as nasal drops, and has been used in Russian neurology since the 1990s. It is not a stimulant, not a steroid and not a substitute for prescribed ADHD treatment.

What research has looked at
The human clinical work
| Study | Who | What was reported |
|---|---|---|
| 1997, Korsakov Journal of Neurology and Psychiatry | 30 patients in acute ischaemic stroke given intranasal Semax with standard care, compared with 80 matched patients | Faster regression of deficits on rating scales, EEG and evoked potentials |
| 2005, same journal | 187 patients with chronic cerebrovascular insufficiency | Clinical improvement and good tolerability, including in older patients; comparative, not randomised |
| 2018 clinical trial, same group | 110 patients after ischaemic stroke | Plasma BDNF rose; faster improvement and better Barthel Index scores |
| 2018, Bulletin of Experimental Biology and Medicine | 14 healthy adults, one intranasal dose | Changes in the brain's default mode network on functional MRI; a mechanism study, not a performance trial |
What they show. That Semax has a real clinical history in Russian neurology, mostly in people with stroke or vascular disease.
What they do not show. Results for women separately, because the live research does not report them. Anything about healthy women with brain fog, because the clinical studies were in people with brain injury. There are no randomised, placebo-controlled trials of Semax in English-language peer-reviewed journals, and the FDA found no published pharmacokinetic work on subcutaneous Semax, the route Pepti prescribes. The FDA's July 2026 review excluded the Russian studies because complete English translations were not supplied.
The mechanism
Most of the mechanistic detail is from rats. A 2006 paper in Brain Research reported that Semax increased BDNF protein and TrkB receptor phosphorylation in the hippocampus; a 2010 paper described increased transcription of BDNF, NGF and their receptors in ischaemic cortex. A 2024 paper in the European Journal of Pharmacology reported that Semax counteracted stress-induced anhedonia, adrenal enlargement and a fall in hippocampal BDNF in stressed rats; those rats were male, so the one stress study in the live research says nothing about female animals, let alone women. The detail is in what is Semax.

What matters for women
Brain fog has findable causes worth ruling out first
Semax is prescribed around focus and mental energy, and in women those complaints have common, checkable causes. An underactive thyroid produces fatigue, low mood, poor sleep and slowed thinking, and autoimmune thyroid disease is heavily skewed toward women. Iron deficiency is common because of menstrual blood loss, and ferritin can be low while a full blood count reads normal. Low B12, short or broken sleep, sleep apnoea, low mood, regular drinking and medications all belong on the list. None of them responds to Semax. Your physician works through this from your intake. More is in peptides for brain fog and focus.
Perimenopause
Fog arriving with cycle changes and 3am waking is listed in Pepti's brain-fog guide as something "reviewed as a hormonal transition rather than treated around." The transition typically begins in the forties, ovarian hormone output fluctuates rather than declining smoothly, and fog during it is often driven by sleep disruption, thyroid disease or iron deficiency, which should be excluded first. Semax is not a treatment for the transition, and peptides do not replace menopausal hormone therapy. If the transition is the driver, that belongs in the conversation with your physician. See peptides for menopause.
Cortisol and the stress axis
Because Semax is a fragment of ACTH, people sometimes ask whether it raises cortisol. The design point is that it does not keep the cortisol-raising part of ACTH: it is described in the literature as devoid of corticotropic activity. Cortisol has not been measured in women on treatment.
Hormones, the cycle, birth control and hormone therapy
Semax is not a hormone and is not described as acting on estrogen, progesterone or the reproductive axis. No study has looked at cycle effects, at hormonal contraception alongside Semax, or at menopausal hormone therapy alongside it. The accurate answer is "no interaction described, none studied." List your contraception and any hormone therapy on the intake. If your cycle changes on treatment, report it; it is more likely to have another cause, but it should be looked at.
Pregnancy, breastfeeding and trying to conceive
Semax has no human pregnancy or lactation data, and the research behind it was conducted entirely in non-pregnant populations. It is not used in pregnancy or while breastfeeding, and its effect on fertility is unknown. If you are trying to conceive, tell your physician before starting. If you become pregnant on treatment, stop and contact your physician the same day. Can you take peptides while pregnant and do peptides affect fertility cover the detail.
Stimulant medication
This is the point most specific to Semax. A mouse study reported that Semax potentiated amphetamine-induced dopamine release in the striatum, one of the reasons the FDA's reviewers were cautious. No equivalent human finding exists. If you take a stimulant, including prescription ADHD medication, your physician needs to know before starting, and Semax is not a replacement for that treatment.
Migraine, stroke and vascular history
Migraine is one of the uses nominated to the FDA for Semax, but Semax is not a treatment for migraine or stroke in the United States. Sudden weakness, facial droop, slurred speech, loss of vision or the worst headache of your life is a reason to call 911, not to reach for any peptide. A history of stroke, a TIA, vascular disease or migraine goes on the intake in full.
Sport
If you compete in a tested sport, check the current WADA Prohibited List and your governing body before starting, and tell your physician. A prescription is not an exemption. Is Semax banned in sports covers its status.
Semax for Women
What the evidence does not establish
- An effect in women specifically. No human result for women has been reported separately.
- An effect on memory, focus or attention in healthy adults of either sex. No trial has measured that.
- Any effect on menopause symptoms, the menstrual cycle or fertility. None was studied.
- Equivalence between the intranasal route of the Russian studies and subcutaneous injection.
- Safety in pregnancy or breastfeeding. There is no data.
- Long-term safety over years of continuous use.
- Efficacy for any condition as an FDA-approved treatment. Semax is not approved for anything in the United States.

Side effects and considerations
Nothing in the live research suggests the side-effect profile differs between women and men.
| What is reported or relevant | When | What to do |
|---|---|---|
| Injection-site redness, mild soreness, occasional bruising | The most commonly reported effect | Rotate sites; report anything that spreads or worsens after 48 hours |
| Restlessness or feeling over-alert | Transient, mainly when dosed late in the day | Morning dosing is the usual direction; tell your care team if it persists |
| Headache | Occasional, early in a course | Report it if it persists |
| Taking a stimulant | Before starting | Tell your physician; a mouse study reported an interaction with amphetamine |
| Facial or throat swelling, widespread rash, difficulty breathing | Rare, possible with any injectable | Call 911, then tell your physician |
The FDA's stated safety concern in its 2026 review was a quality one rather than a known toxicity: an injectable peptide could provoke an immune response if it aggregates or carries impurities. That is a reason to care where a compounded peptide comes from. More is in Semax side effects.

Where Semax stands with the FDA
Semax was placed in Category 2 of the FDA's 503A bulk substances list in 2023, and as of April 2026 it is no longer on the Category 2 list. On 24 July 2026 the Pharmacy Compounding Advisory Committee considered it for the 503A Bulks List, for the nominated uses of cerebral ischaemia, migraine and trigeminal neuralgia. FDA staff recommended against listing; the committee voted 8 to 5, with one abstention, to recommend adding it. The FDA's final determination is pending. Semax remains legal to prescribe and dispense as a compounded medication, and compounded medications are not FDA approved.

How Pepti supplies Semax
| Product | What is in it | Typical directions | Price |
|---|---|---|---|
| Semax | 5 mL multi-dose vial, Semax 2 mg/mL (10 mg per vial) | 0.25 mL (25 units) subcutaneously once daily Monday through Friday; 0.5 mg per dose | $229/mo |
| Semax + Selank | 1 mg Semax and 1 mg Selank | Printed on the vial | $259/mo |
| SYNAPSE | 5 mL: 5 mg Semax, 5 mg Selank, 10 mg MOTS-c, 10 mg tesamorelin | 0.25 mL (25 units) once daily Monday through Friday | $279/mo |
| ELEVATE | 5 mL: 5 mg Semax, 5 mg Selank, 10 mg BPC-157, 10 mg KPV | 0.25 mL (25 units) once daily Monday through Friday | $249/mo |
The single Semax vial holds 20 doses at 0.25 mL, about four weeks of weekday dosing, so refills run every 28 days, one vial per fill. SYNAPSE contains tesamorelin, whose FDA-approved product is contraindicated in pregnancy. Where stress is the bigger complaint than focus, Selank is the sibling peptide from the same programme; the comparison is in Selank vs Semax, and dosing detail is in Semax dosage. Your physician sets your directions after reviewing your intake, and they are printed on the vial.
Every price is all-in, covering medication, physician review, refill management and shipping, with no membership fee. Pepti is cash-pay and does not bill insurance. Vials arrive as solutions ready to use, with sterile syringes and alcohol prep pads, shipped refrigerated with ice packs. Lot test results are at /quality/lab-results.

Where to get Semax prescribed
Semax cannot be bought legitimately without a prescription. Sites selling it "for research use only" are not accountable for identity, purity, sterility or concentration, which is the FDA's stated concern. The prescription route:
- Complete a free online intake covering your history, medications, including any stimulant, pregnancy and contraception status, and what you want help with.
- A physician licensed in your state reviews it, usually within 24 hours.
- If it is appropriate, a PCAB-accredited, FDA-registered US 503A compounding pharmacy prepares it to that prescription.
- It ships refrigerated with your directions printed on the vial.
- Your physician stays reachable for questions and side effects.
Step by step: how to get Semax prescribed online. Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.

Common questions
Semax for Women, answered.
Research has not looked at women as a group. The human evidence is Russian clinical work in stroke and vascular disease, using a nasal product. No trial has tested Semax for focus in healthy women or men, so any benefit for women is described, not established.
What research has looked at is stroke recovery, chronic cerebrovascular disease and optic nerve disease in people, and BDNF and stress measures in animals. No benefit specific to women has been studied. See Semax benefits.
It has not been tested in menopause. Fog during the transition is often driven by sleep disruption, thyroid disease or iron deficiency, which should be excluded first, and the transition itself is reviewed as a hormonal question rather than treated around.
Semax is not a hormone and is not described as acting on estrogen, progesterone or the reproductive axis. No study has looked at cycle effects. Report any change to your physician.
It is built from a fragment of ACTH but is described as devoid of the corticotropic activity that raises cortisol. Cortisol has not been measured in women on treatment.
No interaction has been described, but none has been studied. List both on the intake so your physician can review them together.
No. There is no human pregnancy or lactation data, and Semax is not used in either situation. If you become pregnant on treatment, stop and contact your physician the same day.
Tell your physician before starting. A mouse study reported Semax increasing amphetamine-induced dopamine release; no human finding exists, but it is the reason stimulant use matters at intake. Semax is not a substitute for ADHD treatment.
No. It is a peptide described around BDNF and neurotrophin signalling, not a stimulant. Some people describe feeling over-alert if they dose late in the day, which is why morning dosing is the usual direction.
The same as for men: mostly injection-site reactions, sometimes restlessness when dosed late in the day, and occasional early headache.
At Pepti, Semax is $229 a month, all-in, covering medication, physician review, refill management and shipping.
No. Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account. The free assessment goes to a physician licensed in your state. A consultation does not guarantee a prescription.
