Semax Side Effects: What Is Reported, What to Watch, and Who Should Not Take It

This page covers the Semax injection only. It is a practical reference to what is reported, when, what to watch, and when to message your care team. What Semax is, how it is described to work and its full research and FDA history are in what is Semax.

Semax (Injectable)
Semax$229/mo

The side effects reported with Semax injections are mostly at the injection site: redness, mild soreness and occasional bruising. Some people describe transient restlessness if it is taken late in the day, which is why morning dosing is the usual direction, and an occasional headache early on. The Russian clinical series describe Semax as well tolerated, including in older stroke patients. There is no established interaction list, and a mouse study reported Semax increasing amphetamine-induced dopamine release, so anyone taking a stimulant should tell their physician before starting.

Semax Side Effects

The short answer

Side effect How it is described What to do
Injection-site redness, mild soreness, occasional bruising The most commonly reported effect Rotate sites; report anything that spreads or worsens after 48 hours
Restlessness or feeling over-alert Transient, mainly when dosed late in the day Morning dosing is the usual direction; tell your care team if it persists
Headache Occasional, early in a course Report it if it persists
Interaction with stimulants A mouse finding, no human signal reported Tell your physician about any stimulant before starting
Serious allergic reaction Not a documented pattern, but possible with any injectable Call 911

The product this page covers:

Product What is in it Typical printed directions Price
Semax 5 mL multi-dose vial, Semax 2 mg/mL (10 mg per vial) 0.25 mL (25 units) subcutaneously once daily, Monday through Friday $229/mo

At 2 mg/mL, 0.25 mL is 0.5 mg of Semax per dose (arithmetic from the vial strength, on a U-100 insulin syringe), and a vial holds 20 doses, about four weeks of weekday dosing. Your own directions are set by your physician after reviewing your intake and are printed on the vial. The price is all-in: medication, physician review, refill management and shipping, with no membership fee. Semax is compounded by a PCAB-accredited, FDA-registered US 503A pharmacy. Compounded medications are not FDA approved.

Semax Side Effects

Why Semax has a short side-effect list

Semax is a heptapeptide, seven amino acids: Met-Glu-His-Phe-Pro-Gly-Pro. The first four are positions 4 to 7 of adrenocorticotropic hormone (ACTH), the pituitary hormone that tells the adrenal glands to make cortisol. The last three were added to slow its breakdown. The design keeps the part of ACTH studied for attention and learning in animals and leaves out the part that raises cortisol and stimulates pigment cells, so Semax is described in the literature as devoid of corticotropic and melanotropic activity.

That shapes what you should and should not expect:

What people sometimes expect What the sources describe
Cortisol effects, as with ACTH Not expected; the corticotropic part of ACTH is not in Semax
Skin darkening, as with ACTH Not expected; the melanotropic part is not in Semax
A stimulant crash or jitters, as with stimulant medication Semax is not a stimulant; some people feel over-alert if dosed late in the day
Steroid side effects Semax is not a steroid or an anabolic agent
Withdrawal on stopping No withdrawal or rebound is described in the literature

Semax Side Effects

Common Semax side effects

  • What the Russian clinical series report

Semax was developed at the Institute of Molecular Genetics of the Russian Academy of Sciences and is a registered medicine in Russia, used there in neurology for acute ischaemic stroke and chronic cerebrovascular disease since the 1990s. Its clinical series describe it as well tolerated, including in older stroke patients. A 2005 evaluation in the Korsakov Journal of Neurology and Psychiatry, in 187 patients with chronic cerebrovascular insufficiency, made tolerability an explicit endpoint and reported it as good.

One limit applies to all of that: the Russian product and the Russian studies used an intranasal form, not an injection. Published pharmacokinetic work on subcutaneous Semax has not been identified, and there is no published trial of Semax by injection. The tolerability record is real, but it was built on a different route.

  • What is reported with the injection

    Effect Detail
Redness at the site The most common report; tissue responding to the needle and fluid
Mild soreness Usually settles within a day or two
Occasional bruising More likely if the same spot is used repeatedly
Transient restlessness Mainly when dosed late in the day
Headache Occasional, early in a course

These are reported clinically rather than measured in a trial. Most people report nothing dramatic in the first week, and noticing nothing is normal.

  • Restlessness and timing

Some people describe feeling over-alert or restless if they take Semax late in the day. That is the reason the usual direction is a morning dose. If restlessness or trouble sleeping persists, tell your care team; any change to timing comes from your physician and appears on your vial. Never change your own dose or timing.

Semax Side Effects

Semax and stimulants

Animal work describes Semax increasing serotonergic signalling, and a separate mouse study reported that Semax potentiated amphetamine-induced dopamine release in the striatum. That finding was one of the reasons the FDA's reviewers were cautious in 2026. No equivalent human finding has been reported.

The practical point is simple: if you take a stimulant, including prescribed ADHD medication, your physician needs to know before Semax is prescribed. Semax is not a substitute for prescribed ADHD treatment, and there is no evidence it works for ADHD.

Beyond that, no combination has been studied. Semax has not been studied alongside SSRIs, SNRIs, stimulants or antipsychotics, and there is no established interaction list, because the trials that would produce one have not been run. That is why the intake asks for every medication you take.

What the FDA's 2026 review said about safety

Semax was placed in Category 2 of the FDA's 503A bulk drug substances list in 2023. The concern recorded was potential immunogenicity from aggregation and peptide-related impurities, with limited safety data for the injectable and intranasal routes. As of April 2026 Semax is no longer on the Category 2 list.

On 24 July 2026 the FDA's Pharmacy Compounding Advisory Committee considered Semax for the 503A Bulks List. The FDA's own briefing document recommended against listing, and the committee voted 8 to 5, with one abstention, to recommend adding it. A committee vote is a recommendation, and the FDA's final determination is pending. Semax remains legal to prescribe and dispense as a compounded medication, and it is not an FDA-approved drug.

Two points from that review matter for side effects:

  • The safety concern is a quality one, not a known toxicity. An injectable peptide could in principle provoke an immune response if it aggregates or carries impurities, and the FDA could not rule that out on the data submitted. That is a reason to care where a compounded peptide comes from; Pepti publishes lot test results at /quality/lab-results.
  • The dopamine finding is from mice. It is a reason to disclose stimulant use, not evidence of a human effect.

Serious reactions: what needs urgent care

Serious reactions are not a documented pattern with Semax, but any injectable can cause them.

Symptom What it may be What to do
Swelling of the face, lips or throat, widespread hives, trouble breathing Serious allergic reaction Call 911 first, then tell your care team
Chest pain, fainting, or sudden weakness or numbness on one side Needs emergency assessment, whatever the cause Call 911
You think you injected far more than your directions Possible overdose Call Poison Control at 1-800-222-1222 and message your care team
An injection site that is hot, spreading or worse after 48 hours Possible infection rather than irritation Message your care team the same day
A reaction that is severe or spreading Not an expected effect Stop and contact your care team

Semax is not a treatment for stroke in the United States. If you have symptoms of a stroke, call 911; nothing about Semax is a reason to delay emergency care.

Semax Side Effects

Injection-site reactions and how to reduce them

Semax is injected under the skin, into the fat layer, usually in the abdomen or thigh. There is no reason to inject near the head; the peptide is carried systemically. Rotating sites so the same spot is not used repeatedly reduces soreness and bruising. A small lump that appears as you inject and softens over a day or two is fluid in the tissue. One that appears days later and is warm or spreading is not and needs a same-day message. Technique is in how to inject peptides safely.

Semax ships refrigerated with ice packs, with sterile syringes and alcohol prep pads. Keep it in the fridge between doses; beyond-use dating runs from first puncture and is on the label.

Semax Side Effects

When to message your care team

  • restlessness or trouble sleeping that persists despite morning dosing
  • a headache that persists beyond the first doses
  • an injection site that is getting worse rather than better
  • any change in mood that worries you
  • you start, stop or change any medication, especially a stimulant or psychiatric medication

Semax Side Effects

Who should not take Semax, or should discuss it first

Situation Why
Taking a stimulant, including ADHD medication The mouse dopamine finding; disclose it before starting
Taking SSRIs, SNRIs or antipsychotics No combination has been studied
Under psychiatric care Your prescriber and psychiatrist should be aware of each other
Pregnancy or breastfeeding Not used; safety data is absent
Tested athletes Many peptides are prohibited in competition; check the current list
Under 18 Pepti prescribes to adults only

Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.

Semax Side Effects

Semax vs Selank side effects

Selank is a sibling peptide from the same Russian research programme, built from tuftsin rather than ACTH. Both are described as well tolerated in their Russian series, and both produce mostly injection-site reactions by injection. The difference in feel is usually described as opposite: Semax occasionally causes restlessness if dosed late, while Selank is sometimes described as causing early fatigue or flatness and less often a wired feeling. Taken together in the Semax + Selank vial, the same points apply to each component. The comparison is in Selank vs Semax, and the combined vial is covered in what is Semax + Selank.

Semax Side Effects

What the evidence shows, honestly

Semax has decades of clinical use in Russia, and its clinical series report good tolerability, including in older patients. What it does not have is an English-language, randomised, placebo-controlled trial, a published trial by injection, long-term safety data over years of continuous use, or an interaction study of any kind. The side-effect profile on this page is therefore a picture of what is reported, not a measured table of rates against placebo. Any source giving percentages for Semax injection side effects is citing something that does not exist in the published literature. The general picture for this category is in peptide side effects: what to watch for.

Where to get Semax prescribed

Semax cannot be bought legitimately without a prescription. Products sold "for research use only" have no accountable pharmacy behind their identity, purity, sterility or concentration, which is the exact quality question the FDA raised. The prescription route:

  1. Complete a free online intake covering your history, medications and what you want help with.
  2. A physician licensed in your state reviews it, usually within 24 hours.
  3. If it is appropriate, a PCAB-accredited, FDA-registered US 503A compounding pharmacy prepares it to that prescription. Lot test results are published at /quality/lab-results.
  4. It ships refrigerated with ice packs, with sterile syringes and alcohol prep pads, and your directions printed on the vial.
  5. Your care team stays reachable for side effects and questions. Refills run every 28 days, one vial per fill.

Pepti is cash-pay and does not bill insurance, and prescribes Semax in every state except Oklahoma, plus DC The step-by-step page is how to get Semax prescribed online.
