Is Tesamorelin Banned in Sports? WADA, NCAA, TUEs and Drug Tests

This page covers what the anti-doping rules say about tesamorelin, why it is on the list, what a therapeutic use exemption would involve, the products that contain it, detection, workplace testing, and where tesamorelin stands with the FDA. It is general information, not legal or anti-doping advice. For what tesamorelin is and what its trials measured, see what is tesamorelin.

Tesamorelin (Injectable)
Tesamorelin$249/mo

Yes. Tesamorelin is banned in tested sport: the 2026 World Anti-Doping Agency Prohibited List names it under S2.2.4, growth hormone releasing factors, as an example of a GHRH analogue, prohibited at all times, in and out of competition, and the 2027 List keeps it there. It is a non-Specified Substance, a prescription does not make it permitted, and every Pepti blend that contains tesamorelin is prohibited because of it. An FDA-approved tesamorelin product exists for one narrow indication, but that approval is not an exemption. Tesamorelin is not a controlled substance, and standard workplace drug panels do not look for it.

Why tesamorelin is on the list

Tesamorelin is a synthetic analogue of growth hormone-releasing hormone (GHRH). It acts on the pituitary to prompt the body's own growth hormone release, which in turn raises IGF-1. It is not growth hormone itself, and it is not a weight-loss drug. But anti-doping rules treat the whole growth hormone axis as one problem: growth hormone, the hormones that release it, and the growth factors downstream of it all appear in section S2 of the Prohibited List.

  • S2.2.4, by name

S2.2.4 covers growth hormone releasing factors and secretagogues in three groups. The first, GHRH and its analogues, gives CJC-1293, CJC-1295, sermorelin and tesamorelin as examples. The second covers growth hormone secretagogues and their mimetics, including ipamorelin and ibutamoren (MK-677). The third covers GH-releasing peptides, including hexarelin and GHRP-1 through GHRP-6. Tesamorelin is named in the List itself, not captured by a catch-all.

  • Prohibited at all times, and non-Specified

S2 substances are prohibited in and out of competition, so there is no off-season window: using tesamorelin in a summer training block is a violation exactly as using it before a final is. Everything in S2 is a non-Specified Substance, which matters when sanctions are decided after a violation is established. It is not a lesser category.

  • The 2027 List

The 2027 Prohibited List has been published, and it carries the classification forward unchanged: CJC-1295, sermorelin, tesamorelin, ipamorelin and hexarelin all remain in S2.2.4.

The approved product does not make it permitted

This is the question that comes up most with tesamorelin, because it is unusual among compounded peptides: an FDA-approved tesamorelin product exists. It was approved in November 2010 for one narrow indication, the reduction of excess abdominal fat in adults with HIV who have lipodystrophy, and that approval was deemed a biologics licence in 2020.

None of that affects its anti-doping status. The Prohibited List names substances, not unapproved products. An approved drug can be prohibited in sport, and many are; regulatory approval is relevant to the S0 catch-all for non-approved substances, but tesamorelin is not in S0. It is named in S2.2.4, which applies whether the vial came from a manufacturer or a compounding pharmacy.

The compounded tesamorelin Pepti supplies is a different thing again: prepared by a licensed pharmacy to an individual prescription, prescribed off-label, and not FDA approved. For an athlete, both are prohibited.

Therapeutic use exemptions

A therapeutic use exemption (TUE) is the only route by which a prohibited substance can be permitted, and it has to be granted in advance by an anti-doping body, not by a prescriber. Under the International Standard for Therapeutic Use Exemptions the athlete must show four things:

  1. A diagnosed medical condition supported by relevant clinical evidence.
  2. That use will not produce any additional enhancement of performance beyond a return to normal health.
  3. That it is an indicated treatment, with no reasonable permitted therapeutic alternative.
  4. That the need is not a consequence of earlier prohibited use without a TUE.

Read the third condition against tesamorelin. Its approved indication is one specific syndrome, and the off-label reasons people ask about it (body composition, recovery, sleep, ageing) are not indicated treatments for a diagnosed condition. Growth hormone releasing factors are prohibited precisely because their effects overlap with performance, and TUEs for this class are not a routine grant. Anyone with a genuine diagnosis should take it to their national anti-doping organisation or international federation before starting anything.

In the United States, USADA requires athletes in its Registered Testing Pool, Clean Athlete Program and Education Pool to hold a TUE in advance of using a prohibited substance, and directs other competitors, including recreational ones, to submit a TUE Pre-Check Form.

The products that contain tesamorelin

A blend is prohibited by virtue of its components. These Pepti products contain tesamorelin:

Product What is in it (per 5 mL vial) Price Classes for a tested athlete
Tesamorelin Tesamorelin 2 mg/mL (10 mg) $249/mo S2.2.4
Tesamorelin + Ipamorelin 10 mg tesamorelin, 5 mg ipamorelin $259/mo S2.2.4 (both)
RADIANCE 50 mg GHK-Cu, 10 mg tesamorelin $249/mo S2.2.4
TITAN 10 mg tesamorelin, 6 mg CJC-1295, 12 mg ipamorelin, 10 mg BPC-157 $289/mo S2.2.4 and S0
PHYSIQ 10 mg tesamorelin, 10 mg ipamorelin, 10 mg AOD-9604, 10 mg MOTS-c $289/mo S2.2.4, S2.2.3 and S4.4.1
ASCEND 6 mg CJC-1295, 12 mg ipamorelin, 10 mg tesamorelin, 1 mg IGF-1 LR3 $289/mo S2.2.4 and S2.3
FUSION 10 mg BPC-157, 10 mg MOTS-c, 10 mg 5-amino-1MQ, 15 mg tesamorelin $279/mo S2.2.4, S0 and S4.4.1
SYNAPSE 5 mg semax, 5 mg selank, 10 mg MOTS-c, 10 mg tesamorelin $279/mo S2.2.4 and S4.4.1

Prices are all-in monthly figures covering medication, physician review, refill management and shipping, with no membership fee. All are compounded by PCAB-accredited, FDA-registered US 503A pharmacies, and none is FDA approved.

Two points from that table. First, the quantity does not matter: anti-doping rules prohibit the presence of a substance, not a dose above a threshold. Second, blends often carry more than one problem. A blend marketed for skin, focus or metabolism can still contain a growth hormone releasing factor.

Can tesamorelin be detected?

Growth hormone releasing factors and secretagogues are prohibited at all times, and detection methods for this class are established and improving. The anti-doping journals have published method after method for peptides on the List: Drug Testing and Analysis published the determination of growth hormone releasing peptide metabolites in human urine in 2015, and a 2026 paper in The Analyst described a harmonised workflow for determining peptidic and non-peptidic doping agents in dried and liquid blood.

Detection windows for small peptides are generally short, often hours to a few days. That is not a loophole:

  • A sample reported negative may be stored and reanalysed later under Code Article 6.6.
  • Article 17 allows a charge within ten years of the asserted violation.
  • Strict liability, under Article 2.1.1 of the Code, means intent, fault and negligence do not need to be shown to establish a violation.

There is also a clinical point. Treatment with growth-hormone-axis peptides can raise IGF-1 on a blood test, because that is the expected consequence of how they work. That is not a drug test result, but it is one more reason your medical record should be accurate.

What about workplace drug tests?

Standard pre-employment and workplace panels look for drugs of abuse, using immunoassays raised against specific small molecules and confirmatory mass spectrometry. No screening assay exists for tesamorelin, and a peptide of its size has no structural similarity to the targets, so it will not trigger a false positive. Expanded panels in safety-sensitive jobs add more drugs of abuse, not therapeutic peptides.

If an employer asks you to declare prescriptions, use the confidential medical review route and declare accurately, including compounded medications. Before surgery or a hospital procedure, tell the treating clinician everything you take. The full comparison is in do peptides show up on a drug test.

Where tesamorelin stands with the FDA

Point Status
Approved product Yes, for reducing excess abdominal fat in adults with HIV-associated lipodystrophy (approved 2010, deemed a biologics licence 2020)
Compounded tesamorelin Prepared to an individual prescription; not FDA approved
503A bulks category list Never on it, because it is the active ingredient of an approved product
2026 Category 2 removals and July 2026 advisory committee Not involved
Controlled substance No

Because tesamorelin is the active ingredient of an approved, biologics-licensed product, its compounding status does not change when the bulks lists change. None of that bears on its anti-doping status.

If you are a tested athlete

If you are What to do
In any testing pool, at any level Do not use tesamorelin or any blend containing it
Already holding a prescription Talk to your anti-doping organisation or team physician, and tell the prescriber you are tested
A woman in tested sport The same rule applies; S2.2.4 is not sex-specific
Diagnosed with a condition you think needs it Apply for a TUE in advance, not after a test
Unsure which rules cover you Ask your governing body which list applies

Checking a specific substance is done through Global DRO, which gives a status for your sport and country and a reference number for the search. Keep it. Major US professional leagues mostly run their own programs under collective bargaining; if you compete in one, read that policy.

If you are not tested

The anti-doping rules do not reach a recreational athlete outside any testing program, and tesamorelin has a better-characterised evidence base than most compounded peptides: randomised, placebo-controlled trials, including two phase 3 trials, in people with HIV-associated lipodystrophy. Those trials measured visceral fat on CT, not performance. What they do and do not establish is in tesamorelin benefits, the printed directions are in tesamorelin dosage, and what to watch for is in tesamorelin side effects. How it compares with the other GHRH options is in sermorelin vs tesamorelin vs ipamorelin and CJC-1295 vs tesamorelin.

Where to get tesamorelin prescribed

Tesamorelin cannot be bought legitimately without a prescription. Sites selling it "for research use only" are not accountable for identity, purity, sterility or concentration. The prescription route:

  1. Complete a free online intake covering your history, medications, what you want help with, and whether you are subject to drug testing.
  2. A physician licensed in your state reviews it, usually within 24 hours. If you are tested, say so again in your first message.
  3. If it is appropriate, a PCAB-accredited, FDA-registered US 503A compounding pharmacy prepares it to that prescription. Lot test results are at /quality/lab-results.
  4. It ships refrigerated with ice packs, ready to use, with sterile syringes and alcohol prep pads, and your directions printed on the vial.
  5. Refills run every 28 days, one vial per fill.
