Step 2: the medical intake
The intake is the step that makes this a prescription rather than a purchase. For SYNAPSE, expect questions about:
- What you are hoping to address, and what you have already tried.
- Any existing psychiatric treatment. Selank is described as modifying GABA signalling, so anything already acting on mood or anxiety is reviewed before adding it.
- Diabetes or impaired glucose tolerance, because tesamorelin can worsen glucose tolerance.
- Any personal history of cancer. The approved tesamorelin product is contraindicated in active malignancy, and that applies to compounded tesamorelin too.
- Pregnancy or breastfeeding. The approved tesamorelin product is contraindicated in pregnancy.
- Pituitary conditions, corticosteroid replacement, and any medication cleared by cytochrome P450 enzymes, all raised on the approved tesamorelin label.
- Whether you are a tested athlete. MOTS-c is named on the 2026 WADA Prohibited List under S4.4.1 and prohibited at all times.

Step 6: shipping and what is in the box
The vial ships refrigerated in insulated packaging with ice packs. What gets reported clinically is mostly at the injection site: redness, mild soreness, occasional bruising. Some people report nausea or light-headedness early on, and tesamorelin can cause fluid retention, with swelling, joint aches or occasionally carpal tunnel symptoms. Message your care team about any of these, and for any reaction that is severe, spreading or involves difficulty breathing, call 911.

Where SYNAPSE stands legally
SYNAPSE is a compounded prescription medication. It has no FDA file of its own, because the FDA regulates the bulk substances a pharmacy compounds from. Semax, Selank and MOTS-c were placed in Category 2 of the FDA's 503A list in 2023 and none sits in any active category now. In July 2026 the Pharmacy Compounding Advisory Committee voted to recommend adding Semax (8 to 5) and MOTS-c (7 to 5) to the 503A Bulks List, with the FDA's final determinations pending; Selank has never been reviewed. Tesamorelin is the active ingredient of an FDA-approved product, approved in 2010 for one indication, and the compounded tesamorelin in SYNAPSE is prescribed off-label.
