PE-22-28 for Women: What the Research Has Looked At, and What to Raise First

This page covers what PE-22-28 is, what research has and has not looked at, the considerations that change for women, how it sits alongside perimenopause and menopause, and how Pepti supplies it. For the evidence benefit by benefit, see PE-22-28 benefits; the companion page is PE-22-28 for men.

PE-22-28 (Injectable)
PE-22-28$219/mo

PE-22-28 has not been studied in women, or in men, because it has no human trials of any kind: its evidence is cell and rodent work on antidepressant-like behaviour, and the live research does not report results by sex. It is not used in pregnancy or while breastfeeding, where safety data is absent. It is not a hormone and does not act on estrogen or progesterone. For women, the points to raise first are pregnancy plans, any antidepressant or triptan (because PE-22-28's proposed mechanism overlaps with serotonin signalling), and the common, findable causes of low mood, iron deficiency and an underactive thyroid among them. Compounded PE-22-28 is a prescription injection, has never been evaluated by the FDA, and is not FDA approved.

If you are in crisis or thinking about harming yourself, call or text 988 in the US, or call 911, now.

PE-22-28 for Women

The short answer

Question What the live research shows Strength of evidence
Has it been studied in women specifically? No; it has no human trials at all None
What has been studied? Antidepressant-like behaviour, neurogenesis and synapse formation in mice and rats Animal and cell only
Does it act on hormones or the cycle? Not a hormone; not described as acting on the reproductive axis No study
Does it help menopause symptoms or mood in menopause? Not tested None
Pregnancy or breastfeeding Not used; safety data is absent Clear exclusion
On an antidepressant or a triptan? Never studied together; the mechanism overlaps with serotonin A caution, not a known interaction
Tested sport Not named on the 2026 WADA list; the S0 section can apply Confirm with your anti-doping organisation

The honest summary: nothing in the PE-22-28 literature is specific to women, and the main women-specific questions are about pregnancy, medications and what else could be driving low mood.

PE-22-28 for Women

What PE-22-28 is, in one paragraph

PE-22-28 is a seven-amino-acid fragment of the sortilin propeptide, a piece clipped off when the body makes the sortilin receptor. Researchers in Nice, France named a longer section of it spadin (residues 12 to 28) and showed it blocks TREK-1, a background potassium channel that helps set how easily a neuron fires. PE-22-28 is residues 22 to 28 only, described in Frontiers in Pharmacology in 2017 as more potent at TREK-1 in cells and longer-lasting in mice. In rodents, blocking TREK-1 on serotonin neurons was associated with increased firing, the same direction of effect SSRIs are thought to produce by a different route. It is not an antidepressant in the regulatory sense, not a stimulant and not a sedative.

What research has looked at

The rodent work

The 2010 PLoS Biology paper reported antidepressant-like effects of spadin in mice across four standard behavioural tests after four days, with increased hippocampal neurogenesis. The 2017 paper reported PE-22-28 reproducing those effects after a four-day course. A 2015 British Journal of Pharmacology paper described spadin regulating synapse formation, and a 2019 Neuropharmacology paper reported sortilin-derived peptides associated with improved recovery in a mouse stroke model.

What it shows. A well-characterised mechanism in rodents.

What it does not show. Anything in a woman. None of the live research describes results broken down by sex, and no human has been given PE-22-28 in a published study.

The human biomarker papers

A 2017 Journal of Affective Disorders paper reported the body's own sortilin propeptide being lower in the blood of patients with major depressive disorder, and a 2018 paper reported it rising after electroconvulsive therapy. These measure a natural molecule as a possible marker; they are not studies of giving PE-22-28 to anyone, and are not described as reporting results for women separately.

What changes for women

  • Low mood has findable causes that are more common in women

Fatigue, hair shedding, cold intolerance, brain fog and low mood are the classic presentation of iron deficiency and of an underactive thyroid, and both are far more common in women than in men: iron deficiency because of menstrual blood loss, thyroid disease because autoimmune thyroiditis is heavily skewed toward women. Ferritin can be low while a full blood count reads normal. Neither responds to a peptide, and both are inexpensive to find.

What is checked Why it matters
Iron and B12 Deficiency presents as low mood and mental fatigue
Thyroid function An underactive thyroid produces low mood, fatigue and slowed thinking; an overactive one looks like anxiety
Sleep Broken sleep produces low mood directly
Alcohol, cannabis, stimulants, caffeine Common, reversible causes
Existing psychiatric care and medications Interaction review, and that team should be involved
Suicidal thoughts or self-harm Needs urgent care, not a prescription from a website

If something on that list is driving how you feel, a peptide is the wrong tool and a physician should say so. More is in peptides for anxiety and mood and peptides for women.

  • Pregnancy and breastfeeding

PE-22-28 is not used in pregnancy or while breastfeeding, where safety data is absent. If you become pregnant on treatment, stop and contact your physician the same day. Low mood during or after pregnancy is a reason to talk to your obstetric or primary care team, which has treatments with evidence for that situation. Can you take peptides while pregnant covers the detail.

  • Trying to conceive

Nothing in the live research looks at PE-22-28 and fertility in either direction. Tell your physician before starting if you are trying to conceive; the usual approach is not to start an elective injectable while actively trying. See do peptides affect fertility.

  • Perimenopause and menopause

The perimenopausal transition typically begins in the forties, and low mood, irritability and poor sleep in those years are often the transition itself, or the night sweats and fragmented sleep it brings. PE-22-28 is not hormone therapy, does not act on the hormones that change, and has not been tested for mood in menopause or for any menopause symptom. If the transition is the driver, it belongs in the conversation with your physician rather than being treated around. Pepti does not prescribe menopausal hormone therapy. See peptides for menopause.

  • Contraception and the cycle

PE-22-28 is not described as acting on the reproductive axis or on hormonal contraception, and no study has looked at either directly. Any cycle change on treatment is worth reporting, even if another cause is likelier. List your contraception or hormone therapy on the intake.

  • Antidepressants and migraine medication

Because the proposed mechanism runs through serotonin signalling, anyone on an antidepressant, a triptan for migraine, lithium, tramadol or any other serotonergic drug must say so on the intake. The combination has never been studied in people. If you are being treated for depression or anxiety, the clinician managing it should be part of the decision, and no existing medication should be stopped on the strength of a rodent literature.

  • Diabetes and seizures

Cell work described PE-22-28's parent peptide increasing insulin secretion from pancreatic beta cells (Journal of Diabetes Research, 2016), so women with diabetes, including a history of diabetes in pregnancy, or on glucose-lowering medication, should raise it. TREK-1 is involved in seizure protection; the rodent work found no problem, but a seizure history is discussed rather than assumed away.

  • Tested sport

The 2026 WADA list does not name PE-22-28, but its S0 non-approved substances section can apply to a compounded peptide with no human therapeutic approval anywhere. Women in a tested sport should check with their anti-doping organisation first. See peptides for athletes.

PE-22-28 for Women

What the evidence does not establish

  • That PE-22-28 does anything in women, or in anyone. There are no human trials.
  • Any effect on mood in perimenopause or menopause, the cycle or hormone levels. None was tested.
  • Safety in pregnancy or breastfeeding.
  • A human dose, an effect size or a timeline.
  • Anything about combining it with antidepressants, triptans or hormonal contraception.

Side effects and considerations

Nothing in the live research suggests the profile differs between women and men, and the human profile is unknown.

What is reported or relevant When What to do
Injection-site redness, mild soreness, occasional bruising The commonest clinical report Rotate sites between abdomen and thigh
Headache in the first days Occasional Report it if it persists
A change in mood that worries you or those around you Not a described effect, always reportable Contact your physician promptly
Pregnancy, trying to conceive, or breastfeeding Before and during treatment Not used in pregnancy or breastfeeding; tell your physician
Thoughts of harming yourself An emergency Call or text 988, or call 911, now
Facial or throat swelling, difficulty breathing Rare Call 911 first, then tell your physician

If you think you may have taken too much, call Poison Control at 1-800-222-1222 and message your care team. Do not skip, double or change a dose to compensate. The full detail is in PE-22-28 side effects.

Where PE-22-28 stands with the FDA

PE-22-28 does not appear in Category 1, 2 or 3 of the FDA's 503A nominated-substances document updated 14 May 2026, is not on the significant-safety-risk page, and was not on the July 2026 Pharmacy Compounding Advisory Committee agenda. No safety notice, no vote either way: it has not been through the process. Compounded medications are not FDA approved.

How Pepti supplies PE-22-28

Product What is in it Typical directions Price
PE-22-28 5 mL multi-dose vial, 2 mg/mL (10 mg per vial) 0.25 mL (25 units) subcutaneously once daily Monday through Friday; 0.5 mg per dose, 20 doses $219/mo
Selank 5 mL multi-dose vial, 2 mg/mL (10 mg per vial) Its own directions $229/mo
DSIP 5 mL multi-dose vial, 1 mg/mL (5 mg per vial) Its own directions $249/mo

Selank has small Russian human studies in anxiety, which PE-22-28 does not, and DSIP is the more direct conversation when sleep is the real problem. Twenty weekday doses of PE-22-28 is four weeks, so refills run every 28 days, one vial per fill. The arithmetic is in PE-22-28 dosage. Your physician sets your directions after reviewing your intake, and they are printed on the vial.

Every price is all-in, covering medication, physician review, refill management and shipping, with no membership fee. Pepti is cash-pay and does not bill insurance. Vials arrive as ready-to-use multi-dose solutions with sterile syringes and alcohol prep pads, shipped refrigerated with ice packs. Lot test results are at /quality/lab-results.

Where to get PE-22-28 prescribed

PE-22-28 cannot be bought legitimately without a prescription. Research-use-only vials have no pharmacy accountable for identity, purity, sterility or concentration, and nobody reviewing your medications. The prescription route:

  1. Complete a free online intake covering your history, including mental health history, pregnancy, breastfeeding and contraception status, every medication you take, and what you want help with.
  2. A physician licensed in your state reviews it, usually within 24 hours.
  3. If it is appropriate, a PCAB-accredited, FDA-registered US 503A compounding pharmacy prepares it to that prescription.
  4. It ships refrigerated with your directions printed on the vial.
  5. Your care team stays reachable for questions, side effects and mood changes.

Step by step: how to get PE-22-28 prescribed online. The legal questions are in do you need a prescription for PE-22-28. Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.
