The MOTS-c research most often quoted for women comes from ovariectomised mice, a standard animal model of post-menopausal metabolic change: a 2019 study reported less fat accumulation and more brown-fat activity in treated animals, and a 2016 study reported less bone loss. Those are mouse findings. There are no randomized controlled trials of MOTS-c in humans of either sex, and the one human genetic finding linking MOTS-c to type 2 diabetes was seen in men. MOTS-c is not hormone therapy, is not a treatment for osteoporosis, and is not used in pregnancy or breastfeeding. It is a compounded prescription injection and is not FDA approved.

MOTS-c for Women

The short answer

Question What the live research shows Strength of evidence
Fat gain after loss of ovarian hormones Less fat accumulation and less inflammation in white fat, ovariectomised mice (Journal of Molecular Medicine, 2019) Animal only
Brown-fat activity More brown-fat activity, same 2019 mouse study Animal only
Bone loss after loss of ovarian hormones Less bone loss on micro-CT, ovariectomised mice (Biochemical and Biophysical Research Communications, 2016) Animal and cell only
Insulin sensitivity Mouse studies (2015); a human genetic association seen in men (2021); lower levels in type 2 diabetes in a mixed group (2019) Preclinical, with human association; no treatment trial
Has it been tested in women? No human treatment trial in either sex None
Menopause symptoms Not studied None
Pregnancy or breastfeeding Not used; safety data is absent Clear exclusion

The honest summary: a coherent mouse story that happens to use a menopause model, and nothing yet in women.

What MOTS-c is, in one paragraph

MOTS-c is a 16-amino-acid peptide whose gene sits in mitochondrial DNA rather than in the cell nucleus. It was identified in a 2015 Cell Metabolism paper from a group at the University of Southern California. The described mechanism is metabolic: in cultured cells it slows the folate cycle, a molecule called AICAR builds up, and AICAR activates AMPK, the enzyme cells use as a low-fuel sensor, the same one exercise switches on. Skeletal muscle appeared to be the main target. It is not a hormone, not a growth hormone secretagogue, not a GLP-1 medication, does not work through appetite, and does not act on estrogen or the reproductive axis.

What research has looked at in relation to women

  • Why ovariectomised mice

Ovariectomy, surgical removal of the ovaries, is a standard animal model of post-menopausal metabolic change. That is why these studies are quoted for women. It is also their limit: a mouse without ovaries is a model, not a woman in perimenopause or after menopause.

  • Fat and brown fat: the 2019 study

A 2019 paper in the Journal of Molecular Medicine treated ovariectomised mice with MOTS-c and reported less fat accumulation and less inflammatory infiltration in white fat, and more brown-fat activity, through AMPK. Brown fat burns fuel to make heat rather than storing it, which is why the finding is quoted.

What it shows. Prevention of fat gain, in mice, after their ovaries were removed.

What it does not show. Loss of fat already there, or anything in people. There is no human trial showing weight or fat change from MOTS-c. If weight is the actual concern around menopause, semaglutide and tirzepatide have large randomized human trials with weight as the primary endpoint, and the STEP trials enrolled a majority of women; see peptides for menopause.

  • Bone: the 2016 study

A 2016 paper in Biochemical and Biophysical Research Communications, also in ovariectomised mice, reported less bone loss on micro-CT in treated animals, and that MOTS-c inhibited RANKL-driven formation of osteoclasts, the cells that break bone down, in an AMPK-dependent way.

What it means. This is why osteoporosis sat alongside obesity as a nominated use when the FDA's advisory committee reviewed MOTS-c in July 2026. Nomination is not evidence. There is no human bone study of MOTS-c, and it is not a treatment for osteoporosis. The fastest bone loss happens in the years around the final period, and protecting bone is a DEXA scan, calcium, vitamin D, resistance training and a medical conversation, not a peptide.

  • Insulin sensitivity: the human data, and who it was in

The founding 2015 work reported that MOTS-c prevented age-related and diet-induced insulin resistance in mice. Two kinds of human data sit alongside it, and neither involved giving anyone MOTS-c:

    Study Who What was reported
Aging, 2021 Three cohorts totalling 27,527 people An East Asian mitochondrial DNA variant that changes one amino acid in MOTS-c was associated with higher type 2 diabetes prevalence in men, particularly sedentary men
Frontiers in Endocrinology, 2019 225 people in Qatar Lower serum MOTS-c in people with type 2 diabetes than in controls
Journal of Investigative Medicine, 2018 Ten lean and ten obese adults Similar levels with and without obesity
Archives of Endocrinology and Metabolism, 2025 85 adults Similar levels with and without obesity

The genetic association was a finding in men. The observational studies do not line up neatly, and raising MOTS-c by injection has not been shown to do what naturally higher levels correlate with.

  • Exercise and late life

A 2021 paper in Nature Communications reported better physical performance in treated mice of every age tested, a longer healthy lifespan with intermittent treatment started late in life, and, in humans, that a bout of exercise raised the body's own MOTS-c. The human part describes what the body does after exercise, not what an injection does. None of it was reported for women separately.

  • The analogue trial

A modified analogue, CB4211, went through a small company-reported Phase 1 program in adults with obesity and fatty liver disease. It is a different molecule, the results were company announcements rather than a peer-reviewed publication, and the program did not advance beyond Phase 1.

What changes for women

  • Menopause is a separate conversation

MOTS-c has not been tested for hot flashes, night sweats, sleep, mood or any other menopause symptom, and it does not replace the hormones that change. If the transition is the driver, it belongs in its own conversation with your physician; Pepti does not prescribe menopausal hormone therapy. Peptides for women covers what else changes.

  • Pregnancy and breastfeeding

MOTS-c is not used in pregnancy or breastfeeding. There is no human safety data, no approved product to carry a label, and no human lactation data of any kind. If you become pregnant on treatment, stop and contact your physician the same day. See can you take peptides while pregnant and do peptides affect fertility.

  • Fatigue has findable causes

Some people describe steadier energy on MOTS-c; others notice nothing, and no study has measured energy or fatigue. In women, iron deficiency and thyroid disease are common, findable causes of fatigue and are worth checking first. See peptides for energy and fatigue.

  • Blood sugar

Because the described mechanism is insulin sensitivity, anything you take that lowers blood glucose belongs on your intake.

  • Contraception and the cycle

MOTS-c is not described as acting on the reproductive axis, and no study has looked at the cycle or hormonal contraception. List your contraception on your intake and report any change.

  • Sport

MOTS-c is on the World Anti-Doping Agency Prohibited List at all times as an AMPK activator, USADA has stated it is not eligible for a therapeutic use exemption, and a validated plasma test was published in 2019. If you compete in a tested sport, it is not a peptide for you.

What the evidence does not establish

  • That MOTS-c changes fat, weight, bone or glucose in women. Those trials have not been run in anyone.
  • That ovariectomised-mouse findings transfer to women after menopause.
  • Any effect on menopause symptoms.
  • An effect size or a timeline.
  • Long-term safety over years of use.
  • Efficacy for any condition as an approved treatment. MOTS-c is not an approved drug anywhere.

Side effects

What is reported When What to do
Injection-site redness, mild soreness, occasional bruising Any time Rotate sites
Transient jitteriness, a faster pulse or mild light-headedness The hour or two after a morning dose, mostly early in a course Usually settles; tell your physician if it does not
A reaction that is severe, spreading, or involves difficulty breathing Rare Call 911, then tell your physician

The published animal work reports MOTS-c as well tolerated. There is no human safety data from trials and no established interaction list, which is why the intake asks for every medication you take.

Where MOTS-c stands with the FDA

MOTS-c was removed from Category 2 of the FDA's interim 503A list on 15 April 2026. When the FDA's own reviewers assessed it for the advisory committee in July 2026, they recommended against adding it to the 503A Bulks List, citing the absence of human clinical data. On 23 July 2026 the committee voted 7 to 5, with two abstentions, to recommend adding it anyway, for the nominated uses of obesity and osteoporosis. A committee vote is a recommendation, not a decision; the FDA's final determination is pending, and MOTS-c remains legal to prescribe and dispense. Compounded MOTS-c is not FDA approved.

SHRED 10 mg AOD-9604, 6 mg CJC-1295, 10 mg ipamorelin, 10 mg MOTS-c in 5 mL Printed on the vial $279/mo

On the MOTS-c vial, 0.25 mL at 2 mg/mL is 0.5 mg per dose (math only), 20 doses per vial, about four weeks, which is why refills run every 28 days, one vial per fill. Morning timing places the dose near the day's activity. EUPHORIA also contains kisspeptin, which acts on the reproductive axis; see kisspeptin for women. No study has tested MOTS-c in combination with anything. Your physician sets your directions after reviewing your intake, and they are printed on the vial. The dosing detail is in MOTS-c dosage.

Every price is all-in, covering medication, physician review, refill management and shipping, with no membership fee. Pepti is cash-pay and does not bill insurance. Vials arrive ready to use with sterile syringes and alcohol prep pads, shipped refrigerated with ice packs. Lot test results are at /quality/lab-results.
