KPV vs BPC-157: Inflammation, Gut and Skin, and Whether to Combine Them

This page explains each on its own terms, puts them side by side for inflammation, gut and skin, covers what the research does and does not show, where each stands with the FDA, how each is dosed, what each costs at Pepti, and when combining them makes sense.

KPV (Injectable)
KPV$229/mo

KPV and BPC-157 are both prescribed for gut and inflammation complaints, but they are described as doing different jobs. KPV is a three-amino-acid fragment of alpha-MSH, described in cell and mouse work as damping the signalling that produces inflammatory cytokines. BPC-157 is a fifteen-amino-acid fragment of a gastric protein, described in animal work around protecting the gut lining, growing new blood vessels and moving repair cells into damaged tissue. They are frequently prescribed together, and three Pepti blends, KLOW, FORTIFY and ELEVATE, contain both. Both are prescription-only, compounded by a licensed US pharmacy, and neither is FDA approved.

KPV vs BPC-157

KPV on its own terms

What it is

KPV is a tripeptide, lysine-proline-valine, the C-terminal end of alpha-melanocyte-stimulating hormone (alpha-MSH), a hormone the body makes from pro-opiomelanocortin. Alpha-MSH drives pigmentation, but it also acts as an anti-inflammatory signal. Researchers noticed the anti-inflammatory effect tracked with the tail of the molecule, and a 2008 review in Endocrine Reviews describes the three-amino-acid fragment keeping much of that activity while pigmentation depends on a different part of the sequence. KPV is not a steroid, not a hormone in its own right, and not a tanning peptide. The full reference is at what is KPV.

How KPV is described to work

  • Entry through PepT1. A 2008 paper in Gastroenterology reported that KPV is carried into intestinal cells by PepT1, the transporter the gut lining uses to absorb small peptides, and that its effect depended on that transporter. PepT1 is described as upregulated in inflamed intestinal tissue.
  • NF-kB and MAPK. Once inside, KPV is described as interfering with NF-kB and MAPK, two of the main pathways that turn an inflammatory trigger into a response. A 2012 paper in the International Journal of Physiology, Pathophysiology and Pharmacology reported this in human bronchial epithelial cells, through a route that did not depend on the pigment receptor.
  • Lower cytokine output. In cell and mouse work the result is less TNF-alpha, IL-6, IL-1beta and IL-8. It is a "less fuel on the fire" description rather than "puts the fire out."

All of the mechanism work is in cells and animals.

KPV vs BPC-157

BPC-157 on its own terms

What it is

BPC-157 is a pentadecapeptide, fifteen amino acids in the sequence GEPPPGKPADDAGLV, a partial sequence of a protein originally isolated from human gastric juice. The literature calls it "stable gastric pentadecapeptide BPC 157" because laboratory work reported it staying intact in gastric juice for hours. The gut is where its research began; the tendon and soft-tissue work came later. It is not a hormone or a steroid. The full reference is at what is BPC-157.

How BPC-157 is described to work

  • New blood vessels. Animal work describes BPC-157 activating the VEGFR2 receptor and downstream nitric-oxide signalling, the axis the body uses to grow blood vessels into healing tissue.
  • Cell migration. Cell studies describe activation of the FAK-paxillin pathway in tendon fibroblasts, which governs how cells attach and move into a wound.
  • Gut-lining protection. The largest body of BPC-157 research describes protection of the stomach and intestinal lining against NSAIDs and alcohol in animal models. A 2020 paper in Current Pharmaceutical Design reported, in cell and rat models of NSAID damage, stabilized intestinal permeability and tight-junction integrity.

All of the mechanism work is in animals and cells.

The real difference is the job, not the strength

People often ask which is "stronger." No study has measured both in the same people, or measured either in people for inflammation at all. The useful difference is what each is described as doing: KPV turns down a signal, BPC-157 supports a repair process. That is why they are combined more often than chosen between.

KPV vs BPC-157

By goal: inflammation, gut and skin

Goal KPV BPC-157
Inflammation without a specific injury The one product whose primary described action is anti-inflammatory Some anti-inflammatory description, but its story is repair
Gut inflammation Mouse colitis work; PepT1 uptake in inflamed tissue Animal colitis work; lining protection against NSAIDs and alcohol
The gut lining itself Not its main story The original and deepest part of its literature
A tendon, muscle or surgical site Not its territory Rat tendon and muscle work
Skin Mouse and keratinocyte work, much of it on the parent hormone Angiogenesis in animal models; no randomized human trials for skin
  • Inflammation

KPV suits the person whose inflammation is not attached to a specific injury: the diffuse ache, the gut that reacts to everything, the skin that flares. BPC-157 suits inflammation that comes with tissue damage, where the goal is repair. If the picture is inflammation plus an injury that will not settle, KLOW combines KPV with BPC-157, TB-500 and GHK-Cu. The wider topic is at peptides for inflammation.

  • Gut

Both have animal colitis data and neither has a human trial of the injectable form. BPC-157 is the usual choice when the lining itself is the concern, such as upper-gut soreness after NSAID use or recovery after a properly managed flare. KPV is the usual choice when the complaint is inflammation-led. Because KPV's gut work centres on inflamed tissue, there is little in its literature to suggest it does much in a gut that is not inflamed. The full guide, including the symptoms that need investigation before any peptide, is at peptides for gut health.

  • Skin

Neither is primarily a skin peptide. The KPV-specific skin literature is thinner than its gut literature, and much of it concerns alpha-MSH rather than the tripeptide. BPC-157 is in skin blends for its blood-supply mechanism. When skin is the main goal, GHK-Cu is aimed at it more directly; see peptides for skin.

KPV vs BPC-157

Can you take KPV and BPC-157 together?

Yes, they are frequently prescribed together, on the rationale that one addresses inflammatory signalling and the other tissue repair. No study has tested the combination, so it means two unknowns at once rather than one. Some people start with one, keep a simple log for four weeks, and add the second only if there is a reason to. Others want both in one injection. Pepti has three blends that contain both:

Blend KPV per vial BPC-157 per vial Also contains Per 0.25 mL dose Price
KLOW 10 mg 10 mg 50 mg GHK-Cu, 10 mg TB-500 0.5 mg KPV, 0.5 mg BPC-157 $259/mo
ELEVATE 10 mg 10 mg 5 mg Semax, 5 mg Selank 0.5 mg KPV, 0.5 mg BPC-157 $249/mo
FORTIFY 2 mg 5 mg 5 mg Thymosin A-1, 1 mg LL-37 0.1 mg KPV, 0.25 mg BPC-157 $289/mo
  • KLOW suits gut or inflammation complaints alongside a skin or connective-tissue goal. See what is KLOW.
  • ELEVATE suits a gut complaint entangled with mood or concentration complaints. See what is ELEVATE.
  • FORTIFY suits gut symptoms alongside frequent infections or an immune reason for thymosin alpha-1. See what is FORTIFY.

There is no Pepti blend of KPV and BPC-157 alone. On their own, each is one variable, which makes it easier to judge what is doing what; a blend is a convenience, not a stronger version. Can you take two peptides together covers the general question.

KPV vs BPC-157

When neither is the right first step

Situation What makes more sense
Blood in the stool, unintentional weight loss, persistent vomiting or symptoms that wake you at night Investigation by a gastroenterologist or your primary physician first
A diagnosed inflammatory condition on prescribed treatment Stay on it; a peptide can sit alongside only with your specialist aware
Frequent infections are the real problem Thymosin alpha-1, the immune conversation
A specific soft-tissue injury BPC-157 + TB-500
Gut symptoms driven mainly by stress Selank or Semax + Selank

KPV vs BPC-157

What the evidence shows, honestly

KPV has no human studies. That is the FDA's finding in its review for the July 2026 advisory committee: no published studies in which KPV products were used in humans, and none of the nine references the nominator cited were human studies. The evidence is cell work, including human intestinal, T-cell and bronchial cell lines, and mouse models of colitis, skin inflammation and brain injury. The 2008 Inflammatory Bowel Diseases and Gastroenterology papers reported reduced colitis severity in mice, much of it with KPV given orally in drinking water; the subcutaneous route prescribers use has not been compared.

BPC-157 has very limited human data. The FDA's review identified one randomized, double-blind, placebo-controlled study: a two-week enema trial in 53 people with mild to moderate ulcerative colitis, reported only as a 2005 conference abstract. The confidence interval for the difference between groups crossed zero, and the reviewers judged the data inadequate to support efficacy. Beyond that, a phase 1 oral tablet study in healthy volunteers with no results posted and a 2025 pilot of intravenous infusion in two healthy people. There is no randomized human trial of the injectable form for any indication. The animal literature, by contrast, is extensive and internally consistent.

Neither has been shown in people to reduce inflammation, protect the gut lining or improve skin. Physicians prescribe both on mechanism plus clinical experience. The accurate framing is "described in preclinical research and used clinically," not "shown to work in people."

KPV vs BPC-157

Where each stands with the FDA

Both were placed in Category 2 of the FDA's 503A bulk substances list in 2023. As of the FDA pages checked in September 2026:

KPV BPC-157
2023 Category 2 Category 2
15 April 2026 Removal from Category 2 announced, effective 22 April Removal from Category 2 announced
Advisory committee 23 July 2026: voted 8 to 6, one abstention, to recommend adding KPV to the 503A Bulks List 23 July 2026: voted 8 to 6, one abstention, to recommend adding BPC-157 to the 503A Bulks List
FDA staff position Recommended against inclusion Recommended against inclusion
Final FDA determination Pending Pending

A committee vote is a recommendation, not a decision. Both remain legal to prescribe and dispense as compounded medications. Compounded medications are not FDA approved, and neither KPV nor BPC-157 is an FDA-approved drug. Are peptides FDA approved explains the difference.

KPV vs BPC-157

Dosing, side by side

Both are injected under the skin (subcutaneously) into the abdomen or thigh, rotating sites. The schedules differ:

  • KPV: 0.25 mL, 25 units on a U-100 insulin syringe, once daily Monday through Friday. At 2 mg/mL that is 0.5 mg per dose, 20 doses per vial, about four weeks.
  • BPC-157: 0.18 mL, 18 units, once daily. At 1 mg/mL that is 0.18 mg per dose, 27 doses per vial, about 3.9 weeks. It also comes as a pre-filled Pen cartridge.
  • The blends that contain both are dosed at 0.25 mL once daily Monday through Friday, 20 doses per vial.

You do not need to calculate anything: your directions are printed on the vial, and your physician sets them after reviewing your intake. Refills run every 28 days, one vial at a time. Both ship refrigerated with ice packs and stay in the fridge between doses. Technique is covered in how to inject peptides safely.

Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.

KPV vs BPC-157

Side effects

KPV. Described as well tolerated in the published animal work, which has not reported toxicity at the doses studied. Reported clinically: injection-site redness, mild soreness or bruising, and some early mild nausea or a transient headache. With no human trials, "well tolerated" means nothing concerning has emerged, not that it has been tested.

BPC-157. Described in animal work as having a wide margin between effective doses and doses that cause problems. Reported clinically: injection-site redness, mild soreness or bruising, and some early nausea or light-headedness. In the enema studies, headache and flatulence were the most frequent adverse events, with no serious ones.

Neither has an established interaction list. Anyone on immunosuppressants or biologic therapy should raise that specifically. Stop and contact your physician for any reaction that is severe, spreading, or involves difficulty breathing.

KPV vs BPC-157

Who should not take either, or should discuss it first

Situation Why
Undiagnosed red-flag gut symptoms Investigation comes first
Personal history of cancer Especially for BPC-157, which is described as supporting new blood-vessel growth
Anticoagulant therapy Especially for BPC-157, given the vascular mechanisms
Immunosuppressants or biologic therapy Raise it at intake
Pregnancy or breastfeeding Not used; safety data is absent
Tested athletes BPC-157 is named on the WADA Prohibited List; KPV is not named but falls under non-approved substances

KPV vs BPC-157

Where to get KPV or BPC-157 prescribed

Neither can be bought legitimately without a prescription. Sites selling them "for research use only" are not accountable for what is in the vial: identity, purity, sterility and concentration are untested. The prescription route:

  1. Complete a free online intake covering your history, medications and what you want help with.
  2. A physician licensed in your state reviews it, usually within 24 hours.
  3. If it is appropriate, a state-licensed, FDA-registered US pharmacy compounds it to that prescription. Lot test results are published at /quality/lab-results.
  4. It ships refrigerated with your directions printed on the vial.
  5. Your physician stays reachable for questions and dose changes.

Pepti prescribes both in all 50 states and DC. Step-by-step pages: how to get KPV prescribed online, how to get BPC-157 prescribed online and how to get KLOW prescribed online.
