Can You Take BPC-157 With Tirzepatide or Semaglutide?
This page covers how the two medications differ, what the evidence says about using them together, the stomach-side-effect question, how a daily vial and a weekly vial fit into one routine, what to tell your physician, and how to add BPC-157 properly.

Treatments mentioned
BPC-157 and tirzepatide work through unrelated pathways, and a physician can prescribe them together, but no study has tested the combination and nothing shows BPC-157 adds anything to tirzepatide. Tirzepatide acts on appetite and gastric emptying through the GIP and GLP-1 receptors; BPC-157 is described in animal and cell work around blood-vessel growth, tendon repair and protection of the gut lining. The most common reason people ask is stomach side effects on a GLP-1, and those are a dose and titration question for your physician, not a reason to add a second vial. Everything on this page applies to semaglutide the same way.

Can You Take BPC-157 With Tirzepatide or Semaglutide?
The short answer
| Question | Answer |
|---|---|
| Can they be prescribed together? | Yes, when a physician reviewing your intake judges it appropriate |
| Has the combination been studied? | No. There is no human or animal study of BPC-157 with tirzepatide or semaglutide |
| Do they act on the same system? | No. Tirzepatide acts at the GIP and GLP-1 receptors; BPC-157 is described around angiogenesis, nitric oxide signalling and tissue repair |
| Does BPC-157 treat GLP-1 nausea or constipation? | No. That is a dose and titration question for your physician |
| Is there an established interaction? | No interaction list exists for BPC-157, because the trials that would produce one have not been run |
| Is it the same answer for semaglutide? | Yes |
| What is never combined? | Tirzepatide with semaglutide, or with any other GLP-1 medication |
The medications this page covers:
| Product | What is in it | Typical directions | Price |
|---|---|---|---|
| BPC-157 | 5 mL multi-dose vial, BPC-157 1 mg/mL (5 mg per vial) | 0.18 mL (18 units) under the skin once daily | $209/mo |
| Tirzepatide | Dose-specific 2 mL vials at six strengths, 2.5 mg to 15 mg weekly; glycine in the 2.5 mg and 7.5 mg vials | 0.50 mL (50 units) once weekly at every strength | From $159/mo |
| Semaglutide | Dose-specific vials at six strengths, starting with 1 mg per 1 mL at the 0.25 mg starting dose | Printed on the vial for your strength | From $99/mo |
Every price is all-in, covering medication, physician review, refill management and shipping, with no membership fee. GLP-1 prices are "from" prices because they are set by dose. Compounded medications are not FDA approved.

Two medications doing two different jobs
What tirzepatide does
Tirzepatide is a single peptide that activates two incretin receptors, GIP and GLP-1. It acts on appetite centres in the brain, slows the rate at which the stomach empties, and changes insulin and glucagon signalling. Its trials measured weight over a long period, and nearly every common side effect follows from the slowed stomach: nausea, reflux, early fullness, diarrhea and constipation, clustered after starting and after each dose increase. The full reference is what is tirzepatide.
What BPC-157 is described to do
BPC-157 is a fifteen-amino-acid fragment of a protein first isolated from human gastric juice. In animal and cell studies it is described as activating the VEGFR2 receptor and nitric oxide signalling, the axis the body uses to grow new blood vessels into healing tissue, and as speeding tendon fibroblast migration through the FAK and paxillin pathway. Its largest body of research is protection of the gut lining in animal models, against damage from NSAIDs and alcohol. It is not a hormone, it does not act on appetite, and it does not raise growth hormone. The full reference is what is BPC-157.

Tirzepatide BPC-157 What it is A dual GIP and GLP-1 receptor agonist A 15-amino-acid gastric peptide fragment Main described action Appetite, gastric emptying, insulin and glucagon signalling Angiogenesis, cell migration, gut-lining protection Acts on appetite? Yes No Human evidence Large randomised trials of the approved product No large randomised trials; early-phase human work for inflammatory bowel indications Schedule at Pepti Once weekly Once daily Route Under the skin Under the skin Because the two do not share a receptor or a described pathway, there is no mechanistic reason to expect one to amplify or block the other. That is a statement about mechanism, not a safety finding. No one has studied them together.

Why people ask: GLP-1 stomach side effects
The question usually comes from someone on tirzepatide or semaglutide who has nausea, reflux or constipation and has read that BPC-157 is a "gut peptide." The live gut-health reference answers it directly: nausea, reflux and constipation on semaglutide or tirzepatide are a dose and titration question for the prescribing physician, not a reason to add a second vial.
There are three reasons for that answer:
- The cause is known. GLP-1 stomach side effects come from slowed gastric emptying, and the approved labels state that most nausea, vomiting and diarrhea occur during dose escalation and decrease over time. Holding at the current strength, or stepping back, is the most powerful adjustment available, and it is made by your physician.
- The BPC-157 gut research is a different question. It concerns protection of the gut lining against injury from NSAIDs and alcohol in animals. Nobody has studied whether it changes GLP-1 nausea, and there is no reason in the research to expect it to speed up or slow down a stomach that a GLP-1 is deliberately slowing.
- A second vial can blur the picture. Some people report early nausea or light-headedness on BPC-157. Starting it while you are already queasy on a GLP-1 makes it harder for anyone to tell which medication is responsible.
What helps GLP-1 nausea, from portion size to meal timing, is in GLP-1 nausea: how to manage it. The full side-effect reference is tirzepatide side effects.
Where BPC-157 does appear alongside a GLP-1
BPC-157 is one of the four agents in the FUSION Blend, with MOTS-c, 5-Amino-1MQ and tesamorelin, which is positioned for use with or without a GLP-1. Prescribers using FUSION alongside a GLP-1 describe less of the gut discomfort patients associate with the GLP-1. That has not been tested in a trial, and it is a description of experience, not a finding.

When BPC-157 alongside a GLP-1 makes sense
The legitimate case for the pairing is an unrelated reason to take BPC-157 that happens to arise while you are on tirzepatide: most often a tendon, ligament or muscle problem, which is the area where the BPC-157 animal literature is strongest. A person on a weight program who starts training more often is a common example. In that case the two medications are treating different things, and the question is simply whether your physician is comfortable adding a second agent to your record.
Even then, some honest limits apply. For tendinopathy, progressive loading under a physiotherapist has human trial evidence, which BPC-157 does not. A complete rupture or displaced fracture is a surgical question. And BPC-157 is not a weight treatment and adds nothing known to tirzepatide's effect.

Fitting a daily vial and a weekly vial together
| BPC-157 | Tirzepatide | |
|---|---|---|
| Draw | 0.18 mL (18 units on a U-100 insulin syringe) | 0.50 mL (50 units) |
| Medication per dose | 0.18 mg BPC-157 | Your weekly strength, 2.5 mg to 15 mg |
| How often | Once daily | Once weekly, same day each week |
| Doses per vial | 27 | 4 |
| One vial lasts | About 3.9 weeks | Four weeks |
The BPC-157 figures are math from the vial concentration: 0.18 mL of a 1 mg/mL solution is 0.18 mg, and 5 mL divided by 0.18 mL gives 27 full doses. Both medications run on a 28-day refill cycle, one vial per fill.
Practical points:
- Two vials, two syringes. Never draw both into one syringe. Each label was calculated against its own vial's concentration, and neither describes a mixture.
- Rotate sites. Both go into the fat layer of the abdomen or thigh. On the day they coincide, use a different site for each.
- Keep the tirzepatide day fixed. Your weekly injection day stays the same; the daily BPC-157 dose fits around it.
- Refrigerate both. Both ship cold with ice packs, and beyond-use dating runs from first puncture. See how to store peptides.
How each product's vial is counted is in how long does a peptide vial last.

What to tell your physician first
| Situation | Why it matters |
|---|---|
| Personal history of cancer | Angiogenesis supports any tissue that needs blood supply, so it is raised before BPC-157 |
| Personal or family history of medullary thyroid carcinoma or MEN 2 | Rules out tirzepatide and semaglutide |
| Anticoagulant therapy | Raised before BPC-157, given its vascular mechanisms |
| Pregnancy, planning pregnancy or breastfeeding | Neither is used; tirzepatide is ruled out and BPC-157 has no safety data |
| Insulin or a sulfonylurea | Low blood glucose on tirzepatide becomes more likely |
| Oral contraceptives | May be absorbed less reliably for four weeks after starting tirzepatide and after each dose increase |
| Current nausea, vomiting or constipation on a GLP-1 | Report it first; it is a titration question |
| Tested athlete | Many peptides are prohibited in competition; check your sport's list |
Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.
Can You Take BPC-157 With Tirzepatide or Semaglutide?
Semaglutide works the same way
Semaglutide acts at one incretin receptor, GLP-1, where tirzepatide acts at two, but both work through appetite and gastric emptying, and both cause the same family of stomach side effects. So the answer is identical: BPC-157 can be prescribed alongside semaglutide for an unrelated reason, the combination has not been studied, and BPC-157 is not a treatment for semaglutide's side effects. The comparison between the two GLP-1 medications is in semaglutide vs tirzepatide. The one firm rule: semaglutide and tirzepatide are never taken together.

How to add BPC-157 to tirzepatide properly
- Name the reason. "My Achilles has been sore since I started running" gives your physician something to work with. "My stomach is upset on tirzepatide" is a message to your care team about the GLP-1, not a BPC-157 request.
- List everything. Include every medication and supplement, and anything you already take that contains BPC-157, such as the BPC-157 + TB-500 stack, GLOW, KLOW or FUSION, so nothing is duplicated.
- Start one thing at a time. Avoid starting BPC-157 in the same week as a tirzepatide dose increase, so any change has one plausible cause.
- Report what happens, including nothing. Your physician stays reachable through your account.
Your physician sets your directions after reviewing your intake, and every vial arrives with its directions printed on the label. More on combining is in can you take two peptides together and what peptides can you take with tirzepatide. Step-by-step guides: how to get BPC-157 prescribed and how to get tirzepatide prescribed.
Can You Take BPC-157 With Tirzepatide or Semaglutide?
What the evidence shows, honestly
Tirzepatide has a large human trial base in its approved form. BPC-157 has animal and cell research, early-phase human work for inflammatory bowel indications, and substantial prescribing experience, but no large randomised human trials. There is no study of the two together, no data on whether BPC-157 changes GLP-1 side effects, and no established interaction list for BPC-157 with anything. What can be said is narrower: the mechanisms are unrelated, the reasons to take each are different, and the pairing is a reasonable thing to discuss when there is a separate reason for BPC-157. It is not a stack that has been shown to work better than tirzepatide alone.
On regulation: BPC-157 was removed from Category 2 of the FDA's 503A bulk substances list on 15 April 2026, and in July 2026 the FDA's Pharmacy Compounding Advisory Committee voted 8 to 6 to recommend adding it to the 503A Bulks List. The FDA's final determination is pending.

Common questions
Can You Take BPC-157 With Tirzepatide or Semaglutide?, answered.
It can be prescribed alongside tirzepatide when a physician reviewing your intake judges it appropriate. The two act through unrelated pathways, but no study has tested the combination, so the reason for adding BPC-157 should be something other than the GLP-1 itself.
The answer is the same as for tirzepatide. Different mechanisms, no study of the pairing, and BPC-157 is not a treatment for semaglutide's side effects.
There is no evidence that it does. GLP-1 nausea comes from slowed gastric emptying and is managed by meal size, meal composition and the pace of dose increases. Message your care team if it is not settling.
Not on any evidence. BPC-157's gut research concerns protection of the gut lining against NSAID and alcohol injury in animals. Constipation and reflux on a GLP-1 are reported to your care team so your physician can review your directions.
No study suggests it does. BPC-157 does not act on appetite and is not a weight treatment.
On the day they coincide, they are two separate injections from two separate vials, each drawn with its own syringe and given at a different site. Follow the directions printed on each vial.
No. Each label was calculated against its own vial's concentration, and neither describes a mixture.
There is no study of the combination and no established interaction list for BPC-157, so nobody can describe the pairing as tested. What is reported with BPC-157 is mostly injection-site redness or soreness, with some early nausea or light-headedness. Your physician reviews your full medication list before prescribing.
No. Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.
FUSION contains BPC-157 with MOTS-c, 5-Amino-1MQ and tesamorelin and is positioned for use with or without a GLP-1, at $279 a month. Prescribers describe less GLP-1 gut discomfort with it; that has not been tested in a trial.
No. Never stop or change a GLP-1 dose on your own. Tell your care team what you want to add and why. The free assessment goes to a physician licensed in your state. A consultation does not guarantee a prescription.
