Do You Need a Prescription for PE-22-28? Is It Legal to Buy Online?
At Pepti, PE-22-28 is prescribed by a physician licensed in your state and costs $219 a month all-in for a 5 mL vial holding 10 mg at 2 mg/mL. The ordering steps are on the PE-22-28 how-to-get page; this page covers the prescription and legal questions.

Treatments mentioned
Yes. PE-22-28 for human use in the United States requires a prescription from a licensed physician, filled by a licensed compounding pharmacy. PE-22-28 sold online without a prescription is a research-use-only product whose seller disclaims human use. The FDA has never evaluated PE-22-28: it is not in Category 1, 2 or 3 of the FDA's 503A nominated-substances list, has no FDA safety notice and was not on the July 2026 advisory committee agenda. It is not FDA approved, compounded medications never are, and it has no published human trials of any kind, which is exactly why a physician review matters.

What PE-22-28 is, and why that matters for the prescription question
PE-22-28 is a seven-amino-acid fragment of the sortilin propeptide, a 44-amino-acid piece clipped off when the body makes the sortilin receptor. Researchers in Nice, France found that part of that propeptide, which they named spadin (residues 12 to 28), blocks a potassium channel in nerve cells called TREK-1. PE-22-28 is the shorter version, residues 22 to 28 only, described in Frontiers in Pharmacology in 2017. The same peptide is written PE 22-28, PE-22-28 and PE22-28. The full background is in what is PE-22-28.
Two features shape the legal and practical picture:
- It has no human trials. Everything published on PE-22-28 itself is cell and rodent work. There is no human dose-finding study, no human safety data and no human efficacy data.
- Its proposed mechanism runs through serotonin signalling. In rodents, blocking TREK-1 on serotonergic neurons was associated with increased firing, the same direction of effect SSRIs are thought to produce by a different route. That makes the medication list the most important part of the intake.

Why PE-22-28 is prescription-only
The federal rule
Section 503(b) of the Federal Food, Drug, and Cosmetic Act makes a drug prescription-only when it is "not safe for use except under the supervision of a practitioner licensed by law to administer such drug," including because of "the method of its use." An injectable peptide with no human safety data meets that description. Dispensing it without a prescription makes it misbranded, and the violation attaches to the dispenser. See are peptides legal in the US.
How a compounded prescription is lawful
Compounded PE-22-28 is not FDA approved. Its legal footing is section 503A, which exempts a drug compounded "for an identified individual patient based on the receipt of a valid prescription order" by a licensed pharmacist in a state-licensed pharmacy. That is why the vial carries your name and directions. State law decides who may prescribe to you and which pharmacies may ship to your state. Pepti prescribes PE-22-28 in every state except Oklahoma, plus DC.
Why the physician review matters for PE-22-28
What a physician weighs Why Antidepressants, triptans, lithium, tramadol or any other serotonergic drug The proposed mechanism overlaps with serotonin signalling, and the combination has never been studied in people A diagnosed mood disorder PE-22-28 is not a treatment for depression, and no existing antidepressant should be stopped on the strength of a rodent literature Diabetes or glucose-lowering medication Cell work described the parent peptide increasing insulin secretion from pancreatic beta cells A seizure history TREK-1 is involved in seizure protection; the rodent work found no problem, but it should be discussed Pregnancy or breastfeeding Not used; safety data is absent Tested sport See the sport row below There is no established interaction list, because the trials that would produce one have not been run. That is why a physician reads your medications rather than a form. New or worsening thoughts of self-harm are an emergency that no peptide addresses: contact a crisis line or call 911.

Where PE-22-28 stands with the FDA
As Pepti's reference page records it, checked in September 2026:
| FDA list or process | PE-22-28 |
|---|---|
| 503A nominated substances, Category 1, 2 or 3 (updated 14 May 2026) | Not listed in any category |
| Significant-safety-risk page | Not mentioned, either in the current Category 2 table or among substances withdrawn from it |
| The peptides removed from Category 2 in April 2026 | Not among them; it was never there |
| 503A bulks list at 21 CFR 216.23 | Not listed; it still holds the six substances placed there in 2019 |
| Pharmacy Compounding Advisory Committee, 23 to 24 July 2026 | Not on the agenda, which covered BPC-157, KPV, TB-500, MOTS-c, DSIP, Semax and Epitalon |
| FDA approval as a drug | None |
The accurate summary is never categorised, never flagged, never reviewed. The FDA's 503A page also notes that substances nominated on or after 7 January 2025 are no longer placed into the interim categories at all. The FDA has expressed no view on PE-22-28 either way, which is different from a substance that has been through the process and been recommended for or against. Where a substance sits with the FDA is one input into a compounding pharmacy's decision to prepare it; state pharmacy law and the pharmacy's own regulatory judgement are the others.

What a research-use-only vial is
Research sellers rely on 21 CFR 201.125, which exempts a drug from carrying directions for use when it is sold to people "regularly and lawfully engaged" in research "not involving clinical use." A person injecting it is the clinical use excluded. Under 21 CFR 201.128, the seller's own advertising shows a product's intended use, so mood or focus claims beside a "not for human consumption" label contradict each other. PE-22-28 and tested sport
The 2026 WADA Prohibited List does not name PE-22-28. Its S0 section, non-approved substances, prohibits at all times "any pharmacological substance which is not addressed by any of the subsequent sections of the List and with no current approval by any governmental regulatory health authority for human therapeutic use." PE-22-28 has no such approval anywhere, so a compounded preparation can fall under S0. A prescription does not change anti-doping rules. Tested athletes should check with their anti-doping organisation before starting. See peptides for athletes.

The evidence, in one paragraph
There are no randomised controlled trials of PE-22-28 in humans, and no published human trials of any kind. The 2017 Frontiers in Pharmacology paper reported PE-22-28 reproducing spadin's antidepressant-like effects in mice after a four-day course, with stronger TREK-1 inhibition in cultured cells and a half-life in the mouse of roughly a day. A 2019 Neuropharmacology paper reported sortilin-derived peptides associated with improved recovery in a mouse stroke model. The only human data in the area is observational measurement of the body's own sortilin propeptide in blood, which is not a study of giving PE-22-28 to anyone. Broader context on mood and focus peptides is in peptides for anxiety and mood.

Yes, to obtain it as a medication for your own use. A physician licensed in your state prescribes it and a licensed compounding pharmacy prepares it.
Yes, with a valid prescription filled by a licensed pharmacy. It is not FDA approved, and the FDA has not evaluated it.
Yes, through a telehealth provider whose physician is licensed in your state. Sellers shipping without a prescription sell it as a product not intended for human use.
No. No drug agency has approved it for anything, and compounded medications are not FDA approved. See are peptides FDA approved.
No. It has never been placed in any of the FDA's 503A interim categories, has had no FDA safety notice, and was not on the advisory committee's July 2026 agenda.
Closely related but not identical. Spadin is residues 12 to 28 of the sortilin propeptide; PE-22-28 is the last seven residues of it. Both come from the same French research group and act on the same channel.
Only if the physician who prescribes both says so. The combination has never been studied in people, and no existing antidepressant should be stopped on your own. List every psychiatric medication on the intake.
The 2026 WADA list does not name it, but the S0 non-approved substances section can apply to a compounded peptide with no human therapeutic approval. Check with your anti-doping organisation.
