PT-141 + Oxytocin for Men: What Each Component's Research in Men Shows
This page covers what each component's research in men measured, what it does not cover, how the combination differs from a PDE5 inhibitor and from PT-141 alone, the considerations that matter for men, side effects, and how to get it prescribed. The PT-141-only version of this page is PT-141 for men, and the full product reference is what is PT-141 + Oxytocin.

Treatments mentioned
For men, the PT-141 + Oxytocin vial combines one component with small early studies in men and one with mostly null results in men, and the combination has never been studied. PT-141 (bremelanotide) produced statistically significant erectile responses in two 2004 early-phase studies, including men who responded inadequately to sildenafil, but its men's programme never reached a phase 3 trial and the approved bremelanotide product is indicated only for premenopausal women. Oxytocin, the second component, did not change erection in a 2014 couples study, where men reported higher sexual satiety after sex, and it did not change sexual behaviour in a 2008 study of ten healthy men. Prescribing the vial to men is off-label use of approved actives, and compounded medications are not FDA approved.

The short answer
| Question | What the research shows | Strength of evidence |
|---|---|---|
| PT-141 and erectile response | 2004 studies, International Journal of Impotence Research, healthy men and men with erectile dysfunction | Randomised, early-phase; measured over hours |
| PT-141 with sildenafil | 2005 study in Urology, low-dose intranasal PT-141 | Small, early-phase |
| Oxytocin and erection | 2014 couples study, Hormones and Behavior: no effect on erection | Small study, nasal spray; negative |
| Oxytocin and men's experience after sex | Same study: men reported higher sexual satiety | Small study; small to moderate effects |
| Oxytocin and sexual behaviour in men | 2008 study in ten healthy men, Psychoneuroendocrinology: no change | Very small; negative |
| Oxytocin and anorgasmia | One 2008 case report in one man | Anecdotal |
| The combination in men | Never studied | None |
PT-141 + Oxytocin for Men
What is in the vial
| Component | Per 0.10 mL dose | What it is | Where it acts |
|---|---|---|---|
| PT-141 (bremelanotide) | 0.2 mg | A synthetic analogue of alpha-melanocyte-stimulating hormone | Melanocortin receptors in the brain, mainly MC4R for sexual function |
| Oxytocin | 0.1 mg | A hormone made in the hypothalamus and released from the pituitary | Smooth muscle of the reproductive tract, and oxytocin receptors in the brain |
PT-141 + Oxytocin is a 5 mL vial holding 10 mg PT-141 and 5 mg oxytocin. The prescribing rationale is that PT-141 is aimed at desire and initiation and oxytocin at the orgasmic phase and closeness afterwards. That is a rationale, not a finding. Neither component is a PDE5 inhibitor, neither acts on penile blood flow, and neither raises testosterone.

What PT-141's research in men looked at
PT-141 came out of the Melanotan II research line in the 1990s, when male volunteers testing that tanning peptide reported spontaneous erections. Bremelanotide was taken forward for the sexual-function effect.
| Population | Route | What was reported |
|---|---|---|
| Healthy men, without visual sexual stimulation | Intranasal | Statistically significant erectile response, measured by RigiScan, at doses above 7 mg |
| Men with mild-to-moderate erectile dysfunction | Intranasal | Statistically significant erectile response |
| Healthy men | Subcutaneous | Erectile response at doses above 1 mg |
| Men with an inadequate response to sildenafil 100 mg | Subcutaneous | Erectile response at 4 mg and 6 mg |
The detail that matters is "without visual sexual stimulation." An erectile response without stimulation is the signature of a drug acting in the brain rather than on blood flow. A 2005 paper in Urology reported that low-dose intranasal PT-141 with sildenafil produced a larger erectile response than either alone, consistent with the two acting at different points.
The men's programme did not continue to an approved product. The late-phase trials enrolled women only, and the approved label states it is not indicated in men.

What oxytocin's research in men looked at
Oxytocin rises around orgasm
A 1987 paper in the Journal of Clinical Endocrinology and Metabolism measured plasma oxytocin rising during arousal and peaking around orgasm in both sexes. A 2015 review in the European Journal of Pharmacology summarises animal work describing roles for oxytocin in male erection and ejaculation. That physiology is the starting point for the oxytocin half of the vial.
The 2014 couples study
29 healthy heterosexual couples took 24 IU of intranasal oxytocin or placebo in a double-blind crossover and were assessed after intercourse at home. Oxytocin did not change sexual drive, arousal, erection or lubrication. It was associated with higher reported orgasm intensity and contentment after intercourse, and men reported higher sexual satiety; the authors described the effects as small to moderate and more pronounced in men.
The 2008 study in ten men
A double-blind crossover study in Psychoneuroendocrinology gave ten healthy men 24 IU of intranasal oxytocin before laboratory-induced arousal and orgasm. Plasma oxytocin rose sharply, but appetitive, consummatory and refractory sexual behaviour did not change on the scale used, though eight of ten could correctly guess when they had received oxytocin.
Anorgasmia
A 2008 paper in the Journal of Sexual Medicine is a single case report: one man with treatment-resistant acquired anorgasmia, given intranasal oxytocin during intercourse, with ejaculation reported as restored. It is one patient, not a trial.
Partners
A 2017 follow-up to a 2015 trial in women found the male partners' sexual quality of life improved whichever spray their partner used, oxytocin or placebo.

What the research in men does not cover
- The combination. No study has tested PT-141 and oxytocin together in men, or anyone.
- The compounded dose. Every subcutaneous PT-141 dose in the men's studies was above 1 mg; a combination dose is 0.2 mg PT-141 with 0.1 mg oxytocin.
- The injection route for oxytocin. Every oxytocin study above used a nasal spray measured in IU, which cannot be converted to the milligrams on the vial.
- Desire in men. The PT-141 studies measured erections in a laboratory; the couples study found no oxytocin effect on drive.
- A phase 3 trial in men, for either component's sexual use.
- Long-term safety in men. Most PT-141 safety data come from women, and there are no systematic safety data for oxytocin injection at these doses.

PT-141 + Oxytocin vs the alternatives men usually weigh
| PT-141 + Oxytocin | PT-141 alone | Oxytocin alone | PDE5 inhibitors (sildenafil, tadalafil) | |
|---|---|---|---|---|
| Where it acts | Melanocortin receptors in the brain, plus oxytocin receptors | Melanocortin receptors in the brain | Oxytocin receptors | Blood vessels of the penis |
| Human evidence in men | None for the combination | Small early-phase studies | Small nasal spray studies, mostly null | Decades of large outcome trials |
| Effect on erection in studies | Not studied | Reported in 2004 studies | None in the couples study | The established use |
| Per dose at Pepti | 0.2 mg PT-141, 0.1 mg oxytocin | Printed on the vial | Printed on the vial | Set by the prescriber |
| Key cautions | PT-141's blood pressure rise, nausea, pigmentation | Same | Few described at these doses | Their own interactions, set by the prescriber |
If erection quality is the issue and desire is intact, a PDE5 inhibitor has far larger trials for that job, and a physician may reasonably suggest one first. If the question is desire, PT-141 alone carries the component with the evidence, without an oxytocin component that has no controlled evidence of adding to it. The combination is for a man whose physician judges the orgasm-and-closeness part of the picture worth addressing in the same injection, accepting that the evidence for that part is thin. Using either alongside sildenafil or tadalafil depends on your blood pressure and heart history; raise it at intake rather than combining them yourself.

What changes for men
Low desire often has a findable cause
Sleep, stress, a relationship, a medication or hormones are common drivers. SSRIs and SNRIs are among the commonest reversible causes of low libido, and beta blockers and finasteride are each documented to affect libido in some people. If desire changed the month a new medication started, that is a conversation with whoever prescribed it. Peptides for libido sets out what a physician looks at first.
Testosterone
Neither component raises testosterone or replaces testosterone therapy. Where low desire comes with fatigue, low mood or loss of morning erections, the hormone axis is a different conversation; kisspeptin acts upstream on that axis, and the comparison is in kisspeptin vs PT-141. If you already take testosterone, tell your physician; see can you take peptides with testosterone.
Blood pressure and the heart
Each dose of PT-141 produces a transient rise of about 6 mmHg systolic and 3 mmHg diastolic at most, peaking two to four hours after the dose and usually back to baseline within about twelve, with heart rate falling by up to 5 beats a minute. The approved bremelanotide product is contraindicated in uncontrolled hypertension and known cardiovascular disease, and the combination vial carries the same pharmacology. Erectile problems are often vascular, so heart history and blood pressure medication belong on the intake. Peptides for men over 40 covers the wider picture.
Pigmentation
Bremelanotide binds MC1R, the melanocyte receptor, most strongly. Focal darkening of the face, gums or other skin was reported in 1% of women dosing up to eight times a month and in 38% of people given it daily for eight days in a separate study, and it did not always resolve after stopping. The mechanism is the same in men, which is why dosing is capped by month and a history of melanoma or atypical moles goes on the intake.
Naltrexone and oral medicines
Bremelanotide can significantly reduce exposure to oral naltrexone, so men taking naltrexone for alcohol or opioid dependence should not use this vial. It may also slow gastric emptying and reduce absorption of oral medicines taken around the same time.
Sport
Neither bremelanotide nor oxytocin is named on the 2026 WADA Prohibited List; kisspeptin is, for males. Tested athletes should still check their own sport's current rules. The detail is in is PT-141 banned in sports.

Side effects
Almost all published rates come from the PT-141 trials in women at 1.75 mg. The mechanism is the same in men.
| What was reported | How often in the phase 3 trials | What to do |
|---|---|---|
| Nausea | 40%, against 1.3% on placebo | Usually worst with the first dose; tell your care team if it is marked |
| Flushing | 20% | Usually brief |
| Injection-site reactions | 13% | Rotate sites |
| Headache | 11% | Report it if it persists |
| A transient blood pressure rise | Each dose | Blood pressure should be controlled before starting |
| New skin or gum darkening | 1% at up to eight doses a month | Report it promptly |
| Chest pain, a severe headache, trouble breathing, or an erection lasting more than four hours | Not routine | Call 911 or go to an emergency department, then tell your physician |
There are no systematic safety data for oxytocin at the small subcutaneous doses in this vial. Never take a second dose within 24 hours: the approved bremelanotide label states that consecutive doses within 24 hours have not been shown to add anything and may add to the blood pressure effect. The full picture is in PT-141 side effects.

Where it stands with the FDA
Bremelanotide is the active ingredient of a product the FDA approved on 21 June 2019 for premenopausal women with acquired, generalised hypoactive sexual desire disorder; that label states it is not indicated in men. Oxytocin is the active ingredient of an approved hospital injection for labour and postpartum bleeding. Both may be compounded by a licensed 503A pharmacy as components of approved products. The combination vial is a different formulation prepared to an individual prescription, has not been reviewed by the FDA, and is not FDA approved.

Common questions
PT-141 + Oxytocin for Men, answered.
The combination has not been studied in men or anyone. PT-141 produced erectile responses in small early-phase studies in men; oxytocin did not change erection in the couples study or sexual behaviour in a study of ten men.
In the 2014 couples study, men on nasal oxytocin reported higher sexual satiety and more intense orgasms, with small to moderate effects. Whether an injected dose alongside PT-141 adds anything has not been tested.
There is no evidence that it is. PT-141 carries the men's data; the oxytocin component's controlled evidence in men is small and mostly null.
They have not been compared and do different things. Sildenafil acts on blood flow; PT-141 acts in the brain. For erection quality with desire intact, PDE5 inhibitors have far larger trials.
One small 2005 study combined low-dose intranasal PT-141 with sildenafil. Whether it is appropriate for you depends on your blood pressure and heart history, so raise it at intake.
No. Neither component acts on the hormone axis.
The only report is a single 2008 case of one man with anorgasmia, using a nasal spray. That is not a trial.
Pepti's typical directions are 2 to 4 hours before sexual activity, never more than once in 24 hours and no more than 8 doses a month. Your own directions are printed on the vial.
The published rates come from women on PT-141: nausea in 40%, flushing in 20%, injection-site reactions and headache. Each dose briefly raises blood pressure. An erection lasting more than four hours needs emergency care.
Not if it is uncontrolled. The PT-141 component raises blood pressure for a few hours after each dose, and the approved product is contraindicated in uncontrolled hypertension and known cardiovascular disease.
