Can You Take PE-22-28 With Tirzepatide or Semaglutide?

This page covers what each medication does, where they meet, the GLP-1 effects worth ruling out first, blood sugar, psychiatric medication, and how the two schedules fit.

PE-22-28 (Injectable)
PE-22-28$219/mo

PE-22-28 can be prescribed alongside tirzepatide or semaglutide after a physician reviews your intake: PE-22-28 is described as blocking the TREK-1 potassium channel in neurons, tirzepatide acts on appetite and gastric emptying, and no interaction between them is described. Nobody has studied the pairing, and PE-22-28 has no human trials of any kind, so "no interaction described" means none has been looked for. Two points matter most. Some GLP-1 side effects, such as fatigue, dizziness and low blood sugar, can feel like low mood or anxiety and should be reported before anything is added. And cell work described PE-22-28's parent peptide increasing insulin secretion, which puts blood sugar on the watch list for anyone on a GLP-1 who also takes insulin or a sulfonylurea. Everything on this page applies to semaglutide the same way.

Can You Take PE-22-28 With Tirzepatide or Semaglutide?

Two medications doing unrelated jobs

PE-22-28

PE-22-28 is a seven-amino-acid fragment of the sortilin propeptide, a piece clipped off when the body makes the sortilin receptor. It blocks TREK-1, a background potassium channel that helps set how easily a neuron fires. In rodents, blocking TREK-1 on serotonin neurons was associated with increased firing and antidepressant-like behaviour after a four-day course. There are no human trials. It is not described as acting on appetite, digestion or weight. See what is PE-22-28.

Tirzepatide

Tirzepatide activates the GIP and GLP-1 receptors. It acts on appetite centres in the brain, slows gastric emptying and changes insulin and glucagon signalling, and it is dosed once a week. See what is tirzepatide.

PE-22-28 Tirzepatide
Described target TREK-1 potassium channel in neurons GIP and GLP-1 receptors
Described around Mood-related behaviour in rodents Appetite, gastric emptying, insulin and glucagon
Acts on appetite? Not described to Yes, its main action
Human evidence None Large randomised trials of the approved product
Schedule Weekdays Once weekly

Rule out the GLP-1 first

People sometimes ask about PE-22-28 because their mood dipped after starting a GLP-1. Pepti's references do not list anxiety or depression among tirzepatide's side effects. For semaglutide, a large 2024 Nature Medicine cohort of 240,618 people reported lower rates of new suicidal ideation than on other weight-management drugs, and the suicidal-behavior warning on the approved semaglutide weight-management label was removed in February 2026. But several physical GLP-1 effects can feel like low mood or anxiety.

What you feel A possible GLP-1 cause What to do
Tiredness, low energy, a flat feeling Eating and drinking too little; fatigue 5 to 7% on tirzepatide against 3% on placebo Report it if it persists
Shakiness, sweating, confusion Low blood sugar, mainly with insulin or a sulfonylurea Treat it, then report it
Dizziness, light-headedness on standing Low blood pressure or dehydration Drink enough; same-day call if you cannot keep fluids down
A knotted, queasy stomach Slower gastric emptying Smaller meals; report it

Describe exactly what you feel and when, so your care team can tell these apart. Adding PE-22-28 to treat what is actually under-eating or low blood sugar would miss the cause, and PE-22-28 has never been tested for mood in a person. More is in GLP-1 anxiety and depression.

Blood sugar: the specific question for this pairing

Cell work described the parent peptide of PE-22-28 increasing insulin secretion from pancreatic beta cells (Journal of Diabetes Research, 2016). Nobody has measured that in people taking PE-22-28. Tirzepatide on its own rarely causes low blood sugar, but the risk rises with insulin or a sulfonylurea.

Your situation What it means
No diabetes, no glucose-lowering medication No specific signal; report shakiness or sweating if it happens
Type 2 diabetes on tirzepatide alone List it; your physician reviews the cell-work point
On insulin or a sulfonylurea as well Low blood sugar is the main thing to watch; your physician reviews all three

Know the signs of low blood sugar: shakiness, sweating, a fast heartbeat, confusion. Treat it first, then report it. Never change an insulin or sulfonylurea dose on your own.

Can You Take PE-22-28 With Tirzepatide or Semaglutide?

Where PE-22-28 and a GLP-1 overlap

Effect On PE-22-28 On tirzepatide or semaglutide What to do
Tiredness Not a described effect Fatigue is a listed side effect Start on different dates; report it if it persists
Headache Occasional in the first days Not a main point in Pepti's tirzepatide references Report it if it persists
Nausea Not a described effect The most common side effect Message your care team if it affects eating
Injection-site reactions The commonest clinical report 6 to 8% on tirzepatide against 2% Rotate sites; do not inject both in the same spot
A marked change in mood Not a described effect, always reportable Not listed for tirzepatide; always reportable Contact your physician promptly; 988 or 911 if you feel unsafe

Starting both in the same week makes tiredness, headache and mood changes impossible to place. Starting one thing at a time is simpler.

Can You Take PE-22-28 With Tirzepatide or Semaglutide?

Psychiatric medication

PE-22-28's proposed mechanism overlaps with serotonin signalling. If you take an antidepressant, a triptan, lithium, tramadol or any other serotonergic drug, or have been diagnosed with major depression, bipolar disorder or psychosis, that treatment comes first, and any decision about PE-22-28 involves the clinician managing it. The combination has never been studied in people. Never stop or reduce a psychiatric medication to make room for PE-22-28 or to help a GLP-1 work.

Two more points connect the three. Several antidepressants and antipsychotics are described in reviews as promoting weight gain, which can be part of why weight is hard to shift. And a GLP-1 slows stomach emptying, which can change how quickly oral medications are absorbed in the first weeks after starting or increasing the dose. List every medication on your intake, and if another prescriber manages your mood, tell them you have started a GLP-1 and PE-22-28. An active eating disorder is a situation where a GLP-1 is usually avoided; say so on the intake if it applies.

Can You Take PE-22-28 With Tirzepatide or Semaglutide?

How the two schedules fit together

PE-22-28 Tirzepatide
Draw 0.25 mL (25 units) on a U-100 insulin syringe 0.50 mL (50 units) at every strength
Per dose 0.5 mg PE-22-28 (2 mg/mL vial) Depends on the vial strength, 2.5 mg to 15 mg
When Once daily, Monday through Friday Once weekly
Doses per vial 20, about four weeks 4, four weeks
Refills Every 28 days, one vial per fill Every 28 days, one vial per fill

The schedules are independent, and there is no described need to separate them by hours. Use a separate fresh syringe for each and never draw both into one. Dosing detail is in PE-22-28 dosage and tirzepatide dosing chart. Never change either dose yourself.

Can You Take PE-22-28 With Tirzepatide or Semaglutide?

What not to combine

Combination Why not
Tirzepatide with semaglutide or any other GLP-1 Same receptor system, stacked side effects, no mechanism gained
PE-22-28 in place of a prescribed antidepressant That treatment comes first; never stop it on your own
Two of your vials in one syringe Neither label describes the mixture
Starting PE-22-28 in the same week as a GLP-1 dose increase Tiredness and mood changes become impossible to place
Anything sold "for research use only" Nobody is accountable for identity, purity, sterility or concentration

Can You Take PE-22-28 With Tirzepatide or Semaglutide?

Semaglutide works the same way

Everything above applies to semaglutide. It acts at the GLP-1 receptor only, where tirzepatide acts at two, but its effects that can feel like low mood are the same kind: fatigue in the early weeks, dizziness, and low blood sugar with insulin or a sulfonylurea. Semaglutide and tirzepatide are never taken together. See semaglutide side effects.

Can You Take PE-22-28 With Tirzepatide or Semaglutide?

What the evidence shows, honestly

There are no human trials of PE-22-28 of any kind. Its evidence is cell and rodent work, mostly from one French research group, and the only human data in the area measures the body's own sortilin propeptide in blood. PE-22-28 has not been evaluated by the FDA at all: it is not in any category of the 503A nominated-substances list updated 14 May 2026 and was not on the July 2026 advisory committee agenda.

There is no study of PE-22-28 with a GLP-1. What can be said is that the two act on unrelated systems, no interaction is described, and the overlaps worth managing are early tiredness, injection-site reactions, mood changes that need reporting, and blood sugar for people on insulin or a sulfonylurea. The fuller pictures are in PE-22-28 side effects and tirzepatide side effects, and pairings in general are in what peptides can you take with tirzepatide.

How to add PE-22-28 to a GLP-1 properly

  1. Report a mood change first. If it started with the GLP-1, your care team needs to know before anything is added.
  2. List every medication. Especially antidepressants, triptans, lithium, tramadol, insulin and sulfonylureas.
  3. Start one thing at a time. Begin PE-22-28 away from a GLP-1 dose increase.
  4. Report what happens, including nothing. Your care team is reachable through your account.

Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.
