Melanotan II Side Effects: Nausea, Flushing, Moles, and What to Report

This page is a practical reference: what to expect, when, what to watch on your skin, and what to report. What Melanotan II is, how it works and the full research history are in what is Melanotan II, and the comparison with its descendant is in PT-141 vs Melanotan II.

Melanotan II (Injectable)
Melanotan II$249/mo

The most common Melanotan II side effects in the human studies were nausea, facial flushing, a stretching and yawning sensation, reduced appetite, sleepiness at the top of the dose range, and in men spontaneous erections lasting on and off for one to five hours after a dose. They are dose-related and prescribers describe them as most noticeable at the start of a course. The side effect to take most seriously is on the skin: Melanotan II darkens existing moles and freckles, and case reports describe new and changing moles and a small number of melanoma diagnoses during or after use, without a causal link having been established. A personal history of melanoma or atypical moles rules it out.

Melanotan II Side Effects

The short answer

Side effect How common What to do
Nausea The most commonly reported effect in the human studies, at most dose levels Usually settles the same day; tell your care team if it is a problem
Facial flushing and warmth Common, usually within an hour or two of a dose Expected; report it if it does not settle
Stretching and yawning Noted in every human study, often before erections Transient; worth knowing about in advance
Reduced appetite Recorded on the study side-effect questionnaires Eat regular meals; it is not a weight treatment
Sleepiness Reported at the top of the study dose escalation Report it if it affects driving or work
Spontaneous erections (men) Expected in the hours after a dose; on and off for one to five hours in the studies An erection lasting more than four hours is an emergency
Darker moles and freckles Expected; it is the pigment pathway working Have moles examined before starting
New or changing moles Described in case reports Have any change in shape, border or texture looked at promptly

Product What is in it Typical printed directions Price
Melanotan II 5 mL multi-dose vial, Melanotan II 2 mg/mL (10 mg per vial) 0.25 mL (25 units) subcutaneously once daily, Monday through Friday $249/mo

At 2 mg/mL, 0.25 mL is 0.5 mg of Melanotan II per dose (arithmetic from the vial strength, on a U-100 insulin syringe), and a vial holds 20 doses, about four weeks of weekday dosing. Your own directions are set by your physician after reviewing your intake and are printed on the vial. The price is all-in: medication, physician review, refill management and shipping, with no membership fee. Melanotan II is compounded by a PCAB-accredited, FDA-registered US 503A pharmacy. Compounded medications are not FDA approved.

Melanotan II Side Effects

Why Melanotan II causes the side effects it does

Melanotan II is a synthetic, ring-shaped analogue of alpha-melanocyte-stimulating hormone (alpha-MSH), the peptide skin releases after sun exposure. It is non-selective: it activates several melanocortin receptors at once rather than only the skin's MC1 receptor. Almost every side effect traces back to one of those receptors.

Receptor Where it sits What it does The side effect it explains
MC1R Pigment cells (melanocytes) in the skin Shifts production toward eumelanin, the brown-black pigment of a tan Darker moles and freckles, uneven early pigmentation
MC4R Hypothalamus and spinal cord Initiates erections centrally and is linked to sexual desire Spontaneous erections in men, a change in desire
MC3R and MC4R Hypothalamus Part of the body's satiety system Reduced appetite
The melanocortin system broadly Brain A recognised effect seen in animals decades ago The stretching and yawning complex

That is why the side-effect list is longer than for a skin-only compound. It is also why taking Melanotan II for libido means accepting skin changes, and why taking it for a tan comes with the erection and appetite effects.

What the human studies recorded

Melanotan II has a better-characterised side-effect profile than most compounded peptides, because its human studies recorded side effects systematically. Those studies are small and from one research group in the 1990s: a three-person pigmentation pilot in Life Sciences in 1996, two ten-person, double-blind, placebo-controlled crossover studies in men with erectile dysfunction (Journal of Urology in 1998 and Urology in 2000), and a pooled review in the International Journal of Impotence Research in 2000. The consistent findings across them:

  • Nausea. Reported at most dose levels, and severe after a minority of doses at the higher study dose.
  • Facial flushing. Common, often with a feeling of warmth.
  • Stretching and yawning. Every study noted it, typically before the erections began. It is transient and dose-related, and it is alarming only if you are not expecting it.
  • Reduced appetite. A documented effect of the melanocortin system, recorded on the questionnaires. No study has looked at Melanotan II for appetite or weight, and it is not prescribed for that.
  • Sleepiness. Reported at the top of the dose escalation.
  • Spontaneous erections in men. Erections began without sexual stimulation and came and went for one to five hours after a dose.

The study authors described these effects as manageable. None of those studies tested repeated dosing over weeks or months, which is how Melanotan II is prescribed, so the long-run pattern comes from clinical experience rather than trials.

When they tend to appear

These are patterns described by prescribers, not trial endpoints, and they vary from person to person.

Point in treatment What is typical
The hour or two after an injection Nausea, flushing, warmth, the stretching and yawning sensation; usually settling the same day
The hours after a dose, in men Spontaneous erections, on and off
The first week Mostly side effects rather than colour
Weeks one to four Pigment showing first where pigment already is: moles, freckles, the face, the upper body
Repeated dosing Prescribers describe the early effects as fading
After the vial No withdrawal described; colour fades over weeks as skin cells turn over

If nausea is a problem, tell your care team. Evening injection is a change prescribers commonly make for nausea, but it is a change to your directions, so it comes from your physician and appears on your vial. Never adjust the dose or timing yourself.

Melanotan II Side Effects

Melanotan II and your moles

This is the section that matters most.

  • What is expected

Melanotan II darkens existing moles and freckles. That is the pigment pathway doing what it does, and a mole that is simply darker is not by itself a warning sign. It is the reason your skin is examined before you start: you need a baseline to tell an expected darkening from a real change.

  • What the case reports describe

Case reports describe new moles appearing and existing ones changing during use. A small number, including reports in Dermatology in 2014 and the Australasian Journal of Dermatology in 2012, describe melanoma diagnosed during or shortly after a course. A 2017 review of the alpha-MSH analogue literature in the International Journal of Dermatology concluded that conclusive evidence of a causal link is lacking, but that the reports warrant caution and skin surveillance.

So the honest position is that nobody has shown Melanotan II causes melanoma, and nobody has shown it does not. It has not been studied over years, and it does not establish any change in melanoma risk in either direction.

  • What to watch for

Change What it means What to do
A mole or freckle that is darker but otherwise unchanged Expected pigment effect Note it at your regular skin checks
A mole that changes shape, border or texture Not explained by darkening Have it looked at promptly
A new pigmented spot Needs a look Have it looked at promptly

Check your skin regularly for as long as you are using it. It is the single most important practical point about Melanotan II. A darker tone is not sun protection: Melanotan II is not a sunscreen, the research does not describe it preventing sunburn or skin cancer, and nothing about it is a reason to use sunbeds or skip sunscreen.

Serious side effects: what the FDA keeps on file

Melanotan II was placed in Category 2 of the FDA's 503A bulk drug substances list in 2023, the category for substances flagged for significant safety concerns. It is no longer in Category 2: the FDA's current three-category list, updated 14 May 2026, does not include it there. The FDA's safety-risk page, current as of 22 April 2026, lists Melanotan II among substances previously in Category 2 and keeps its original note on file: a possible immunogenicity risk from aggregation or peptide impurities, and published case reports of melanoma, posterior reversible encephalopathy syndrome, sympathomimetic toxidrome and priapism.

Context matters for those reports. The toxicity case in Clinical Toxicology in 2012 involved a single self-administered dose of product bought online, many times larger than any reviewed direction. The priapism case in BMJ Case Reports in 2019 likewise involved self-sourced product. They are real events, and they are the reason the dose is set by a physician, printed on the vial, and drawn from a vial of known concentration.

The FDA has announced that its Pharmacy Compounding Advisory Committee will meet before the end of February 2027 to discuss Melanotan II for possible inclusion on the 503A bulks list, alongside LL-37, GHK-Cu, Dihexa and PEG-MGF. It remains legal to prescribe and dispense with a valid prescription, and it is not an FDA-approved drug.

Symptoms that need emergency care or a same-day call

Symptom What it may be What to do
An erection lasting more than four hours Priapism Emergency care: call 911 or go to an emergency department
Severe headache with visual change or confusion Needs urgent assessment Call 911
A racing heart with agitation and muscle pain Possible sympathomimetic toxicity Call 911
Swelling of the face, lips or throat, widespread hives, trouble breathing Serious allergic reaction Call 911 first, then tell your care team
You think you injected far more than your directions Possible overdose Call Poison Control at 1-800-222-1222 and message your care team
An injection site that is hot, spreading or worse after 48 hours Possible infection rather than irritation Message your care team the same day
Nausea that stops you eating or drinking properly More than the usual short-lived effect Message your care team the same day

Melanotan II Side Effects

Injection-site reactions

Melanotan II is injected under the skin into the abdomen or thigh, rotating sites. Because the signal is systemic, where you inject does not decide where you tan. Redness, mild soreness or a small lump that appears as you inject and settles within a day or two is the tissue responding to a needle and a volume of fluid. A lump that appears days later and is warm or spreading is different and needs a same-day message. Technique is covered in how to inject peptides safely. Use a fresh sterile syringe every time; Pepti ships sterile syringes and alcohol prep pads with every vial, and the vial ships refrigerated with ice packs.

When to message your care team

Message your care team, rather than waiting for your refill, if:

  • nausea is stopping you eating properly or has not eased with repeated doses
  • flushing, sleepiness or yawning is interfering with work or driving
  • any mole changes shape, border or texture, or a new pigmented spot appears
  • erections are frequent enough to be a problem, even if none has lasted four hours
  • you start any new medication, particularly anything for erectile dysfunction or blood pressure

There is no established drug-interaction list for Melanotan II, because the trials that would produce one have not been run. That is why the intake asks for every medication you take and why a physician reviews it.

Melanotan II Side Effects

Who should not take Melanotan II

Situation Status
Personal history of melanoma or atypical moles A stop, not a caveat
Existing pigmented lesions Examined before starting
Very fair skin that burns rather than tans Your physician may judge the balance is not there
Pregnancy or breastfeeding Not used; safety data is absent
Taking medication for erectile dysfunction or blood pressure Must be on your intake
Tested athletes Check your sport's current prohibited list
Under 18 Pepti prescribes to adults only

Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account.

Melanotan II Side Effects

Melanotan II vs PT-141 side effects

They overlap, because both act on the melanocortin family. PT-141 (bremelanotide) is a metabolite of Melanotan II developed for sexual desire. In its phase 3 trials nausea was reported by 40% of women against 1.3% on placebo, with flushing, injection-site reactions, headache and a transient blood-pressure rise after each dose, and focal skin darkening in about 1% at capped, as-needed dosing. Melanotan II adds yawning, spontaneous erections, appetite changes and the mole darkening that comes with tanning. Taking both stacks the same side effects, and no study has tested the combination. The full comparison is in PT-141 vs Melanotan II.

Melanotan II Side Effects

What the evidence shows, honestly

Melanotan II's short-term side effects are documented in randomised, placebo-controlled human studies, which is more than most compounded peptides have. The limits are real: the studies are small, from one group, used single doses or short courses, and did not follow anyone over repeated courses. Nothing establishes long-term safety, and the relationship between Melanotan II and melanoma is unresolved. The broader context is in are peptides safe long term and peptide side effects: what to watch for.

Where to get Melanotan II prescribed

Melanotan II cannot be bought legitimately without a prescription. Products sold "for research use only" have no accountable pharmacy behind their identity, purity, sterility or concentration, and the most serious case reports involved exactly that kind of product. The prescription route:

  1. Complete a free online intake covering your history, medications, skin history and what you want help with.
  2. A physician licensed in your state reviews it, usually within 24 hours.
  3. If it is appropriate, a PCAB-accredited, FDA-registered US 503A compounding pharmacy prepares it to that prescription. Lot test results are published at /quality/lab-results.
  4. It ships refrigerated with ice packs, with sterile syringes and alcohol prep pads, and your directions printed on the vial.
  5. Your care team stays reachable for side effects and questions. Refills run every 28 days, one vial per fill.

Pepti is cash-pay and does not bill insurance, and prescribes Melanotan II in every state except Oklahoma, plus DC The step-by-step page is how to get Melanotan II prescribed online.
