How Long Does Tirzepatide Stay in Your System? Half-Life, and Semaglutide Too
This page explains what half-life means, what the labels say for each medication, what that looks like week by week, and why it matters for missed doses, switching, procedures, pregnancy planning and drug tests.

Treatments mentioned
Tirzepatide has an elimination half-life of about five days, or five to six days in people with overweight or obesity, according to the approved labels, so roughly half of a dose is still present a week after the injection. Semaglutide lasts longer: its approved weight-management label gives a half-life of about one week and states it is present in the circulation for about five to seven weeks after the last dose. Neither stops working on the day you stop; levels slide over weeks rather than drop, and most people describe appetite returning over the second to fourth week after the last injection. Your own timing questions, such as before surgery or a planned pregnancy, are worth raising with your care team.

How Long Does Tirzepatide Stay in Your System? Half-Life, and Semaglutide Too
The short answer
| Tirzepatide | Semaglutide | |
|---|---|---|
| Elimination half-life | About 5 days (type 2 diabetes label); about 5 to 6 days in overweight or obesity (weight-management label) | About 1 week |
| Peak level after an injection | About 24 hours | Not stated on Pepti's reference pages |
| Time to steady state on weekly dosing | About 4 weeks | Not stated on Pepti's reference pages |
| How long it remains after the last dose | No figure stated on the labels as quoted on Pepti's pages | About 5 to 7 weeks after the last dose (weight-management label); about 5 weeks (type 2 diabetes label) |
| Pre-pregnancy stopping interval on the label | None stated | At least 2 months before a planned pregnancy |
| At Pepti | Tirzepatide, from $159/mo | Semaglutide, from $99/mo |
Those figures come from the labels of the approved products. Compounded tirzepatide and compounded semaglutide contain the same molecules, so the same pharmacokinetics apply, but a compounded preparation is not FDA approved and carries no label of its own. Every Pepti price is all-in, covering medication, physician review, refill management and shipping.

What half-life means
Half-life is the time it takes for the amount of a drug in your circulation to fall by half. After one half-life, half remains; after two, a quarter; after three, an eighth. A drug with a half-life measured in days is still substantially present a week later, which is what makes weekly dosing possible.
Both molecules were engineered for that. Native GLP-1 is cleared within minutes. Tirzepatide carries a C20 fatty diacid side chain that lets it bind albumin, the main protein in blood, and it is about 99 percent albumin-bound; for semaglutide, the label describes albumin binding as the principal mechanism that slows its clearance and protects it from breakdown. That is the whole reason both are injected once a week rather than every day.

How the half-life plays out, week by week
The arithmetic below shows how much of a single dose remains after each half-life. It is math, not a label figure, and real levels after regular weekly dosing are higher because doses overlap.
| Half-lives passed | Share of one dose remaining | Tirzepatide, at about 5 days each | Semaglutide, at about 7 days each |
|---|---|---|---|
| 1 | 50% | About day 5 | About day 7 |
| 2 | 25% | About day 10 | About day 14 |
| 3 | 12.5% | About day 15 | About day 21 |
| 4 | About 6% | About day 20 | About day 28 |
| 5 | About 3% | About day 25 | About day 35 |
A common pharmacology rule of thumb is that a drug is largely cleared after about four to five half-lives. For semaglutide that lands at roughly four to five weeks for a single dose, consistent with the type 2 diabetes label's figure of about five weeks, and the weight-management label's longer range of five to seven weeks after the last dose. For tirzepatide, the same rule of thumb gives roughly three to four weeks after the last dose. That tirzepatide figure is arithmetic from the half-life, not a number stated on the label, and individual clearance varies.
Steady state
On regular weekly dosing, a medication with a multi-day half-life builds up over several weeks before what goes in each week equals what clears. The tirzepatide weight-management label reports that steady-state levels were reached after 4 weeks of once-weekly dosing, which matches its escalation schedule of at least four weeks on each step. Pepti's 28-day refill, one vial per fill, maps onto the same four-week block.

What happens after the last dose
Nothing happens on day one. The first skipped week sits inside the half-life of the dose before it, so the level slides rather than drops. There is no withdrawal syndrome. As the medication clears, its effects fade and the body's own signalling returns to baseline:
| Effect | What tends to happen as levels fall |
|---|---|
| Appetite | Most people describe it returning over the second to fourth week after the last injection; a pattern, not a fixed timeline |
| Stomach emptying | Speeds up over the same window |
| Side effects | Taper over weeks rather than hours |
| Weight | The withdrawal trials, STEP 4 (JAMA, 2021) for semaglutide and SURMOUNT-4 (JAMA, 2024) for tirzepatide, reported regain in people switched to placebo |
Because the medication persists, a stop can feel as though it worked for a while, and then it does not. The longer-term picture is in what happens when you stop taking peptides. Stopping is something to plan with your care team rather than do quietly.

Why it matters
Missed doses
The half-life is why one missed week is a dip, not a cliff. The approved tirzepatide labels say a missed dose can be taken as soon as possible within 4 days (96 hours); past 4 days, skip it and take the next dose on the usual day. Four days is under one half-life. The semaglutide weight-management label works from the other direction: take a missed dose if the next scheduled one is more than 2 days away, and skip it if it is less. With semaglutide's longer half-life, two injections landing too close together is the larger risk. Never take two doses to catch up. The detail is in what if I miss a dose of tirzepatide.
Switching between them
When someone switches from semaglutide to tirzepatide, semaglutide is still in the system for weeks. Both act at the GLP-1 receptor, and both approved labels state that coadministration with another GLP-1 receptor agonist is not recommended. That is why a switch is a clean handover on the weekly schedule, handled through your physician, rather than an overlap. See switching from semaglutide to tirzepatide.
Surgery and anaesthesia
Both medications slow stomach emptying, and the long half-life means that effect persists well past the last dose. The approved labels carry a warning about rare reports of pulmonary aspiration under general anaesthesia or deep sedation. Tell every surgeon and anaesthesiologist that you take a GLP-1 medication, the dose, and the date of your last injection. Whether to hold a dose beforehand is worked out by the anaesthesiologist with your prescriber, so raise it as early as you can.
Planning a pregnancy
Neither medication is used in pregnancy. The semaglutide label directs stopping at least two months before a planned pregnancy, to account for its long half-life: the five-to-seven-week washout plus a margin. The tirzepatide label publishes no pre-pregnancy interval, so the timing for tirzepatide is set with your physician for your situation. Weight reduction can restore ovulation, so tell your care team early if you are planning a pregnancy. More is in do peptides affect fertility and can you take peptides while pregnant.
Birth control pills
Separately from clearance, tirzepatide slows stomach emptying most after the first dose and after each increase, which can reduce absorption of oral contraceptives. The label advises a non-oral or backup method for four weeks after starting and four weeks after each dose increase.
Drug tests
Tirzepatide and semaglutide are not something employment drug panels look for, and as larger peptides they have no structural relationship to the drugs those tests target. In sport, GLP-1 medications are not currently listed as prohibited, but WADA's 2026 and 2027 Monitoring Programs include semaglutide and tirzepatide, so laboratories report prevalence data. Lists change annually, and athletes should check the current one. See do peptides show up on a drug test.

When to contact your care team, and when to call 911
How long the medication lasts also means a side effect can take time to settle after a dose. Contact your care team the same day for persistent vomiting, being unable to keep fluids down, or passing much less urine than usual. Seek urgent care for severe abdominal pain, especially radiating to the back, which can be pancreatitis, and for right upper abdominal pain with fever or yellowing skin. Call 911 for facial or throat swelling, difficulty breathing, or any symptom that feels like an emergency. If you think you have taken too much, call Poison Control at 1-800-222-1222 and message your care team.

What the evidence shows, honestly
The half-life figures are well established: they come from the approved labels of the manufactured products and from published pharmacokinetic studies, including a 2024 population pharmacokinetic analysis of tirzepatide in CPT: Pharmacometrics & Systems Pharmacology and a 2024 systematic review of semaglutide pharmacokinetics in Drug Design, Development and Therapy. Compounded tirzepatide and semaglutide contain the same molecules, but compounded medications are not FDA approved and have no labels of their own.
What is less precise is any individual's experience. The week-by-week table is arithmetic for a single dose. How quickly appetite returns after stopping is described by patients, not measured in a trial of stopping alone, and nobody can give you an exact date the medication will be fully gone.

Common questions
How Long Does Tirzepatide Stay in Your System? Half-Life, and Semaglutide Too, answered.
Its elimination half-life is about five days, or five to six days in people with overweight or obesity, on the approved labels. About half a dose remains after a week. Applying the common four-to-five half-life rule of thumb gives roughly three to four weeks after the last dose, though that is arithmetic rather than a label figure.
About 5 days on the type 2 diabetes label, and about 5 to 6 days in people with overweight or obesity on the weight-management label. Peak level comes about 24 hours after an injection.
The approved weight-management label gives a half-life of about one week and says semaglutide is present in the circulation for about five to seven weeks after the last dose. The type 2 diabetes label says about five weeks.
Semaglutide. Its half-life is about a week, against about five days for tirzepatide.
Most people describe appetite returning over the second to fourth week after the last injection, as the level falls. That is a pattern people describe, not a fixed timeline.
No withdrawal syndrome has been described. Effects fade as the medication clears, and appetite and stomach emptying return toward baseline over a few weeks.
Tell your surgeon and anaesthesiologist that you take it and when you last injected. The labels warn about rare aspiration under anaesthesia because of slowed stomach emptying. Whether to hold a dose is worked out by the anaesthesiologist with your prescriber.
For semaglutide, the label says at least two months before a planned pregnancy. The tirzepatide label gives no interval, so the timing is set with your physician. Tell your care team early.
Not on an employment panel. In sport, it is not currently prohibited but is on WADA's Monitoring Program, and lists change annually.
Partly. Because of the five-day half-life, a missed week is a dip in levels rather than a stop. The label window for a missed dose is 4 days (96 hours); past that, skip it and take the next dose on your usual day. Never double up.
No. Bloodwork is not required for any Pepti treatment. If your physician wants labs for your care, they'll ask through your account. The free assessment goes to a physician licensed in your state. A consultation does not guarantee a prescription.
