How Long Does Tesamorelin Take to Work? What the Trials Measured, and When
This page goes trial by trial through how long each ran and what it measured, explains why weight and how you feel are the wrong clocks, and covers your directions and what to track.

Treatments mentioned
Tesamorelin has the most specific timing evidence of any growth-hormone-axis peptide: its phase 3 trials measured visceral fat on CT after 26 weeks of daily 2 mg injections in adults with HIV and abdominal fat, and reported a reduction against placebo at that point. Other trials ran 20 weeks, six months and 12 months, and the approved product's label advises weighing whether to continue in anyone without a reduction in visceral fat by six months. Those are trial endpoints in named populations, not a promise of when anything will change for you, and Pepti's typical dose is a quarter of the main trial dose. Compounded tesamorelin is not FDA approved.

The short answer
| Trial | Who | How long it ran | Dose | What it measured, and when |
|---|---|---|---|---|
| New England Journal of Medicine, 2007 | 412 people with HIV and abdominal fat | 26 weeks | 2 mg daily | Visceral fat on CT fell 15.2% against a 5.0% rise on placebo; IGF-1 rose 81% |
| Pooled phase 3 analysis, Journal of Clinical Endocrinology and Metabolism, 2010 | 806 randomised patients across two trials | 26 weeks, with an extension to 52 | 2 mg daily | Reduction at week 26, maintained to week 52 on treatment; returned toward baseline in those switched to placebo |
| Archives of Neurology, 2012 | 152 adults aged 55 to 87 | 20 weeks | 1 mg at bedtime | Cognitive testing at weeks 10 and 20; IGF-1 rose 117% |
| Journal of Clinical Endocrinology and Metabolism, 2012 | 60 abdominally obese adults with reduced growth hormone secretion, without HIV | 12 months | 2 mg daily | Visceral fat fell 16 cm² against a 19 cm² rise on placebo |
| JAMA, 2014 | 50 people with HIV and abdominal fat | Six months | 2 mg daily | Visceral fat and liver fat |
| Lancet HIV, 2019 | 61 people with HIV and fatty liver | 12 months | 2 mg daily | Liver fat fraction |
The shortest trial with an imaging endpoint ran 20 weeks, and the main ones ran 26. Nobody designed a tesamorelin trial expecting an answer at week four.
How Long Does Tesamorelin Take to Work? What the Trials Measured, and When
Why the trials are measured in months
The endpoint was imaging, not the scale
The phase 3 trials measured visceral fat, the fat inside the abdomen around the organs, on CT. Growth hormone signals fat cells to release stored triglyceride, and visceral fat is particularly responsive, which is the described reason every trial reported a reduction on CT with no significant change in the fat under the skin. Visceral fat is not something you can see or weigh, and the approved label describes tesamorelin as weight-neutral. Judging tesamorelin by the scale measures something its trials did not treat as an endpoint at any point.
IGF-1 moves before anything structural
Tesamorelin is the full 44-amino-acid growth-hormone-releasing hormone sequence with a stabilising cap, and it prompts your own pituitary to release growth hormone, which works largely through IGF-1 made in the liver. IGF-1 rose 81% over 26 weeks in the 2007 trial and 117% in the 2012 cognitive trial, staying within the physiological range. A blood marker is the earliest objective sign the signal is being received. It is not the same as visceral fat having changed.
The cognitive trial built in a week-10 look
The Archives of Neurology trial tested cognition at weeks 10 and 20 and reported a favourable effect on a composite, driven mainly by executive function, at the end. Adverse events were mild but more frequent on the drug. The week-10 checkpoint was there because nobody expected an answer sooner. It is one trial whose authors called for longer ones.
Stopping reverses it
The pooled extension is the part most pages leave out. People kept on tesamorelin to 52 weeks maintained the visceral fat reduction; in those switched to placebo, visceral fat returned toward baseline. So the honest answer to "how long" includes "for as long as it is taken", which is the pattern of most metabolic therapies. See what happens when you stop taking peptides.
How Long Does Tesamorelin Take to Work? What the Trials Measured, and When
The compounded dose is not the trial dose
| What the trials did | What that means for a compounded vial |
|---|---|
| Used 1 mg or 2 mg daily | Pepti's typical directions deliver 0.5 mg on weekday nights, a quarter of the main trial dose; any timing at that dose is an extrapolation |
| Enrolled mostly people with HIV | Only one trial, of 60 people, tested it outside HIV |
| Measured visceral fat on CT | No trial measured scale weight loss as a goal |
| Ran 12 months at most | No long-term outcome data |
| Enrolled people with abdominal fat or low growth hormone | No trial has enrolled healthy adults with normal growth hormone secretion |
Sleep, recovery and body composition beyond visceral fat have not been measured as tesamorelin outcomes. People commonly describe sleep as the first thing they notice, because the dose is given at bedtime alongside the body's largest natural growth hormone pulse; that is a report consistent with the mechanism, not a trial finding with a date.

What your directions look like
| What the label shows | Value |
|---|---|
| Product | Tesamorelin, 5 mL multi-dose vial at 2 mg/mL (10 mg per vial) |
| Typical directions | Inject 0.25 mL (25 units) subcutaneously Monday through Friday nights at bedtime |
| Tesamorelin per dose | 0.5 mg (2 mg/mL x 0.25 mL) |
| Doses per vial | 20, about four weeks |
| Price | $249/mo, all-in |
| Pairings | Tesamorelin + Ipamorelin, $259/mo; PHYSIQ, $289/mo |
A blend containing tesamorelin does not reach an imaging endpoint sooner than tesamorelin alone; no trial has tested the blends, and the 26-week figure comes from tesamorelin either way. Refills run every 28 days, one vial per fill, and vials ship ready to use, refrigerated with ice packs, with sterile syringes and alcohol prep pads. Your physician sets your directions after reviewing your intake, and they are printed on the vial. The conversion chart is in tesamorelin dosage.

What to track, and when to message your care team
| Keep a note of | Why |
|---|---|
| Waist measurement on a fixed day each month, same tape, same point | A rough proxy; visceral fat itself needs imaging |
| A short morning sleep note | Sleep is the first thing many people describe, and memory is unreliable |
| Training and diet | Body composition depends on them more than on any peptide |
| Puffy hands or ankles, joint aches, tingling or numbness | Classic growth hormone effects that usually mean the dose is above what you need |
| Thirst, frequent urination, or a raised home glucose reading | Growth hormone reduces insulin sensitivity; the label records new-onset diabetes in 5% on treatment against 1% on placebo, at 2 mg daily |
Message your care team about fluid retention, joint aches or tingling rather than pushing through, and never change the dose yourself. Facial or throat swelling, spreading hives or difficulty breathing is an emergency: call 911. If you take insulin or another glucose-lowering medication, make sure it is on your intake. The full list is in tesamorelin side effects.
The six-month point on the approved label is a reasonable moment for a conversation with your care team about how things are going, using what you have recorded. It is not a promise that anything will have changed by then.

What the evidence shows, honestly
Tesamorelin has randomised, double-blind, placebo-controlled human trials, including two phase 3 trials that supported an FDA approval in November 2010 for one narrow use: reducing excess abdominal fat in adults with HIV who have lipodystrophy. That is a materially stronger and more time-specific evidence base than any other growth-hormone secretagogue prescribed as a compound. Compounded tesamorelin shares the active peptide but is not that product, and compounded medications are not FDA approved. The trial results above belong to the approved product at its approved doses.
Tested athletes should know the 2026 WADA Prohibited List names tesamorelin under S2.2.4, prohibited at all times. The benefit-by-benefit evidence is in tesamorelin benefits, and the full reference is what is tesamorelin. How it compares with a GLP-1 on timing and endpoints is in tesamorelin vs semaglutide.

Common questions
How Long Does Tesamorelin Take to Work? What the Trials Measured, and When, answered.
The phase 3 trials measured visceral fat on CT at 26 weeks of 2 mg daily, in adults with HIV and abdominal fat, and reported a reduction at that point. There is no trial-established timeline at the compounded dose, and nobody can promise one.
The trials measured visceral fat, the deep fat around the organs, at 26 weeks and later, with no significant change in the fat under the skin. That is an imaging endpoint, not a visible change anyone can schedule.
At 26 weeks in the phase 3 trials, with an extension to 52; at six and 12 months in the liver-fat and non-HIV trials; and at weeks 10 and 20 in the cognitive trial.
The approved label describes it as weight-neutral and not indicated for weight-loss management. The trials measured visceral fat on imaging, not kilograms.
In the trial extension, visceral fat returned toward baseline in people switched to placebo, while those who continued maintained the reduction to 52 weeks.
Not something to try. Take the dose printed on your vial, on the schedule printed on your vial. Fluid retention, joint aches and tingling are signs of growth hormone above what a person needs.
No trial has tested them. A blend addresses more than one pathway, which is a different claim from working faster.
About four weeks: 20 doses of 0.25 mL, Monday through Friday nights at bedtime.
An approved tesamorelin product exists for excess abdominal fat in adults with HIV-associated lipodystrophy. Compounded tesamorelin is not FDA approved.
