How Long Does PT-141 Take to Work? Timing per Dose, and What the Trials Measured
This page sets out what is known about PT-141 timing per dose, what the trials measured over weeks, why the two are different questions, how PT-141 is taken at Pepti, and what to do if a dose does not seem to work. For the molecule in full, read what is PT-141.

Treatments mentioned
PT-141 is a per-dose medication, so "how long does it take" is about hours before a sexual occasion, not weeks of building up, and nobody can promise how any one person will respond. The approved product containing bremelanotide, the molecule in PT-141, reports peak blood levels about an hour after a dose and a half-life of about 2.7 hours, and its label directs dosing at least 45 minutes before anticipated sexual activity. Pepti's typical directions say 2 to 4 hours before. The phase 3 trials ran for 24 weeks and measured desire and distress scores across that period, with as-needed use; the label states that how long the effect lasts has not been fully characterised.

How Long Does PT-141 Take to Work? Timing per Dose, and What the Trials Measured
The short answer
| Source | What was measured | Timing |
|---|---|---|
| Approved bremelanotide label | Peak blood level after a dose | About 1 hour |
| Approved bremelanotide label | Half-life | About 2.7 hours |
| Approved bremelanotide label | When to dose | At least 45 minutes before anticipated sexual activity |
| Approved bremelanotide label | How long the effect lasts | Not fully characterised |
| Approved bremelanotide label | Blood pressure rise per dose | Peaks two to four hours after a dose, usually back to baseline within twelve |
| Pepti's typical directions | When to dose | 2 to 4 hours prior to sexual activity |
| RECONNECT phase 3 trials, Obstetrics & Gynecology 2019 | Desire and distress scores in premenopausal women with HSDD | 24 weeks of as-needed use |
| 2004 studies, International Journal of Impotence Research | Erectile response in men | Measured over hours after a dose |
The per-dose numbers describe the drug's behaviour in the body. The 24-week trials describe average score changes in a specific population. Neither tells you what you will notice, or on which occasion.
How Long Does PT-141 Take to Work? Timing per Dose, and What the Trials Measured
Two different questions: per dose, and over weeks
| Question | What answers it | What it does not tell you |
|---|---|---|
| How far ahead should a dose be taken? | The label's peak level, half-life and 45-minute instruction, and your printed directions | Whether a given dose will have a noticeable effect |
| Does it help over time? | The 24-week phase 3 trials, which averaged score changes across many occasions | How a single person's first few uses will go |
Unlike the weekday peptides on Pepti's site, PT-141 does not accumulate. It is used on the occasion, no more than once in 24 hours, and judged over a handful of uses rather than a calendar.

What the trials measured, and over what period
Phase 3: 24 weeks in premenopausal women
RECONNECT was two identical 24-week, randomised, double-blind, placebo-controlled trials in 1,267 premenopausal women with acquired, generalised hypoactive sexual desire disorder, using 1.75 mg bremelanotide or placebo, self-injected as needed. On the desire domain of the Female Sexual Function Index, scores improved by 0.30 and 0.42 points more than placebo, and distress improved by 0.37 and 0.29 points more than placebo, all statistically significant and modest in absolute size. A 52-week open-label extension followed 684 women who chose to continue; most used it two to three times a month, and 272 completed the full year.
These trials excluded women whose low desire was caused by a medication, a relationship problem or another condition, did not include postmenopausal women, and used a fixed 1.75 mg autoinjector rather than a compounded vial. Detail is in PT-141 benefits.
Phase 2b: dose-finding
A 2016 Women's Health trial randomised premenopausal women to placebo or one of three doses, used as desired. In the 327 women analysed, the pooled 1.25 and 1.75 mg group reported more satisfying sexual events per month than placebo, +0.7 against +0.2.
Men: hours after a dose
Two 2004 papers in the International Journal of Impotence Research used double-blind, placebo-controlled designs in healthy men and in men with mild-to-moderate erectile dysfunction, and reported erectile responses to PT-141 measured over hours, including without visual sexual stimulation. These were small, early-phase studies. There is no phase 3 trial in men, and prescribing PT-141 to men is off-label use of an approved active ingredient.
How Long Does PT-141 Take to Work? Timing per Dose, and What the Trials Measured
Why the timing works the way it does
PT-141 acts on melanocortin receptors in the brain, with MC4R the one implicated in sexual function. It is not a PDE5 inhibitor and does not act on blood flow, and it does not change testosterone or oestrogen. The proposed model, from animal work, is increased dopamine release in a hypothalamic region associated with sexual motivation, affecting the "wanting" side rather than the physical response. The approved label states the mechanism by which it improves desire is unknown. Because it acts centrally and is cleared within hours, each dose is its own event.

If a dose did not seem to work
| What to do | Why |
|---|---|
| Do not take a second dose within 24 hours | The approved label states consecutive doses within 24 hours have not been shown to add anything and may add to the blood pressure effect |
| Stay within the monthly limit on your label | The ceiling exists because each dose raises blood pressure for hours and frequent dosing raises the risk of skin darkening |
| Check the timing against your directions | Your label says which window applies to you |
| Message your care team | Questions about whether it fits are part of physician review; do not change your own dose |
| Keep a short note of each use | PT-141 is judged over several uses, and a record helps that conversation |
Nausea is the most common side effect, reported by 40% of women in the phase 3 trials against 1.3% on placebo, followed by flushing, injection-site reactions and headache, in the same window as the intended effect. It should not be used with oral naltrexone, with uncontrolled hypertension or with heart disease. Call 911 or go to an emergency department for chest pain, a severe headache, trouble breathing, or an erection lasting more than four hours, then tell your physician. The full list is in PT-141 side effects.

What the evidence does not establish
- How long the effect of a dose lasts; the label says it has not been fully characterised.
- Efficacy in men beyond early-phase erectile-response studies, or in postmenopausal women.
- Anything about the compounded 2 mg/mL vial specifically, or the PT-141 + Oxytocin combination, which has never been studied as a combination.
- Any timeline for how a given person will respond.
The active ingredient, bremelanotide, is the basis of an FDA-approved drug, approved on 21 June 2019 for premenopausal women with HSDD, which is why compounded PT-141 does not depend on the FDA's 503A bulks list. The compounded vial is a different formulation and is not FDA approved; compounded medications are not FDA approved. Bremelanotide is not named on the 2026 WADA Prohibited List. The cross-category picture is in how long do peptides take to work.

Common questions
How Long Does PT-141 Take to Work? Timing per Dose, and What the Trials Measured, answered.
It is used per dose rather than built up over weeks. The approved bremelanotide label reports peak blood levels about an hour after a dose and directs dosing at least 45 minutes before activity; Pepti's typical directions say 2 to 4 hours before. Whether and how much an individual notices on a given occasion varies.
Follow the window printed on your vial. Pepti's typical directions are 2 to 4 hours prior to sexual activity.
The approved label reports a half-life of about 2.7 hours and states that how long the effect lasts has not been fully characterised. The blood pressure rise from a dose peaks two to four hours after it and usually returns to baseline within twelve.
No study can promise that for any one person. The phase 3 trials measured average changes over 24 weeks of as-needed use, and PT-141 is judged over several uses. If you have questions after a few, message your care team.
No. It is used on the occasion, never more than once in 24 hours, and no more than the monthly number on your label: 4 doses a month on Pepti's typical PT-141 directions.
No. The approved label states that consecutive doses within 24 hours have not been shown to add anything and may add to the blood pressure effect. Message your care team instead.
The two phase 3 RECONNECT trials ran for 24 weeks, with a 52-week open-label extension. The men's studies were early-phase and measured erectile response over hours after a dose.
The 2004 studies measured erectile responses over hours after a dose in small, early-phase trials. There is no phase 3 trial in men.
Nausea, flushing and headache arrive in the same window as the intended effect, after a dose. The blood pressure rise peaks two to four hours after a dose.
